Six month insurance requires medical weight loss

I was just at my first physicians appointment. They did my blood work and put me on a half assed diet plan and to exercise 3 times a week and to set a goal of 3 lbs. month weight loss. Lets say I do that. My bmi is like 42. Something. My bmi will most likely be below 40 which is the lowest required bmi my insurance requires. Will this knock me out of my surgery?
