Im just at the point that I do not know what to do. Tricare keeps denying me. Ive done everything they tell me to do! Now I need a physician supervised diet, but yet the base wont do it. My ob gyn will do it but they cant get the form from Tricare. DOES anyone HAVE THE FORM TRICARE REQUIRES???? please I am asking for help

Carla, there is no form that I know of... I went to my doctor on base and told him I needed 6 months of supervised diet and exercise..he documented my weight that day, what I was eating and what exercise I was doing..I did the same thing for the next 5 months...I'm almost certain there is no form...can I ask where you heard this from?

I don't have Tricare, but my PCP did it the same way as Karen's. Then he attached a letter of necessity.

I had the 6 month diet with my surgeon's office. i did not have Tricare. My son's insurance made him see the doctor at 1 month intervals, his apt could not be any earlier than 1 month apart. So, just incase, make sure that yours are not 28 or 29 days apart. Good luck, Karen

thanks yall. I know if I give up I will go on to develop diabetes. I literally watch my dad die of that horrible disease and now my 26 yr old has had it for 11 years. im asking for help but they are slamming the door in my face

Yes! I was denied the first time because I had one month that was 29 days apart. The hoops..the hoops.

I wonder if you post "tricare" as a subject line that perhaps someone will happen to work there who is a member of lapbandtalk and be able to help? I know this is so frustrating for you. Trying to deal with insurance was one of the most difficult things about this.

The doctor that I had do it for me was at a weight loss center.. Do you have any of those in your network? Mine was literally at a place called "physicians weight loss center."

why did tricare deny you?

once i found a dr who accepted tricare

i had tricare approve me a visit to the surgeon

the surgeon gave me a list of things i needed to do (pre ops)

once completed, they submitted for tricare to approve

tricare needs to have proof of past dieting attempts

100+ pounds overweight

and some sort of medical issues, blood pressure , sleep apnea etc..

do you have all the things that tricare says you need?
