Hi Mike I have Anthem BCBS and I was approved yesterday and all we submitted was letter of medical necessity,lab work, Psych and Nutritional Evaluations, BMI, and non-surgical weight reduction documented attempts (i used WW, nutrisystem, etc)..........hope this helps! BTW it took only two days for my approval Anthem BCBS is the best by far!!

Congrats.........I found out yesterday that I was approved as well......Life is Great right now isn't . Who is performing your surgery? I live in Fort Worth and my Dr. is Dr. Nick Nicholson in Plano.

Hi everyone this is my first post but I have been reading everyone's information and feel like I know some of you:biggrin2:. Anyway, I started this process a month ago (May 21 I think) and had my papers submitted to insurance on Thursday (June 26) and OMG I just received my call from the insurance (Anthem BC/BS) and the doctors back to back. I am soooo stoked, I can't believe how fast it was. I was thinking at least six months or so. Just wanted to let everyone know the good news..............Appt. with Doc on the 17th to set a surgery date.......

Hi I live in fort worth as well and I am a African American female trying to get the lapband procedure. I am not sure what documents they are looking for. How did you get you five year history? I have done weight watchers, herbalife, FIG, medifast and all but I am not sure they will take this kind of documentation?? Can you give me any advice to get approved. savardasnell@hotmail.com

I am with BCBS of Texas and I need to get five year history of obesity and I am not sure how i should go about getting it? Please help

I am trying to get approve but I don't understand what is considered five year obesity history. From a doctor or what???
