Anyone not have to complete 6 month medically supervised diet?

Hello,

Has anyone out there been approved through BCBS TX Anthem who did not have to complete the 6 month medically supervised diet plan? If so how? What type of documentation did you provide to the insurance company? Any advice on what type of documentation I should include Please be specific

I Empire BCBS and was approved immediately. I think it depends on your particular plan though. Good luck.

I have BCBS State Health Plan (Minnesota). I just finished my 5th month visit with the dietitian and had my paperwork submitted last week. Haven't heard anything yet but my Dr and dietitian both say they have had approvals after 5 months with the 6th visit scheduled. How far are you in the process?

I have only finished the Dr's consult. I am scheduled for the Psych eval on Saturayda, February 7th. After that my Dr's office with submit my information to the insurance company for approval. Just wondering what to include with the information so that I don't have to complete the 6 month medically supervised diet.

I have BCBS TX. They require 40 BMI or commodities. Documentation for previously weight loss attempts in the last 2 years, doctor stating surgery is medically necessary. I'm just wondering what documentation to include??
