Has anyonw had a leak?

I am researching band to sleeve and I want to make sure I am informed of everything. Had anyone had a band to sleeve revision that resulted in a leak? If so when was leak found? What caused it and how did you fix it? And would you still have the sleeve if you had to do it again?
