Looking for medical insurance in Northern California

I live in Northern California. I'm about 120 pounds overweight. I am self employed. I am searching for medical insurance that would pay for my lap band surgery. These are my questions:

(1) What insurance companies usually approve of lap band surgery?

(2) Should I join Weight Watchers so that there is a record of me attending classes and for how long should I attend?

(3) Once I have medical insurance, when should I start asking my doctor about having the surgery
