Weight history stress

I finally get a record of my weight for 2006 and my BMI was 34. Even though it was 35 or greater for the the following years, the Dr's office says I may not be approved . I have not had my height offically taken since 1990 so I'm hoping that I can ask them to officially check my height and that I've shrunk with age age and they will update my records and that will fix the problem. Would love some feedback. Thanks    
My PCP from another state has been really slow in sending my medical records for 2006 (the only year missing from 5yr history and Its been 6week since my second request for my records.). So I requsted records from an ER visit I had that year for heart palpitations and the Surgeons office got them already But the dag blasted ER failed to record my weight for that visit! How do I find out what the hold up is with the PCP if all they do is refer me to the medical records voice mail. I only have
