Realize Band Trouble

My daughter, 28, just had her realize band removed after only two months. She could not keep anything down for over two weeks. They did tests, barium in her did not allow for a cat-scan. I had her demand an ultra-sound test for her gallbladder. They took it out, it was bad, but they discovered her band had eroded into her stomach. I would like to know why. Was it the wrong size?
