#1. Surgery date? February 5th #2. State you live in? New Jersey #3. Doctor/Surgery Center? Dr. Amit Trivedi/Advanced Laparoscopic Associates #4. Insurance or self pay? Insurance- Oxford I got approved within 5 days of my 1st consultation!! #5. Age and height 32 - 5'5" #6. Current weight and goal weight? 259 Goal 150 #7. What was your deciding factor for having this surgery? Depression. Lack of interest in everything. I was tired of being miserable all the time. #7. Do you have the support of family and friends? YES! my sister had Gastric Bypass. That was a little too drastic for me. She herself wishes she had gone for the lap band instead. #8. Concerns and questions? My only concern is if I will fail at this too. After this i have nothing else.
