    February 2024 Surgery Buddies?

    Guys I need help! I am not sure how to post on here but I had the sleeve on 04/18/24 and everything was going great until almost 2 weeks ago. I have not been able to eat, the only thing I can keep down is water. I am starting to feel weak, and now scared. I called my surgeons nurse and she told me it’s a possible stomach virus or just my hormones getting back to normal. I feel worse day by day. Constant nausea, and gagging all day long. What should I do?!!

    I have yet to see a nutritionist or dietician that truly understands the optimal diet for a SADI or DS patient. Most of the time we just get a modified version of what they give Bypass and Sleeve patients. But as you say, you aren't absorbing as much as they are, therefore to give you their requirements would leave you malnourished. 15 grams of fat is absurd by all medical standards. Most medical studies show we need 60 grams of fat for proper hormone function and even if you ate 60 grams of fat, with an altered digestive tract, you aren't absorbing that much, so you need to eat a little more than that. Even the ASMBS recommends 60 grams of fat by 1 year out!! Your dietician is not following best practices. Most SADI/DS patients I know follow the vets that have been living with this surgery for 10+ years and maintaining their weight loss. We figure they know more about optimal diet than a surgeon who has never lived with this surgery, or a dietician that has never even researched best diet for bariatric patients of various surgeries. Most patients I know are eating 100-120 grams of protein, less than 50 grams of carbs, and over 60 grams of fat, usually around 100 grams of fat for SADI and 150 grams for DS. You can increase your carbs once you have reached your goal weight, they tend to slow/stop weight loss, so in active weight loss it is important to keep them low so you stay in ketosis. A calorie range makes dieticians feel better, but it makes no sense for a SADI or DS patient because we malabsorb so much fat we can't even begin to properly calculate our calories. You could eat 1200 calories, but that wouldn't be what your body is absorbing. I'm sorry this is so complicated. In the end you have to decide what you feel best at. I don't feel good following guidelines like your dietician gave you and when I go over 50 grams of carbs I stop losing weight. So...yeah. I'm going to follow what allows my brain and hormones to function, what makes my skin, hair, and eyes feel best, what gives me the most energy, and what allows me to lose weight steadily. I'm the one that has to live in this body. It is sad these surgeries are so uncommon there are no good studies about optimal nutrition for them. But there is no way I'd go below ASMBS guidelines, they are the closest thing bariatric surgeons have to a standardizing body. Your dietician should be able to give you ASMBS guidelines for your surgery.

    Travel 6 months Post-op

  5. Hummm. I was concerned about the bathroom issues too but was told that it’s mostly only when eating bad or too much. I did read on one site that you can take immodium at night to help prevent the early morning bathroom issues which I assumed based on that it was a more commom thing. I did read that it’s worse right after surgery and usually lessens with time. I have my fingers crossed for you it gets better.

    Travel 6 months Post-op

    I travelled internationally two months ago, and was 6 months post-op. I brought protein bars, protein-enhanced electrolyte water mix packets, an empty water bottle & coffee cup, and snacks like a nut-based trail mix that I like to put together. I also hauled a giant plastic container of protein powder with me because I LIVE for my morning proffees. No problems crossing any borders or with any airports/security checkpoints.

    She said that whatever you drink the week prior to surgery during the fast, many people seem not to like it afterwards. She said everyone is different and sometimes your taste preferences change.

  9. 9 months out and down 76lbs. BUT I'm having a lot of bathroom issues that I would not do it over. I need to be by bathroom all the time. I never had these issues with sleeve.

    @Zingor4 Hours??!! Is this like, right before your surgery? I've had two pre-op appointments, both took about 30 minutes. The first was with a nutritionist who went through the "no kidding" diet restrictions for the post op time and my blood work and the vitamins I'd need to order. She was even more insistent that there should be NO VITAMINS until 2 weeks post op. T The other appointment was with the regular surgical nurse who went through what to bring to the hospital (CPAP machine, etc.) and what not to bring (valuables, all my kids, etc.) whether I had any allergies to meds and my history of prior operations, anesthesia difficulties, whether I needed help with translation, etc. And I'm with you on the clothes. When do we pull the trigger on buying new stuff?

    So I finally got an email back from My dietician and the macros she gave me for diet post Sadi is 1000-1200 calories 90g protein 90g carbs and 15 grams of fat. That doesn’t seem to be anything close to yours. I am so confused and wondering if she is wrong. I feel like if I don’t absorb a descent pirtion of that I would be malnourished.

  13. I'm so excited! I'm coming up on one year in June. I've lost 111-ish pounds to date. A little worried about my 2 months off that are coming up, but I'm proud of my accomplishments so far.

    Does anyone know what is involved in the pre-op appointment? Mine is Monday (April 29), and they told me it would be about 4 hours long, but it didn't occur to me to ask what will happen. I mostly work from home, but had to dig out work clothes for a week in the office last week. Every evening I would come home and think "well, that's probably the last time I will ever wear that." I am having more clothes funerals than food funerals.

  19. a slice of the loaf of bread i baked yesterday with some jeujed up labneh (i added za'atar seasoning, olive oil, walnuts, fresh basil and honey (yummmm!!) ate all the labneh and ½ the bread (it was pretty filling...i really want a nap now but i need to digest first....)

    Question about fats.

    Okay so I am still so terrible with this stuff but my dietician makes it seem so much more confusing and I really need to understand this so I can get it right this time. I asked her what my requirements would be post SADI surgery and she said 1000-1200 calories 90g protein 90 carbs (45 fruit and veggies and 45 from starch), and 15g fat. I am trying to eat similar to that now to see what it’s like and I am logging my food. I figured my portions will be a little larger so I won’t be able to hit the exact goals but wanted to see where I am. This morning I did great with my protein iced coffee which is just a caramel shake with some coffee mixed in. Low calorie and carb and high protein. This afternoon however I thought I would have 2 scrambled eggs and one Turkey sausage Patty. The calories and carbs are fine but it seems like Either that fat goal is unrealistic or the protein goal is from normal food if there is really 15g of fat in two scrambled eggs and only like 13 grams of protein? Even if I forget the 7g of fat leaving out the Turkey sausage just two eggs is too much fat for one meal and reduces the protein. I hate egg whites but assumed two scrambled eggs would be okay. I know post surgery I will not eat two whole eggs which will reduce the fat some but also the protein. Could i add protein powder to the egg or something? Maybe an egg and a yogurt would be a better option? Or Do I have to just suck it up and eat the egg whites. She also said something about three teaspoons of “healthy fat” and I’m not sure if that’s in addition to the 15g of mono and polyunsaturated fat or if 15 is the total which may be where I’m getting confused. I just emailed her asking for clarification but last time it took her like two weeks to get back to me so I figured I’d see if anyone else can make sense of this.

    My date is may 2nd…… On my second week of diet as or yesterday. Having the hardest time w/this diet, only cause I'm diabetic and on depression meds, and I have to put something in my System to take them. I have dizziness, fatigue , and tend to be light headed at times. It took me over a year and a half to get to this surgery. Was scheduled for 3/12/24 but had a issue w/kidney. All good now, so don't wanna do anything to get it postponed again. GOOD LUCK 🎲 TO ALL MAY PATIENTS !!!

  23. wow, I'm so sorry all of this happened! That must've been awful to deal with so props to you. I felt like I was going insane with just a month stay in hospital. I have a doctors appointment on Monday, luckily they got back to me quickly today and triaged my referral. They did ask me to go back to my surgical team first, but I had my surgery about an hour away and it's just awkward getting there when I cant drive or use public transport at the moment, which they understood. I also had a feeding tube - not via nose but via a line in my arm - and the nutrition in that daily... the side effects are not pleasant! So I can totally sympathise with you there. They did mention pre-surgery I could potentially wake up with a drain from one of my incisions, which I didn't in the end, but part of me feels like perhaps it would've helped identify the blood build-up, or something else. Another thing similar to your story was they also thought it could've been a leak as something showed up on the CT scan along the staple line. They eventually ruled that out but my health anxiety is just crazy now.

    I had surgery on 4/8/24. I had a bypass. I am able to drink fluids with no restrictions. I'm on the puree stage. At 2 weeks out I have only lost 16 pounds. The last 3 days the scale actually went up 2 lb. I'm not going to lie. This is an uncomfortable feeling. Specially because I'm sticking with Drs orders. I do notice that even though I eat 3 ounces of food and try to keep up with my fluid intake I don't get a "full" feeling. It does not take me a long time to eat. So I'm not sure how to feel about that.

    Had gastric bypass Feb. 7th. My food intake is hot and cold at times. The last few weeks I've felt like I needed to eat more often. Not more volume, just spread it out more to fuel myself. I've been exhausted lately. I think spreading out my meals has helped some. You should try the baritastic app. I've used that since October. The dietician at the bariatric center can see the food and drinks that I log. But it's a good app just for yourself as well to keep track.
  26. My surgery was the 16th. I’m generally okay but exhausted. Food diary is a good idea, here is my yesterday: Coffee Ray fish with fennel salad and potatoes Square of dark chocolate Alcohol free spritz Asparagus and mayonnaise, bread, strawberries and whipped cream.

    February 2024 Surgery Buddies?

    A food diary is a good idea. I hate doing them but I've been keeping one since my preop diet and will probably continue for a few more months until I really feel like I've got a good routine and no issues with hitting all the goals. I'm feeling pretty much back to normal, no issues with food in more than a week. What kind of pain are you experiencing?
