Spoke with **** at UHC to see of the predetermination letter had been received by them, he called the predetermination dept he said that I don’t even need a letter of predetermination all I need is a BMI of 40 but the more information I have for them is great! He gave a fax number 801 567 5497 attn to UHC Predetermination or call Care Coordinator dept 800 842 3920 to give me my id number and the reason for the surgery I would like to have. He really didnt persuade me into thinking he knew what he was talkin about. :phanvan :phanvan

I called Scott at Dr. Rivas 10:45am

He said he called on the 22nd to give gave a tentative surgery date of 2-26-2007 gave reference number waiting for a call back from the Care Coordination Department. He told me to call and ask the status of predetermination have you reviewed the case have you requested clinical information.

Spoke with the Care Coordination spoke with Erika in the department to see if they have reviewed the case or requested Clinical information. She said they case was called in the 19th it takes a while to go thru the process. After I explain to her my family history and why I need this. She gave me the fax number to give to information she asked for my BMI is 41.2 & weight 267 & height 5 7. She said that would help with the process

Called Scott back gave him the number and information he said my weight was 269 BMI 42.8 height 5 7 when I went to my first consult he said he would send the information over today!!!!!!!!!

My present weight is 264 goes up and down 1 pound!!

Im not mad cause I did the guys job I want the surgery I dont mind working a little to get it. I feel at ease because I had a hand in the process.
