ongoing weight loss after 24 months?

I was sleeved on march 2010.My weight was 355 pounds for 5'11''.And my age was 67.Have always been an athlete:hockey,football,baseball...This summer I rode my bike from 15 to 20 miles about 5 days a week.
My health is great and since the sleeve my diabetes is much improved,same with my hypertension and cholesterol.
The problem is my weight.Today it's 295 pounds. After 12 months it was still 315.So last six months I lost around 20 pounds.But in a perfect world I would like to be at 220 or better.
One of the reason for that disapointing result is that I followed my nut recommendations for carbs.It says that it should be around 160 a day in the Canadian Food Guide.She works in the big hospital where I had my surgery.I know now that many surgeons recommend to be under 50 grams if we still have to lose weight.
Also from a scan my surgeon said that my stomach did not expand much and is still very effective.
I enjoy good food but I dont have any problem restricting myself.
I would appreciate to receive comments and observations from vets members and those who had a similar situation because my surgeon is now offering me the second step,the DS.He says that it would clear all my comorbidities(or nearly).But it is a serious matter the rerouting of the small intestine.
If I would be sure to lose my excess weight by restricting more my diet and increasing my training I would stick with the sleeve but my proposed operating date is close and I have to make a decision.
