    Hi switch to Blue Cross Anthem PPO and went to Dr. P. Quilici in Burbank. my frist visit was Aug 17 my surgery date is Dec 1st, very easy and only out of pocket cost is $800 My friend did Gastric Bypass under Blue Cross then switch to Kaiser,with Dr. Quilici and she has had No problems Hope I been helpful
