    Not only have an heard of that requirement,i was required to abide by it and if I missed a year, say 2017, the clock started in 2018. My requirements were: ] 5 year sequential weight history 12 sequential MD or RD visits (again, miss one, start over) psych eval asthma dr sign off (I have asthma) Note - someone mentioned WW. I have SIX years of WW history and even after going through the appeals process with my insurance, they would NOT accept; it is not a medically licensed plan. Note, my insurance is BCBS.
