    As stated here, the purpose of the pre-op diet is to shrink your liver. Almost all obese people have fatty liver disease. The pre-op diet shrinks your liver because the liver stores a form of sugar called glycogen. There is enough of this sugar stored in your liver for your body to live off of for about 3 days. After the glycogen is exhausted you enter a state called ketosis, when your body begins burning Proteins and fats for fuel. Because glycogen needs a large amount of Water, the loss of it results in weight loss and your liver shrinks. During the beginning of ketosis you will experience temporary side effects such as irritability, headache, and feelings of faint. However, your body will adapt to this in about 2-3 days, assuming you do not consume anymore carbs. If you continue to consume carbs you will never get pass that stage. If you ride it out then you will notice you will return back to normal.

    Glycogen is stored in the liver. Most obese people have a fatty liver. The liver sits on top of the stomach and has to be moved out of the way in order to access your stomach. A fatty liver full of glycogen makes that harder to do. So the idea of the pre-op diet is to deplete your liver of glycogen, making it more manageable to move out of the way. Many people don't have to do a pre-op diet, because either their surgeon suspects they don't have a fatty liver or he/she is confident about working around a fatty liver. EVERYONE has some form of 1-2 days liquid diet because your stomach needs to be completely empty for the surgery to be preformed safely. That is unrelated to the liver-shrinking pre-op diet.
    It's also worth noting that, while many people do lose some weight on the pre-op diet, weight loss is not the goal. Some surgeons will use weight loss during that period as an indicator as to whether the patient actually stuck to the pre-op diet, but it is a side effect of the diet, not the purpose of the diet.

    I received a call from the surgeon's office today, notifying me that I received insurance approval for the sleeve. I'm now scheduled for surgery on June 30! I bawled my eyes out as soon as I hung up the phone. I was worried that I wouldn't be approved because my PCP refused to use the term "medically necessary" and note my family history of severe diabetes (loss of limbs and life) on the recommendation letter. She said I'm just overweight (yeah, OK, 100+ lbs. over the top of the range for my height is just overweight) and I'm not "sick enough" because I'm only pre-diabetic means it's not medically necessary. After coming to her for the 4 months of weigh-ins, and talking about it at every visit, I thought she knew better.
    I was shaking all day and trying not to cry at work. I'm not usually so emotional but I guess I didn't realize how anxious I was about not getting that approval because of the PCP. Since that interaction with her, I've been so demoralized that I've been going up & down the same 2-3 lbs. I think I kind of gave up, expecting to be disapproved. I'm glad I was wrong.
    I had planned on doing self-pay this year. Then our 2016 benefits came out in Oct. 2015 and I saw the news that bariatric surgery would be covered in 2016. I switched PCPs in November to be close to where I now live. Went to info session in mid-December, and started the 4-month process in January. Now I can move on to worrying about the pre-surgery diet, the surgery, and post-surgery life!
    Oh, and I will be changing PCPs.

    I am on day 2 of a 7 day clear liquid diet. Soooo hungry! Headaches and nausea. Can't look at Facebook because of all the pictures of food on there. I want carbs!!! My surgery date is May 18, also!

    So today I woke up and took my normal morning tubby. When I went to dry off the strangest thing happened... The towel went all the way around me and I was even able to tuck a bit in to hold it!
    This is proof that towel gnomes exist and they must have come in the night and magically made all my towels longer and bigger. The husband says it's because I've lost weight but he's a silly man. I know who's a work here. Now if they could just give up the underpants and sock gnomes that infest this house I'd be ever so grateful...
    Is this one of those NSV thingies?
    Wow. What a gift this surgery is. Every day I'm amazed and thankful.

    I hear ya! Mine is May 18, didn't schedule it until two days ago. I didn't have any special diet until today. Today I started Clear Liquids. Basically a starvation diet. Nothing but sugar free drinks, sugar free Jello, sugar free popsicles, and broth. Max 5-10 calories per item. Went in for preop bloodwork today and to meet with nurse and anesthesiologist. Also had my pic taken in Dr's office. Seems to be happening so fast!

    Had to undergo 6 months of dietician visits, then await approval from insurance. I was sure they were going to deny me because I hadn't lost weight while seeing the dietician....basically stayed the same. I got the call on Friday that it had been approved, and scheduled surgery for May 18. It seems to be happening so fast now. I started clear liquids today, and go in for preop testing today. Yikes!

    Dr Snow & Dr Van Wagner no longer share a practice as of Feb 1st. I will call Dr Van Wagner's office though and see if their nutritionist takes outside patients. Thanks so much!!!
