    Russian Roulette

    I had my sleeve revised to RNY 11/10/22 due to GERD. I stayed on fluids/purées/soft foods advancing very slowly. I didn’t start solid foods until end of December (about 6 weeks). Since then I feel like I’m playing Russian Roulette every time I eat. Sometimes I eat and feel fine without pain or restriction. Other times I will eat the same exact thing, same amount and be doubled over in pain until 4-8 hours later when I finally throw up and the food comes up not even partially digested. Every time I freak that I have an obstruction. Nutritionists say it’s too much fiber or too much fat, but it’s not necessarily associated with diarrhea and why, then, can I tolerate it sometimes and others not? Surgeon is sending me in for an upper GI barium study next week. I’m so stressed that I have stretched my pouch somehow but I can sometimes eat far more volume without issue compared to some of these episodes. Has anyone else experienced this? TMI, I’m also noticing pale colored stools but the pain is on the left, not on the right with my gallbladder. He said my gallbladder looked fine during surgery. I just need this to work so I feel better and can get on with my life.

    The GERD is better for the most part. I just stopped the Protonix after 3 months and I’m okay unless I’m having one of those “attacks “

    Postop

    Just had my revision from sleeve to RNY for GERD 2 days ago. Still sitting in the hospital because I can’t pee! I’m itchy all over and have a migraine. Whaaa. Sorry, just needed to vent!

    My father never had surgery but suffered from GERD as long as I can remember. He was diagnosed with esophageal cancer last year. That’s what motivated me to get this done. Before surgery I was also on pantoprazole twice daily and still taking tums and GasX daily too. I also wear a CPAP and had unbearable aerophagia every morning. That’s ultimately why I chose to go through with it. My sleeve didn’t help me much with weight loss but I lost 70 lbs earlier this year between the nausea and vomiting! As I type this I just woke up with similar but milder symptoms and I’m 10 days post op. I really hope it’s just due to the healing process! Everyone I’ve read about says all symptoms of GERD disappear after revision

    7.5 years. My initial surgeon KNEW I already had reflux and sai “well, you’re just going to have to take omeprazole for the rest of your life “. Then taking omeprazole twice daily failed… I’m so much happier with my new surgeon!

    After multiple doses of Benadryl and getting home the itching is under control. I first experienced it way back in 2004. Just head to toe intractable itching. Never did figure out why and it flares up every now and again despite taking Zyrtec twice a day. I keep getting rashes under my breasts and panus. My BMI is only 33 and my blood sugar is normal (not diabetic). I plan to document it as much as possible so I build a case for Kaiser to pay for some future plastic surgeries. Do you have to reach your goal weight in order to qualify?

    Tips for getting into the revision mindset

    These comments are encouraging. I’m revising from my VSG (2015) to RNY in 8 days and I have SOMUCH anxiety about it! I’ve lost more weight this year than I did post VSG thanks to a combination of severe GERD, aerophagia from my CPAP, and Ozempic. It’s worth it if it resolves the GERD and aerophagia (maybe sleep apnea altogether but I won’t hold my breath 😜). I’m just worried I’m trading one set of problems for another. One specific thing I’m worried about is immediately post op. I still can feel viscerally the heaving/gagging in PACU once I was extubated and they were trying to wake me up. It felt like I could breathe due to those dry heaves! I’m scared of going through that again!
  8. Hi guys! I got my sleeve in 2015 and never really felt any restriction. I counted calories and was able to drop 60 lbs my first 3 months… and then my life exploded. The stress I was under made counting calories impossible. I still ate healthy foods for the most part but I stalled out for a full year and then slowly the weight started creeping back until I was only 15 lbs below my starting weight. Well, now my GERD is pretty bad and my father has esophageal cancer so I need a revision as much as I don’t want to go under the knife again. The thing is, I’m currently 86 lbs down from presurgery weight in 2015 and I keep losing! Ozempic and these other injectable meds for diabetes are game changers! I wish they had come up with this stuff decades ago! Of course between the GERD and the side effects I’m often nauseous if not throwing up on occasion but I’m hoping my revision will help with some of that. Has anyone else combined Ozempic (Wegovy) with a revision to RNY? My doc says I can keep taking it after surgery but at $300 plus international shipping I’m hoping I won’t need to!
  9. I had my VSG 7/2015. I never felt much restriction and was hungry from the start. I restricted my calories to 800 per day and followed all the rules for the first 3 months and lost 60lbs… then my mother in law was dropped on my doorstep. LONG story but I was the sole provider for my husband and 2 small kids, working 12 hour night shifts. She had stage 4 emphysema and end stage esophageal cancer with alcoholic dementia to boot. Our house was tiny. She was 69 and weighed 65 lbs … let’s just say I was a bit stressed. I stopped counting calories. I plateaued for a full year and then started gaining. I’m now 6.5 years out and just 27 lbs from where I started. I’m hesitant to revise. My bariatrician tells me people tend to not have as much success with revision. I chose a sleeve because I didn’t want to risk not being able to absorb medications I need but my GERD is so bad I may not have a choice. I’d love to hear if anyone had a similar struggle with their initial surgery and were successful with the second. Those in the thread who had the scar tissue and hernia repair, surely they will allow a follow up procedure.

    How did you figure out if you had dumping syndrome?

    It’s really strange but I’m 6.5 years out from VSG and I get all those dumping symptoms sometimes 3-4 hours after eating. It doesn’t seem to matter what I ate, although I’ve noticed peanut butter seems to do it frequently. I’ve been eating low carb/semi-keto for months so I’m not sure why it’s happening. I’m going in for a ph and manometry screen soon, hopefully that will give some answers!

    Good news is my NUT says I'm doing perfectly and she has no complaints! 27 lbs down in the first month even eating around 1000 cal a day. ☺️

    I'm worried that I don't have enough restriction. 4 weeks out today and I'm hungry. Eating around 1000 cal a day no problem. Getting plenty of protein and water in too. I really hope I can at least stay at this point. I'm losing slowly but still losing (about 3 lbs a week). I see my NUT tomorrow so we'll see what she says!

    How much time off work?

    I'm heading back tonight! Im 3 weeks out and nervous about my 12 hour night shift in the NICU!!! At least my coworkers are awesome and will help me out. I just hope I can stay awake without caffeine!!! Wish me luck!

    Left side pain

    I'll be 3 weeks out on Monday. Advanced to purée 1 night early, soft food 2 days early. Only thing I've had off my NUT's list was tiny bits of chopped celery in chicken salad 3 days ago (commercially prepared). I advanced a little early because the only symptom I've had since surgery is fatigue and I was hungry. These are the only cheats, I eat slowly and chew at least 30 times, no drinking before/after, sometimes eat a max of 1/2 cup which is way more than I expected at this point. Today is the worst pain I've had since surgery so I put myself back on liquids. I spent all day yesterday cleaning and did some furniture moving (my surgeon gave me no weight lifting restrictions after the 2 week mark!) and today my left abdomen, up under my ribs, around to my back, and up to my shoulder and neck hurt so much I started back on pain meds. Doesn't feel like gas but I took gas-x just in case. I'm not nauseous, no fever or pain when I press on the site... Did I just pull muscles or could there be a problem with my sleeve? Since it's one sided I feel like its sleeve related but not enough symptoms to warrant an ER visit...

    Stage 2 diet / full liquids & pureed food

    Ok, now that I'm so close to purée stage... Would it be a mistake and cheat one night early? I want to sit and have dinner with my family tonight even if it's just cottage cheese or refried beans!!!

    Time off work

    I wanted 6 weeks off. My surgeon begrudgingly gave me 3. I work 12 hour night shifts in a busy NICU. Glad to not have burned through ALL my PTO but at 13 days out, my return date is coming fast and I barely have the energy to stay awake 12 hours straight let alone WORK! Hopefully I'll feel better in a week!

    Stage 2 diet / full liquids & pureed food

    Today is my last day of full liquids! Yeah! Tomorrow I get to try puréed foods and 3 days later on to soft! I'm so bored with full liquid I could scream. Drinking doesn't seem to bother my sleeve. I can drink (water especially) relatively quickly. That's a relief since 2 liters a day seems way low for my usual water intake. I'm looking forward to eating real food and hopefully a full sensation after 1/4 -1/2 cup!

    One week out from surgery...

    And I'm down 8 lbs!!! That's not including the 5+ I gained from the IV fluids! Still on full liquids until purée starts Thursday. I'm still pretty tired. Don't think I've been awake for more than 8 hours at a time. Hopefully my stamina will improve this week! Doc wants to send me back to 12 hour NOC shifts next week. I highly doubt that!!!

    Surgery tomorrow!

    So, I'll be under the knife at 7:30 tomorrow morning. I tucked my kids in at their cousin's house, charging my iPad, gassed up the car (hospital is 45 min away without traffic)... I'm starting to get nervous!!! There's a tiny part of my brain that is freaked my kids might never see me again! Totally irrational, I know, but still scary! It doesn't help that I've had a migraine since starting clear fluids!! I know this is worth the discomfort but I really hope the headache and nausea get better quick and I can be a better, happier, more energetic mommy soon!!! Wish me luck!!!

    Surgery tomorrow!

    Thanks for checking in! The Bari RN called yesterday and told me not to worry about the calories too, like that's even possible when we're tracking everything else! :-P I'm ok. Still sore but that's to be expected. Surprisingly I'm a bit hungry but it's not too bad. Maybe I need a stronger PPI... Trying not to take the pain meds during the day so I can hang with my kids!

    Cream of wheat

    Funny how various surgeons have such different protocols!!! I started cream of wheat/rice the day after surgery in the hospital! Now I make one serving and mix it with a premier protein shake, then divide it in half. Super yummy even though it gets cold since it take me an hour to eat it!!!

    Surgery tomorrow!

    Thanks for all the support guys! It went well and aside from the initial severe nausea I'm feeling pretty good. Got home last night around 9pm. Today I've walked for 45 minutes, got all my protein and fluids in. Ate about 800 calories in full liquids. Seems like a lot on day 1. Worries me that I'll get hungry once the swelling goes down!!! I think I'm more thirsty than anything. Most of my life I have been a big water drinker. 3-5 liters a day ! So this 64oz business feels really low!!! Is 800 cal a day an appropriate goal? My NUT said not to worry about anything but protein and water but...
  24. My surgery is on Monday too. I'm lucky the only pre-op diet I'm required to do is 48 hours of clear liquids. Good thing it's so short since it's giving me a migraine!!! I'm feeling a little anxious about surgery but I'm confident in the long run it WILL all be worth it!!!
  25. I did an intensive 12 week exercise and diet class through my work run by PTs and exercise physiologists. I wound up in pain from flared up old injuries and I GAINED weight (not muscle because my body fat went UP .05%!!! ) Surgery is on Monday! I'm nervous but excited!!!
