  1. Kindle I'm glad the surgery went well for you with ortiz at occ and it appears that is the case with the vast majority of patients that have gone to occ. I have just read enough posts on this site were people aren't feeling well three or four days post op that I feel more comfortable staying in a real hospital. I am feel okay right after post op and then when they would discharge me and I would get back to the Marriot I could start developing complications. In the rare case that I come down with complications or just don't feel well I like the thought of being in a hospital where nurses and doctors could take care of me. Right now hospital angeles will let me stay one night pre op as long as i check in at or after 6:00 and four nights post op. I think they want me to stay in the hospital at least three nights post op and it will be my option to stay in the hospital on the fourth night or check into the hotel(camino real). I will most likely stay there all four nights in case I develop some type of post op complication. If for some reason I'm really not feeling well I'm sure they would let me stay longer but I'm guessing I may may have to pay for additional nights at some point. Similar to Rocky, I just feel more comfortable staying in the hospital given that I will be there all alone with no one to take care of me in a foreign country. Staying in the hospital doesn;t guarentee there won't be complications or something serious might happen. On these posts I have seen on extremely rare occassions in 2008 and 2011 where two patients unfortunately passed away having weight loss surgery at Hospital Angeles; however, I believe that staying at the hospital helps to reduce the risk of something seriously happening and if it does hospital angeles is much better equiped to handle the issue versus the TJ Marriot. TIme is of the essence in those type of situations. I would be very comfortable with having ortiz do my procedure. I'm just not comfortable with the occ standard setup of one night in the hospital and then four nights in the hotel.
  2. This is the description of the Ortiz surgery method that for a layman such as myself looks to be very thorough. The OOC marketing email claims Ortiz has had a 0% complication rate which is a bit hard to believe and impossible to verify; however, I'm sure it complication rate is very low. The one concern I have about Ortiz and OCC is that you only get one night in the hospital and four nights in the Marriot. That is why I am currently signed up to go with Corvala at Hospital Angels. I like being able to have the four nights in the hospital if I think I need to stay there that long to recover. I just think the nursing care would help as I'm recovering and and having emergency care at the hospital makes me more comfortable that staying at the Marriot. The thing I'm not sure of yet is Corvala's surgery technique as compared to Ortiz. I would like to better understand Corvala's suturing technique and the percentage of sleeve's he has done without leaks however I'm not sure that information on percentage of post op leaks is readily available. From what I can tell it looks like Ortiz and Corvala are both well qualified surgeons and i understand they used to practise together. Standardizing the sleeve gastrectomy through staple imbrication - note from Dr. Ortiz about his suturing technique With less than a decade from its start, the Sleeve Gastrectomy (“Sleeve”) has had great results and a low complication rate, but these results vary amongst bariatric practices in the United States and the rest of the world. Variations in the technique and surgical materials used for the procedure could be partially responsible for these differences that can result in less weight loss or a higher complication rate. The world renown bariatric surgery team at OCC with vast experience in Sleeve surgery has been able to perform this procedure with a 0% complication rate and greater that average weight loss. In order to achieve these results, we perform all of our surgeries with a standardized approach that involves complete gastric mobilization, and removal of the fat that is around the area where the esophagus meets the stomach. This is done to allow complete visualization of this area (important when stapling and suturing the stomach). After the stapling and removal of the divided stomach, the imbrication or suturing over the staple line is used primarily to avoid bleeding and leakage, it is not routinely performed by all bariatric surgery centers. In most cases an absorbable suture will be used. At OCC we use a non-absorbable suture for the imbrication process, starting in the proximal tip of the staple line, making sure that this area is completely over sutured (imbricated) since this is where most leakages in the medical literature have been reported. The initial removal of the fat in this area allows perfect visualization of the stomach wall and its staple line. The suturing is continued distally along the staple line with the oral gastric calibration tube in place so that we can calibrate the sleeve in order to have an even and perfectly formed tubular or sleeve like stomach. This also allows the suturing to be continued more distally than that of the staple line to extend the sleeve, if necessary. Since the suture is non-absorbable the patient is less likely to lose the restriction of the sleeve gastrectomy.

    I now agree there are some other good surgeons in TJ besides Corvala. Ortiz looks like he would also be good but I am already signed up with Corvala.

  5. Hospital Angeles with dr Corvala is letting me stay one day pre op and four days post op at the hospital(i.e. not a hotel room). I have a bmi of 44. that is one of the reasons i'm going with hospital angeles as I will be in TJ all alone and believe I may need some nursing help with the recovery in the first few days.

  7. for medical travelers who wish to stay in the U.S. for VSG surgery Dallas seems to be the best option if you need to fly to your surgeon. Cost of VSG in Dallas is half what it cost in the Northern part of the U.S. probably due to the competition Texas Bariatric surgeons face from their lower cost colleagues just across the border. Dr. NIcholson is the best option I was able to find in Dallas for an out of town vsg payment. His price is 12,200 for a vsg. I chose DR. Corvala at Hospital Angeles in TJ as I think that offers the best quality to price ratio but for those not comfortable with TJ or catching a lot of grief about going to Mexico like i am, Dallas and Dr.NIcholson are a viable high quality low cost alternative to TJ.

I like that fact the surgery is in a full scale real hospital with 100 plus beds and an ER and not a clinic they call a "bariatric hospital." that gives me more piece of mind if something goes wrong and I like being able to recover in a real hospital where the nurses will help me with the recovery process versus being all alone say in the TJ marriot after having a surgery. Dr. Corvala is also the most experienced surgeon in TJ who as taught many of the other surgeons. I'm paying 6500 for the package which appears to be more than the other surgeons in TJ but I believe you get what you pay for and don't want to go cheap on such a serious surgery that could mess one up for life it is botched. Dr corvala and hospital angeles offer the best quality to price ratio in north america in my opinion but i will be able to give a more accurate assessment after I have the surgery. note they just raised their price to 7500.

    toohighofa#, I never said that I don't believe in bariatricpal and i have noticed they offer other surgeons in Mexico who cost less than using dr. Corvala at hospital Angeles. I have no opinion on those surgeons what so ever as I have not done any research on them. They are considerable lower cost which raises the question why are they do much lower. It may be they are a phenomenal deal for VSG surgery for all I know but I do know VSG surgery is nothing to mess with and it certainly is not something where one should cut corners as they may pay much higher medical bills down the road if the surgery goes bad. That is just a risk analysis that each patient needs to make for themselves and they each need to do their own extensive research. This surgery could mess someone up for life if it is not done correctly. I've reach some articles in the Canadian press about some VSG surgeries in mexico going bad and then the paitent flys back canada and incurs significant medical bills imposed on their government sponsored medical system. it is a bit controversial it appears in canada in that if someone goes cheap and has a poor surgery and then comes home and s a burden on their medical system(i.e privatizing the profit and socializing the cost). in the u.s we don't have that issue as we as patients would get stuck for paying at least a portion of a botched surgery. To be clear cannot offer any opinion on any vsg surgeons in mexico. Alll i know is that so far i haven't found any show stoppers regarding dr. corvala but he is the mexican surgeon I have researched. I find this forum very helpful for all patients to share their ideas but we should read the posts with a healthy bid of skeptiscm and each do our own homework after all it will be each of our bodies operated on and will impact us the rest of our lives.

Rocky1 or anyone, any idea if Dr. Corvala uses an absorbable polyme membrane(APM) to reinforce the staple line for vsg Does he invert the staple line using the david kim "inveted" gastric sleeve technique. that is where three rows of staples are used on the sleeve. IT is then sutured over the staple line. the sutured staple line is then inverted to the inside of the stomach and sutured again(french seam). These techniques supposedly significanlty strengthen the sleeve. Dr David Kim who is a famous bariatric surgeon in dallas advertises that he uses the inverted technique on his vsg patients. I'm a bit paranoid about leaks and would prefer someone who uses APM and the inverted sleeve technique to strenghten the leeve. I 've also read the length of the sleeve is referred to as the boogie and an optimal boogie of 32 or 34 is idea but i have no idea if there is any truth to that. I"m currently scheduled with dr corvala on june 23. I'm also looking at using dr kim or dr nicholson in dallas. my family is not very supportive of my consiering Mexico. Dr Corvala at hospital Angeles is the ony mexican bariatric dr and facility i would consider. The strip mall locaionts in tiajuanasound like butcher shops to some extent based on some of th posts i've been reading on forums. dr covala is 6500 if you make your 650 down payment b the 15th of february which i've done. apparently it goes up to7500 on feb 16 due to an increaase in thehospital fee. dr. nickolson in dallas is at 12,200 for a vsg and I think dr kim is between 13000 and 14000 for a vsg but i am not entirely sure yet what he charges. I'm really pushing hospital angeles to stay at the hospital the five full days they advertise on their website so i have a long recoveryin the hospital and take advangage of the free post 6 pm check in the day before the procedure. that would to be a total of 6 nights in the hospital for me. the hospital care at hospital angeles and dr. corvala's rep is why i might use him versus dr .kim or dr nichsolson in dallas. the case manager at hopital angeles is pushing back saying that 3 nights post op in the hopital is sufficient but i will insist on the five nightes for vsg they advertise on their website. if for some reason i back out of using dr corvalai've been told by my hospital angeles case manager that i would forfeit 10% of the surgery down payment. sine the down payment was 650(i.e 10% of hte 6500 total fee) i fould forfeit $65 should I cancel as a paperwork admin fee. THat is not an unreasonable policy. for anyone who has or is seriouly considering dr . corvaa for vsg please keep me posted. i have a bmi of 44.

    Do you know if Dr. Nicolson uses a technique similar to the inverted gastic sleeve method that Dr. KIm from Dallas advertises and trademarked. Also, I'm from out of state. At three days post op would you feel good enough for a two hour plane ride home or do you think it takes more time to recover before someone should undertake such a trip. Did you also consider Dr. Kim. Dr NIcolson appears to be a bit lower cost than Dr. kim but cost is not by any means the only factor I'm considering. How many nights did you stay in the hospital. I appreciate your comments regarding your experience at that hospital. I'm also looking at dr corvales from hospital Angeles in tiajuana . He is the only doctor i would consider in Mexico but my family is urging me to use a doctor in texas if i have post op complications. DId you have any post op complications.

    my cost is 6500 with dr. Corvala in june. note that hospital Angeles recently raised its price to 7500 but I'm locked in at 6500. I recommend going directly through the hospital and not a thirt party because if you have to cancel you only forfeit 10% of the 650 down payment or 65 dollars. I'm also looking at Dr. Nicolson in Dallas and his website indicates that his cash pay price for VSG is 12,200 which is about half of what I would pay in my home area. I am signed up with Dr. Corvala at hospital angeles for june 23. I'm also considering Dr. Nicolson and Dr Kim in Dallas. I'm not getting support from my spouse about having the surgery in Mexico. I'm in the process of trying to determine if Dr. Corvala's VSG technique is similar to Dr. Kim's inverted gastriac sleeve technique where there are three lines of staples, a suture over the staples, then inverting the suture/staple line into the stomach and then suturing again. Apparenly that significantly reduces the chances of leaks and expansion down the road.
