    Do you log your food into something like my fitness pal? It really helps to know exactly how many calories carbs and fats you're taking in so you know how to adjust.
    You didn't say if you are exercising either.

    I found this is to be very interesting. Hope you guys enjoy it.....
    One of the key reasons why bariatric surgery is so much more effective for treating severe obesity than non-surgical approaches, is because of its profound effect on food intake.
    Thus, bariatric surgery significantly affects hunger and satiety and may even have important effects on “wanting” and “liking” of high-caloric foods.
    However, this effect on ingestive behaviour is neither “guaranteed” not are these effects consistent between individuals.
    As I tell my patients, “The surgeons operate on your gut, not your brain”.
    Thus, it can only be expected that a certain proportion of patients will struggle to control their food intake despite surgery, thereby either losing less weight than expected or putting the weight back on.
    In an article, published in Obesity Surgery, we systematically reviewed the published evidence on the role of dietary factors in this issue.
    As may be expected, patients reporting “loss of control” of eating behaviour post-surgery lost less weight or had a higher risk of gaining back any weight they may have initially lost. Thus, individuals with self-reported “high-adherence” scores tended to lose and sustain greater weight loss than those who did not.
    Given that bariatric surgery may limit the amount of food that can be eaten at a single meal, some patients resorted to grazing behaviours (defined as consumption of smaller amounts of foods over extended periods of time) leading to subsequent weight regain. In addition, it was reported that poor diet quality, characterized by an excessive intake of calories, Snacks and sweets, as well as oils and fatty foods, was statistically higher in patients experiencing weight regain.
    Interestingly, even short-term dietary “indiscretion” (or falling off) can result in very rapid weight regain. This is not surprising as, in this regard there is little difference between someone who has lost weight through surgery or simply through diet and exercise. Irrespective of how the weight is lost, the body appears to retain its ability to rapidly regain lost weight if allowed to do so.
    Thus, as we discuss in our article,

    “The existing literature strongly suggests that nutritional and lifestyle compliance is crucial to weight management post- bariatric surgery.”
    “Comprehensive weight management programs must therefore provide improved patient education and promote adherence to post-bariatric surgery diets in order to ensure success. These programs must take action to support patients towards a long-term goal of healthy and appropriate dietary choices with active monitoring (journaling) and reinforcement (review of food records) strategies provided by a multidisciplinary health care team.” As I have discussed previously, although bariatric surgery increases the chances of success, it is no magic bullet or easy way out – all patients have to work hard at learning to use their surgery as a tool to help them better control their food intake and will likely do better, the more they understand and comply with the post-surgical nutrition recommendations.
    On the other hand, there may be very good reasons why certain individuals, despite best efforts, struggle to meet these recommendations.
    I will discuss how neurohormanal and metabolic factors can make dietary compliance difficult in my next post.

    Why Do Some People Regain Weight After Bariatric Surgery?

    http://www.drsharma.ca/wp-content/uploads/sharma-obesity-bariatric-surgery21.jpgAlthough bariatric surgery is by far the most effective treatment for severe obesity, it is not a magic bullet and there are a significant number of patients, who either fail to lose a significant amount of weight or ultimately regain most of their weight.
    In a paper published in Obesity Surgery, we conducted an in-depth review on the issue of post-surgical weight regain.
    Following an initial screen of 2,204 titles and review of 1,437 abstracts, we found only 16 studies with meaningful information on this issue.
    These studies included seven case series, five surveys and four non-randomized controlled trials, with a total of 4,864 patients for analysis.
    There were a number of methodological issues with these studies as they varied widely in terms of duration and quality of follow-up, definitions of surgical failure or weight recidivism, as well as the actual surgical procedures, all of which had important implications for the interpretation of the data.
    Thus, for example, inconsistent definitions of exactly what constitutes “excessive” weight regain or “insufficient” weight loss makes it virtually impossible to determine even exactly how often this treatment can be considered to have “failed”. (Remember, it is always the treatment that fails the patient and never the patient who fails the treatment!).
    Nevertheless, it is clear that post-surgical weight regain is highly multi-factorial (like obesity itself), involving both patient and surgery related factors.
    Overall, we identified five principle aetiologies representing nutritional indiscretion, mental health issues, endocrine/metabolic alterations, physical inactivity and anatomic surgical failure, each of which I will discuss in upcoming posts.
    There is no doubt that the issue of post-surgical weight regain requires a systematic approach to patient assessment focusing on contributory dietary, psychologic, medical and surgical factors.

    Big hello; This is my first post on this forum after lurking for quite a while. Just wanted to say I have lost 5 pounds in three weeks following plan. Great start for me and I am close to half way there, maybe even want to pursue losing back down to my lowest weight.
    Although the scale has stalled this week - I feel better, less bloated and have found some encouragement. Being on track is a wonderful feeling!

    Like Oregon Daisy, I am more than two years out from surgery. Take her advice, and don't keep goodies in the house.
    What i find is that now my calorie intake is so much lower that any extra calories stay on my hips. I am up a size from my lowest and almost 10 lbs. Went into a support group meeting at my surgeon's office this last week learning an amazing fact. If you only eat an extra 11 calories a day - that is a pound a year. So that means if I eat 100 extra calories over a day for a year, I'm up my 10 pounds. So I really need to cut that 100 calories a day for a year and I will lose my 10 pounds. While I have tried going to the gym for several months, the scale simply does not move.
    More difficult to lose weight now since my caloric intake is so low comparatively to the old me pre VSG. Hmm, take OD's suggestion and don't bring temptation into the home. Good advice.
    I love my sleeve and have no regrets.

    You might be eating too fast. Do you get nauseous no matter what you eat or just certain things? My doctor gave me pills for that. It did go away after a few months but I would check with your doc about it.

    It won't play for you Dee? Well I finally was able to watch it this am. The link Chimera gave worked fine and I watched it from my computer.
    If anyone is short on time, he really starts exploring ADF and 5:2 at about 35 minutes in (it's just shy of 60 mins long), but basically the show was about exploring the link between calorie reduction and longevity. He tried a 4 day fast and did blood panels before and after and there was a dramatic difference in the after, but it didn't last. Then he did the 5:2 and in 5 weeks he lost -14lbs, cut his body fat from 27% to 19% and cut his IGF (cancer risk factor) by 50%. Oh and his blood sugar and cholesterol which were above normal went down into the normal range.
    He ended it by saying there still need to be a lot of scientific trials on humans (vs. animals) but that he thought it was something that really could have widespread positive results if people could try it. He felt he was going to continue with it.
    And Chimera...that exercise one looks really interesting too (I can't get it to play right now...it gave me a blurb about technical difficulties...maybe that is what you got Dee?...anyway will try to view it again later.)

    This morning my husband forgot his bag at home that contains all of his things for work..
    So he called and asked me "How much do you love me?"
    He's an ER PA so he said just pull up to the ER bay and call, I can run out and meet you.
    You see he's worked there for going on six years and I've never stepped foot in there because of embarrassment about my weight..
    He's never been embarrassed about me but I have. some the nurses and techs tease him about not really having a wife because they have never seen me (no holiday parties for me).
    Today at 6:15am, fresh out of bed and looking that way! I pulled up to the ER, parked and walked into my husbands work like a normal spouse delivering something to their husband

    He took me around to meet people and show me his office.
    NSV? I would say normal life, and it felt good to be "normal" doing the little things you never would of done.
