  1. Hello all, i am looking to see if anyone here can share some insight on what their dietician submitted to the insurance company. I have optimum choice and have been seeing a dietician for the past 8 months. Its time for me to submit everything to insurance. The problem is because of all the stories i have heard about people being denied i have grown very weary of what should be sent. I want to make sure what my dietician writes up is 'acceptable'. If anyone can share what format their dietician used or even share the actual letter i would be forever greatful. By the way...is it normal for me to be so nervous about submitting? Its really stressing me out more than i would normally allow things to stress me.
