Getting insurance to cover Lap Band procedure

I am new to this and wanted to ask if anyone else has had issues with getting your insurance to cover your Lap Band procedure? I have Blue Cross and when I took out coverage 8 years ago, they made me sign a "waiver" on weight loss. I've had a consultation with a Bariatric Clinic to have the lap band done but found today that they denied me due to the waiver on my policy.
