Did you have to do the 6 month program, self pay?

I'm in AZ too, but it seemed I would never afford it here.

No pre op diet. I had to get a psych and nutrition eval and provide 2yrs of medical records.

My insurance covers it at 80%

That sounds awesome, may I ask what doc ?

And what did 2 years of records consist of ? Just basic records or something in particular?

May I also ask what insurance?

I have health net.

I have the same insurance as you. And just two doctors notes from the past two years one from 2013 and one from 2014 showing your weight at the time of the visit

Mine was quick too. I inquired about it in March and was approved in May. Having surgery June 26. Good Luck to you!
