That's a tough one. With a high BMI a DS is quite effective. You have 75% of the stomach removed & then the innards are rearranged so you malabsorb. This helps to keep the weight off. However

Have you considered starting with just a sleeve and then revising to DS if your results aren't satisfactory? DS is sometimes done as a two stage procedure- sleeve only first and then rerouting later.
