Weight Loss Program

Can anyone tell me exactly what info they want from the weight loss program I attended? Us there a form that has to be filled out? Please help getting discouraged!!!!!

This is an insurance requirement for many companies. From what I can tell, they just want you to be on a doctor supervised diet. It does not seem to matter whether you gain a or lose. On the pre-op diet be loyal to the instructions since that is aimed at shrinking your liver so it won't be in the way for surgery.

While not required I supplied a check in log from my gym for the last year. But I also went everyday (only day they are closed is Christmas).

I have finished the weight loss program but the office is not sure what they are suppose to be giving me.....they gave me Progress Tracking Sheet which consist of my attendance, weight, BMI, & waist. Then I was given a written note saying that I was a candidate for bariactric surgery.....is there something else I need before submitting this

that is what I needed, I think it was called a letter of medical necessity... you can ask your surgeons office to fax a sample one to your Dr.'s office, that is what Mine did... you'll also need to do a Psych Evaluation before you get your surgery date if you haven't done so already. You answer soooo many questions... they just want to see where you are at mentally and I don't believe there is any right or wrong answer.
