Need to know what all needs to be in the letter of medical necessity. I thought that my clinic would do this for me but was told to get it from my PCP. If anyone has a template or sample I would greatly appreciate it if you can send it to my email bunch.pamela@yahoo.com. Also on the history of obesity is it a letter or just your weight through the years? I don't know what that looks like either. Getting frustrated.
