I suspect that you are headed toward revision, given your lifelong experience with this, but there are always things that should be tried before going for surgical solution. Given your history, I suspect that you have tried most of the various tricks and treatments for GERD, such as sleeping with head elevated, not eating X hours before bedtime, etc. There are some books out there by MDs who specialize in the problem that may give some more insight. One common thread seems to be that dietary fat may be contributory to the problem, so your keto efforts may be working against you if that is the case (OTH it may have little to do with it and is just a throwback to the days when low fat diets would fix anything, much as low carb diets are today's universal solution.) It may be worth a try to shift your dietary balance for a while and see if it has any effect on you, There are also some foods that supposedly help or hinder the functioning of the lower esophageal sphincter, and likewise some medications that target it, so those are things to explore. Given your history, I assume that shifting meds may have been discussed - transitioning from PPI to H2 inhibitors such as Zantac or Pepcid - they are generally somewhat less effective but generally preferable from the long term use perspective. Given that you are still seeing reflux evidence in the EGD even on the PPI, it is unlikely to make a significant difference as symptomatic relief doesn't necessarily mean that the problem doesn't still exist and is doing damage, but it is something else worth considering. Historical curiousity - back in the dark ages, vagotomies were performed, severing part of the vagus nerve to disable some of the protein pump ports as a chronic reflux treatment; this was obsoleted by the introduction of PPIs, but maybe it's time to reintroduce the "old school". Hey everyone so I’ve been really depressed because my weight has only gone down 60 pounds I started at 330 and now 269 (it’s been a year) and I don’t know what to do I’ve tried Keto and fasting ... exercise is a bit tough because I always start off strong and lack later on. I don’t know if im just comparing myself to everyone else’s journey and weight loss and my mind is strictly on losing so I’m stuck. I just feel like a disappointment to myself and my parents. I got the surgery at 18 and I’m now 19 and I feel like a failure . Please please please help .. oh and any workout routine suggestions will help !

Thanks for the feedback! I haven't noticed the BO, but my urine has a very different smell too (sorry TMI, but related). I hadn't connected that until you mentioned the BO, but it started with pre-op diet also. Hmmm, very interesting. FamilyGirl, What are Keto-sticks and how do you get them? Can you tell me more about them. please?

33 since surgery, the rest was before that. This pic was before I started keto and my pre-op. I've been a little bit stalled, but I'm not sweating it too hard. I'm taking my surfboard out this weekend which is a big time calorie burner for me. So I don't think I'll be stalled for long. ETA: I was never able to fit in those pub chairs before (on the right). Whenever we would go to that court bar, I had to stand the whole time and just act like I didn't feel like sitting to save myself from being embarrassed. It feels really good to not have hips the size of a Chevy tailgate anymore. -
we are all familiar with prepared meal delivery services like Blue Apron, Plated, Peached Dish, etc. but this one seems to take the cake in terms of optimally healthy, whole-food, paleo/keto food. it's called Factor 75. https://www.factor75.com/nutrition anyone try this yet? i know all of these services are pretty expensive, but for us bariatric patients with smaller eating capacities, i bet one meal could serve as two (lunch and dinner). the meals seem to be about 400-500 calories each and have the ideal macronutrient ratio. everything is grass-fed, local, organic. seems like the perfect solution for single (or married) folk who haven't the time to shop or the skill to cook.

It really isn't Keto because if the meat is lean and the spinach is plain, that is just low carb. The key to eating keto, is the fat ratio. -

I have been keto - moving to low carb and a "low carb" diet is generally under 150g/day - keto is 20g, but there are some "keto'ers" that go up to 50g/day - I would say you are very low carb, but not necessarily keto. -
The holidays were hard but back at it on 1/2. Getting ready to hit the one year mark and would like to hit onderland. I started at 314 and currently sitting at 219. A Keto approach always gets me going I just hope I continue going down

Following here... I had surgery 11.7.17 and my hubby just had his 7.8.18...He's very interested in doing Keto, my eating plan is close but not as strict...it would be helpful if we were on the same plan...save so much $$ and double cooking!

Keto is a great way to lose weight in the short term. Long term, there are strong concerns in the medical community that it could damage your kidneys. Long term research still needs to be done on this. Regain after keto is also an issue. High blood pressure, diabetes and other comorbidities of obesity also damage your kidneys, so keto is sometimes felt to be the lesser of two evils in trying to help people to lose weight....because it does work. Unfortunately, it might work at a price you'll pay later as you age. Have tried keto in the past. Made my BUN go up (kidney function test) and I regained 50 pounds faster than I lost it. Might work well for some people, but keto is definitely not for me. To each, their own. The highest rates of longevity in the world are correlated to people who eat diets high in complex carbs, vegetables, fruits, plant based proteins and fats, and a small amount of lean animal protein. Ultimately, you pay your money and take your chances. Talking to your doctor is a terrific idea.

Download my fitness pal and it will help you with setting up your macros. Macros are the percentage of carbs, fats, and protein intake. I set mine up as 5%carbs, 70% fat and 25% protein. Keto is a low carb, high fat and moderate protein diet

All I can say is that prior to this, I was eating very well balanced meals. I was also obese and needed to cut carbs to generate a quick change for surgery. I wasn't necessarily eating more than 74grams on purpose. But I was eating less than 20 carbs per day and I still had severe proteinuria. All I'm saying, is that it *can* happen to people. Its not something you can just dismiss. And what I meant by the liver shrinking diet being more extreme than Keto.... in Keto you still get carbs from lactose, more so than the liver shrinking diet. I couldn't eat cheese or yogurt on my liver shrinking diet, but it is OK in a Keto diet.

Actually I started in January 2013 with the simple Dietdoctor.com protocol. I started at 251 lbs and by October 2013 I was down to 178 lbs, (height 5'7") which I was able to maintain for 3-4 years with no major effort on my part. My high blood pressure normalized within months after startin this woe, my skin got better, my moods got better ect. I just followed the Dietdoctor's keep your carbs low and eat real food protocol. But as years got by my underlying binge eating disorder got the best of me and first I started binging on keto foods and soon even eating junk-food again and of-course I gained some weight. I found ketogains last summer and was able to lose 25 lbs in a month or two, but started binging again until I was back up at 224 lbs when I decided to have my sleeve in May this year. I'm a self pay patient and live in a European country. Now, 8 weeks post op I'm down to 187 lbs, my goal is to get to 154 lbs. Although my binge eating failed my life's so far best weight loss effort aka keto, I can't blame this on the diet. Obesity and food addiction is a complex disease. Thanks for clarifying. Low carb and keto are definitely not the same diets, and I wish they wouldn't be used interchangeably. Low carb can certainly make sense. Many WLS patients are low carb without really trying, just by eating protein first, veggies second and carbs last, as most of us are told to. I think your last point is one that is often missed. If you're losing weight, by definition you're in ketosis, no matter your carb intake. "Ketosis describes the metabolic state whereby the body converts fat stores into energy, releasing ketones in the process"

Yeah I’m beginning to think f#%* this keto business. Im already restricting enough by keeping to a 1000 cal diet. Beans, sweet potatoes and fruits might help me feel full longer.

Vets, please help. I'll be three months out on Friday. I had never really lost my hunger and was completely ravenous from weeks 2-11. Suddenly I have zero hunger whatsoever. I only eat because I know I have to and when out to dinner with my boyfriend. I'm on the Keto and modified Gaps diets (gaps because my son who has sensory issues is now on gaps and I'm trying this gross bone broth and sauerkraut mess, and Keto because it kicks me out of stalls and I don't crave carbs one bit on it). Is it the Keto that is suddenly suppressing my hunger or is it the sleeve suddenly starting to suppress my hunger like others have had all along? Like I said I was starving every single day since starting purée stage! Is it normal to suddenly lose appetite around this time? Will it come back again? Not that I necessarily want it back.. Thank you so much for this. Really explains everything. I do know the benefits of your body going into ketosis - which usually happens whenever you even start a diet. But yes, keto would keep that process going. You also answer one of my worries - is it supposed to be lifelong. I can see myself doing it for 6 months. But definitely not my whole journey. I also had gallstones as a child (which is CRAZY), so that worries me too. -

Just an observation, as I have no actual experience in your situation, if meat based fats are tolerable, it sounds like lactose intolerance maybe? Have you tried taking some pre-/probiotics? Have you been tested for gut bacteria or h pylori bacteria? The gut is a fascinating, complex, and annoying organ. If you are worried about getting enough fat to do keto there is coconut oil, avocado, etc. I realize this doesn't let you have beloved yummy dairy for the moment but maybe after you get some probiotics for a few months you will be able to tolerate dairy better. -
I didn't want to tell anyone. I didn't want it to be the topic of all my conversations. I don't want anyone to think they have to do anything different for me because my requirements are different. And I didn't want anyone to try to talk me out of it or say derogatory things about it. It was already bad enough that people would tell me I was fat and needed to lose weight- as if I didn't know. That would have given them an excuse o bring it up. Eventually, I had to tell my sister-in-law because we couldn't care for my mother-in-law that weekend so I thought she needed to know. When I told her she said, "I know. my husband told me. and my mother-in-law told him. " Turns out my husband had already blabbed it to his mother who can't keep a secret or even remember that something is secret. My husband just said, oh I didn't know you didn't want anyone to know. after surgery, my M-I-L kept asking "can you eat this?" several times a day. that got old fast. I still haven't told any of my friends except for one because we've been through a lot together. if people ask, I say I'm seeing a nutritionist (which they seem to accept) and if they keep asking I say its similar to keto- because it sort of is.

One year out! 157 pounds lost. I (generally) consume about 1400 calories. It does vary, but I wouldn’t have gone under 1100 in several months. I am pretty tall - 5 foot 9, but I do very little exercise. Like a bit of yoga and barre. I’m in my thirties. This is something that you have to play around with. During a stall, I freak out and drop to 1200. Then I might go up again. Ive has an excellent month (down about 10 pounds, which is great for me at this stage of the journey). Even so, I do think about dropping my calories down to speed things up but there’s no guarantee it would work. I also eat fruit as a snack and I would have to stop doing that. Also, I want to be able to maintain my weight. I can’t live forever eating only 500 calories a day. Its not just about calories though in my experience (I’ll brace for some people to respond with the usual calories in vs calories out arguement). It really is about what you eat. I avoid bread, pasta, too much processed food, high sugar food, milk chocolate. I do eat dark chocolate every single day. So although it’s a high calorie diet, I generally follow a pretty moderate Mediterranean diet. I have messed around with the Keto diet, but I like eating fruit. My body feels healthier. The bottom line is that you have to talk to your nutritionist and come up with something you can comply with.

I appreciate those giving positive advice. I'm a bit confused why gastric sleeve (pre-op?) are providing advise. I've had a lap band. I want advice from those who have had the same surgery as me. If you don't like the advice, ignore it, it's not your request for advice. Rather than to start some verbal war. I appreciate the person who told me what worked for them. I'm a RN. I know what I am eating is correct which is basically almost all Protein because I can't eat anymore than that. I tried Protein Bars, low sugar, I gained weight. So that means I need 1-2 Protein shakes a day, high protein low sugar especially since I am very active. I play an hour of singles or tennis drills. I alternate swimming 30 minutes or biking 5-8 miles 3-4 times a week. I eat around 1000/1200 calories a day give or take. I work to add more calories in my diet. I squeezed through my plateau and am slowly...slowly losing. It is very frustrating. Surgery Dec. 2015 and I am down since surgery 30 pounds, 40 pre surgery doing everything right. I have 90 to go. At least I've hit the under 100 pounds to lose. So to get through my plateaus I mixed it up. Right now more of the Atkins or Keto diet with I do anyways but without the carbs, that's pretty easy. Most people just shed the weight. I'm still losing it so slowly. Yes, I'm looking long term but I need something to get my body to get rid of this excess weight which is mostly around my stomach. I am down 3 pant sizes. My goal is soon to be down another size by my reunion.

Whole carbs or net carbs? How to break a stall?

I have never counted carbs following my sleeve. I associate that with keto/Atkins. I *do* count protein intake and total calories - every day. What I do find is that pasta/rice/potatoes/bread etc. aren't worth eating most of the time (unless I'm WLS masking, in which case I purposely use them to deflect attention). If I can generally eat meals that are protein first, veg second, carbs third then I am usually in the zone and happy. Best of luck OP!
