Actually I started in January 2013 with the simple Dietdoctor.com protocol. I started at 251 lbs and by October 2013 I was down to 178 lbs, (height 5'7") which I was able to maintain for 3-4 years with no major effort on my part. My high blood pressure normalized within months after startin this woe, my skin got better, my moods got better ect. I just followed the Dietdoctor's keep your carbs low and eat real food protocol. But as years got by my underlying binge eating disorder got the best of me and first I started binging on keto foods and soon even eating junk-food again and of-course I gained some weight. I found ketogains last summer and was able to lose 25 lbs in a month or two, but started binging again until I was back up at 224 lbs when I decided to have my sleeve in May this year. I'm a self pay patient and live in a European country. Now, 8 weeks post op I'm down to 187 lbs, my goal is to get to 154 lbs. Although my binge eating failed my life's so far best weight loss effort aka keto, I can't blame this on the diet. Obesity and food addiction is a complex disease. Thanks for clarifying. Low carb and keto are definitely not the same diets, and I wish they wouldn't be used interchangeably. Low carb can certainly make sense. Many WLS patients are low carb without really trying, just by eating protein first, veggies second and carbs last, as most of us are told to. I think your last point is one that is often missed. If you're losing weight, by definition you're in ketosis, no matter your carb intake. "Ketosis describes the metabolic state whereby the body converts fat stores into energy, releasing ketones in the process"

Vets, please help. I'll be three months out on Friday. I had never really lost my hunger and was completely ravenous from weeks 2-11. Suddenly I have zero hunger whatsoever. I only eat because I know I have to and when out to dinner with my boyfriend. I'm on the Keto and modified Gaps diets (gaps because my son who has sensory issues is now on gaps and I'm trying this gross bone broth and sauerkraut mess, and Keto because it kicks me out of stalls and I don't crave carbs one bit on it). Is it the Keto that is suddenly suppressing my hunger or is it the sleeve suddenly starting to suppress my hunger like others have had all along? Like I said I was starving every single day since starting purée stage! Is it normal to suddenly lose appetite around this time? Will it come back again? Not that I necessarily want it back.. Thank you so much for this. Really explains everything. I do know the benefits of your body going into ketosis - which usually happens whenever you even start a diet. But yes, keto would keep that process going. You also answer one of my worries - is it supposed to be lifelong. I can see myself doing it for 6 months. But definitely not my whole journey. I also had gallstones as a child (which is CRAZY), so that worries me too. -
I didn't want to tell anyone. I didn't want it to be the topic of all my conversations. I don't want anyone to think they have to do anything different for me because my requirements are different. And I didn't want anyone to try to talk me out of it or say derogatory things about it. It was already bad enough that people would tell me I was fat and needed to lose weight- as if I didn't know. That would have given them an excuse o bring it up. Eventually, I had to tell my sister-in-law because we couldn't care for my mother-in-law that weekend so I thought she needed to know. When I told her she said, "I know. my husband told me. and my mother-in-law told him. " Turns out my husband had already blabbed it to his mother who can't keep a secret or even remember that something is secret. My husband just said, oh I didn't know you didn't want anyone to know. after surgery, my M-I-L kept asking "can you eat this?" several times a day. that got old fast. I still haven't told any of my friends except for one because we've been through a lot together. if people ask, I say I'm seeing a nutritionist (which they seem to accept) and if they keep asking I say its similar to keto- because it sort of is.

One year out! 157 pounds lost. I (generally) consume about 1400 calories. It does vary, but I wouldn’t have gone under 1100 in several months. I am pretty tall - 5 foot 9, but I do very little exercise. Like a bit of yoga and barre. I’m in my thirties. This is something that you have to play around with. During a stall, I freak out and drop to 1200. Then I might go up again. Ive has an excellent month (down about 10 pounds, which is great for me at this stage of the journey). Even so, I do think about dropping my calories down to speed things up but there’s no guarantee it would work. I also eat fruit as a snack and I would have to stop doing that. Also, I want to be able to maintain my weight. I can’t live forever eating only 500 calories a day. Its not just about calories though in my experience (I’ll brace for some people to respond with the usual calories in vs calories out arguement). It really is about what you eat. I avoid bread, pasta, too much processed food, high sugar food, milk chocolate. I do eat dark chocolate every single day. So although it’s a high calorie diet, I generally follow a pretty moderate Mediterranean diet. I have messed around with the Keto diet, but I like eating fruit. My body feels healthier. The bottom line is that you have to talk to your nutritionist and come up with something you can comply with.

Whole carbs or net carbs? How to break a stall?

I have never counted carbs following my sleeve. I associate that with keto/Atkins. I *do* count protein intake and total calories - every day. What I do find is that pasta/rice/potatoes/bread etc. aren't worth eating most of the time (unless I'm WLS masking, in which case I purposely use them to deflect attention). If I can generally eat meals that are protein first, veg second, carbs third then I am usually in the zone and happy. Best of luck OP! -

I have to say that Jello-o SF chocolate pudding is my go to treat. I have also tried the Enlightened "ice cream" bars once a week (my nutritionist approved) which also has protein in it (they are low sugar not SF). I have also made homemade Keto "brownies" which are completely SF and were delicious. Texture reminded me more of a cheesecake than a brownie when refrigerated. If you'd like the recipe, I will be happy to share. -

Only if you can stick to it for life. It's more a lifestyle change, not so much a diet. I did it, and I lost 100 pounds. But in my case, I have PCOS and couldn't keep the weight off no matter how hard I tried. If you do keto and then go off, you will gain all the weight back quickly, and the more you start and stop, the less effective it is. -
Worth it or not?

Years before I had VSG, I tried the keto diet. Gained 7 lbs. in one week, so I went off it. I am almost 2 years post-op and do not use the word “diet” for myself, as diets have always failed me. I eat intuitively and so far, it seems to be working for me.

Keto Chow -- Good for Post-Op liquid diet?

** I HAVE NOT BEEN SLEEVED YET ** I am just researching different types of Protein shakes for post-op. I was on a ketogenic diet which did wonders for me. I relied on Keto Chow as a meal substitute, and I can honestly report that nearly every flavor I tried was delicious. Just like melted milk shakes but the net carbs were under 4 grams per serving. I still have two weeks worth of supply (they are a bit pricey--about $10 a day). Each shake has about 27 grams of protein and is about 400 calories per shake. The only downside I see is that you have to MCT Oil and heavy cream to make it nutritional rounded: that is to say, consuming three shakes met all of your nutritional needs per day. I am not sure if the heavy cream (which adds fat--key to keto) would be a bad thing post-op. High protein, low carb for life?

Low carbing as a lifestyle and not a diet is different. As some other people said, eating Protein first makes you a low carber by default even if you add in carbs at the end, you will get so few you are still low carb. I love low carbing, more specifically Iove eating Keto. I had issues with portion control before surgery so I would fail or stop losing. Now with the restriction, it is easy to stick to. Eating after surgery even though are tracking everything and some thing you don't eat, is not like dieting. It is a completely different thing, and I don't think you can understand the difference until after surgery. Your body and your desires are just different. -

Google can give a better answer, but your brain and body have two primary ways of processing food to energy. One is taking carbs and converting them to energy (you brain prefers this method as it is the easiest to take a carb and convert to energy) or having your body convert your fat into usable brain energy by your liver converting fat into ketones. Thus, your body runs off of your fat reserves instead of your food intake. But your body will only do this when carbs are not a viable option. So people who eat enough carbs to sustain their brain energy won't use their fat as a fuel resource. In lamens terms, you are changing your engine to run off of cooking oil instead of gasoline. By eating a higher fat diet, but low carb and moderate Protein. (Fats have to come from quality sources like coconut oil and avocado, not deep fried chicken and gravy) Atkins is basically a keto diet before they knew the science behind it. Thanks ladies ! It’s nice to find support! I’m 37 and just had a hysterectomy last year due to other health problems and everyone was ok with that! I continue to stress to both of them that I’m “SICK OF BEING SICK” lol .. but it’s true!!! I’ve made my mind up and I’m doing this! But it hard when my the person pushing me back the most is my spouse ( my mother is just bugging me about keto) but my spouse tends to bring up everything that could go wrong. It sucks to say the least. -
Hi! I am a type two diabetic and also have Intracranial hypertension, I’m 5”3 and 200 pounds currently( 255 at my highest) , I’m just at a BMI of 34.5 .. I qualify under these guidelines for sleeve surgery , and I’ve decided it’s what’s best for me. HERE IS THE PROBLEM, as soon as I told my spouse and mother they got upset.. they told me I’m “not big enough” or that I could just “lose it on my own, if I just work out .. do keto” .. it’s hard to hear and making me worry I’m making a wrong choice. Because I’ve lost 60 pounds on my own way back in 2014 they seem to think I can do it again.. it’s 2020.. I’ve been stuck at this weight 6 years! I’m sick of taking so much medication , and I need help.. but there is very little support.. so know I’m doubting myself.. I’m alright in the pre op process HELP!

I have personally been using the keto diet to lose weight and it's been pretty 'easy' on most days. Low carb means I can't enjoy the pasta and bread, but high protein and fat means I can still have a ton of other things. My normal daily diet, as of late, is usually a vanilla protein shake for breakfast, tuna with some mayo, sriracha, and lemon juice for lunch, and low-fat yogurt for dinner. Though, there are a ton of options -- these are just work for me and lets me hit my macros that I need. I have slipped up, but it's realizing that its one mistake and that isn't going to define your success in weight loss, is huge. You can take that mistake and continue making more, eating bad for the rest of the day, week, month, etc.. and then you will be right back where you start. The other option? Realize that you had a weak moment, don't get down on yourself too much, and move on with the RIGHT foods. You can do this!

I actually had diabetes when I started the process. My A1c was around 7.6 or so and my glucose levels were usually around the 160-170 rang when I would test. I was on 3 diabetes meds, a cholesterol med, a blood pressure med, 2 anti-inflammatory meds. Not only did having diabetes not delay things, having comorbidities actually helped speed up the approval process for me. I know you wanted gradual changes to your eating, and to a certain extent you can still have that. But it's important to start getting your mindset and habits changed at least a little before surgery. Once you have it, you have to make drastic changes pretty fast. Better to start now. I would start by eliminating soda and anything carbonated. Get caffeine from coffee (the kind you can make at home) using sugar free sweeteners and low/no calorie creamers (or almond milk) and tea (personally, I prefer tea). Swap out regular chocolate, candy, popsicles, pudding, etc to the sugar free kinds. Start lowering your carb intake and try cutting down on pasta, bread, potatoes, etc... There's keto versions of bread that taste fantastic. Look at the way the bariatric diets require you to eat (protein first, then veggies, then carbs) and try to plan your meals around that (protein helps you feel full longer). Practice getting in at least 64oz of water now so you can be used to drinking it a lot. You can still eat yummy foods, just change the way you do it. It's less jarring if you start the process now. -
Good Morning!! I am in the last month of my 6 month supervised diet. I have lost about 23lbs, but it has been a very long and hard journey. I went out of town for a week for work and gained 10lbs! 10!! How crazy!! I'd like to work on getting even better at my nutrition. I have seen a lot of people do a Keto type diet after their surgery. I was wondering if anyone has done Keto and kept going through surgery. Has it helped? If so, what are your macros, any website info you can share, any recipes would be awesome as well!! Thank you

I'm 5 weeks pre-op and get a lot of negative and insensitive feedback with this as well. I've been identical with you in being a life-long veggie gag reflex sufferer. I do fine on the diet from keto and my nutritionist and have no problem with no getting bored with the same things. With the veggie issues, my nut concentrated on slowly introducing veggies into my diet. I found a solution by drinking an 11 oz v-8 healthy greens drink daily. I also have added a half cup of broccoli and a leaf or so of lettuce. I've tried other things a bit at a time and will still keep trying to get things down. My nut ok'd the v8 healthy greens and seems ok with my efforts. She only worried about the fiber, which I've found plenty of with the zero carb bread from aldi, chia seeds and metamucil. Your nut will target this issue with you and a newer way to see food. I've lost 36 lbs since starting the program in Feb and still losing. My liver fast diet is in a few weeks but I already take protein drinks (also giving me fiber) and ready for that. I really, really understand but you can do this. Texture is an issue for me too but I find smoothies and drinks with veggies and limited fruits go down well. Oh, also avacado is my side friend for now too.

Like you I was struggling to lose the weight my doc required me to lose, then I tried Keto ...dropped 30lbs and kept it off til my appointment with surgeon last week & got my surgery date today! You can do this! Keep me updated! -
I’m with the give them a miss until you’re at or close to your goal team. Sticking to your plan will help you take the most advantage of your surgery. When you get closer to goal you may start to look for healthy substitutes for your traditional choices. Try air fried or oven baked not deep fried. Try vegetable based pastas (you can eat zucchini noodles & cauliflower rice from solid foods). There are keto friendly low carb high protein breads you could try later on. If your desire to eat these ‘non bariatric friendly foods’ is driven by emotions, finding a good therapist may be helpful. Recognising the causes of your emotional eating, finding ways to manage them better & identifying other means of coping with your emotional needs than eating, etc. will only help you be more successful with your weight loss & maintenance. A benefit of avoiding those foods while you’re losing is by goal you may actually reduce or lose some of your desire for pizza, pasta, & the other foods you currently want & depend on. Which will make it much easier if you chose to have the odd slice of bread or pizza, etc. now & again in the future. Personally, I don’t eat pasta (only fresh zucchini noodles), rice or bread & avoid sweet things & fried foods. They were all things I ate regularly & enjoyed but I don’t really miss them at all now. I was able to break my need for them & I was eating them for a lot longer than you’ve been alive 😉. It can be done. Exercise will only contribute to you losing a maximum of 20% of the weight you have to lose. Of course it has lots of other benefits so I’m certainly not saying exercise is a waste of time but don’t rely on it to loose your weight. But it’s really up to you & you discovering what works best for you. Good luck.

I ate substitutes sometimes (protein bars, protein cookies, protein chips, protein ice cream, fiber candy, etc) and the real deals in extreme moderation. It has become very easy to find the subs almost everywhere now thanks to the keto phenomenon so I stick to those if I need a dessert. -
I live in a small town and milk is fine for me. I sometimes mix it with cream to lower the carbs a bit. I think the only thing we can do is stick to it. If one thing in the keto diet doesn't work, I try another. We have a choice, find a way that works to lose the weight, or give up and never find success. There are a lot of excuses out there, and I've used them--too busy, too broke, too tired, My disability has taken everything else from me, I'm not giving up this. But they were excuses. Unfortunately, losing weight has never helped me feel better. I might have lost it sooner if it did. I feel the same, no matter what. I understand that isn't normal. And at my age and health, I just get feeling worse as time goes by. At least I know I'm not getting worse as fast as I could.
