I’m kind of in the same boat as you. I’ve tried keto and i do love that you get to eat bacon all day long but it’s not for me. My body doesn’t need all that extra fat and it also loves healthy variety so good carbs like beans and squash and lots of fruits are fine with me. i guess you could say i do the meditternean diet to an extent but i don’t like to view it as a diet. I eat what i want but i cook everything anf watch my portions, carbs and added sugars. I eat plenty of lean protein, fruits, seafood, veggies, almonds, beans, and lower fat dairy like fair life milk and lots of cheeses. I use things like grass fed butter, olive or coconut oil, but in moderation. I even do some meatless days. I love it because it’s not restrictive so i never feel deprived. My body physically feels the best when i do this, i have more energy, no extreme cravings, and it’s something i can stick too. Plus i feel lighter all the way around. I do add supplements like fish oil pills, and mct oil pills but that’s about it other than a multi. Best of luck to you. -
I am 5 months post-op and down 80 pounds so far. I am jogging on the weekends and hitting the gym for 4-5hrs a week. I am still slowly dropping 1-3 pounds a week. I feel as If I can do more and better if I focus more than I am on my diet. I am not following any certain type diet (low carb, keto, weight watchers...) but I do try and stay away from carbs as much as possible. I want to eat healthy and as clean as possible but still be able to not be tied to an app and a calculator. I am really bad at tracking my food via apps, to be honest I probably haven’t done this in two months. What are your favorite diet plans? Why do you like them? I have 60 pounds left to reach my goal and I would like to get there well before my 1 year point I am looking for something that can get me to my goal and that I can maintain after I reach my goal. Thanks for sharing your experiences.

Hey everyone. I know there are some who have started or gone back to Keto after having sleeve surgery. I just saw my doctor and am cleared for Stage 3. Is it too soon to start Keto again or should I wait until I am on Stage 4? Plus any recipes you want to share are great too. Thanks.

Short answer: It's pretty much completely up to you. Long answer: "Keto" means almost nothing in the bariatric community anymore, I feel... in my pre-op phase I would tell my RD about my diet which at that time included things like oatmeal with berries for breakfast, tuna on whole grain bread for lunch, banana and string cheese snack, burrito bowl with rice or chicken with rice for supper, etc. Definitely protein-focused but not really low-carb and certainly not keto. She would nod and smile and encourage me and then my notes would say "Pt is following keto diet and will continue to do so." What????? LOL! Edit: I found the exact quote I was thinking of: "Commended pt on her weight loss, meal planning, and will continue to follow ketogenic diet." Yeah all that bread, oatmeal, and rice is really keto! A keto diet to me has a very narrow definition of restricting carbohydrate intake to fewer than 20-50g per day with an emphasis on high fat intake - 70% or more of the diet coming from fat. Most bariatric plans recommend moderate- or low-fat (20-35% of the diet) so to me they are inherently not keto regardless of the carbs. But the word is used generally in our community to mean low-carb, high-protein. High-fiber, complex carbohydrates - and fruit - are good for you. Period. They are healthy foods that support your cardiovascular health and protect you against cancer. Plenty of bariatric patients cut them out entirely in the interest of low-carb living so that's certainly an option and in that sense your diet can mirror a keto diet. Personally I'm not willing to give up the health benefits of carbs, so the "keto" guideline of 20-50g per day is not something I follow.

Yes, indeed surgery is the key factor. One of the things that I have recognized over the years of being in this game is how little dietary style influences ones weight loss after surgery. Back in the good ol' days 20-30 years ago WLS patients were often advised to just "eat like you always have, just less...." and that generally worked quite well - for the year or so, give or take, that they were losing. Maintaining that loss was not so successful with that approach, obviously, as they never changed what got them into trouble in the first place, and they often regained just as they did when doing diets in the past, though usually somewhat more slowly owing to the restriction that they had. Back when low fat diets were "in", people did just as well with their WLS as they do today with low carb and keto diets being the big thing. The important thing is developing good long term eating habits that one can maintain forever, and none of the popular fad diets do that for most people, and they tend to regain when they get tired of the diet.

Thanks you guys. I have to supplement protein anyway because I am not a big fan of meat or dairy. I prefer dairy free. I was wondering because I knew I couldn't live with or stick to low carb or keto long term. I had heard protein and veggies first so thought I had to stick to a diet like the preop diet which I'm really struggling with. I don't feel satisfied at all. -

Yes, Next week already, it feels like time passed in a flash, I booked the surgery two months ago. The smallest I can ever remember being was 135 in high school too! I was in dance and it kept me in shape, m weight has slowly crept up through the years and after having my kids i packed on 20lbs each kid that just wouldn't come off. I used to be super active and then i tore all the ligaments in my foot and here came more weight. Now i also struggle just to stay under 200, i've tried everything including keto and it just comes right back on, with a vengeance every time. I told my family that I don't want to hear there opinions when they think i look weird if i do hit my goal weight. I know its going to be different for me because i haven't seen myself that small ever, even when i was at that weight i thought i was fat... if i knew then what i know now lol. I'm so ready thought to not feel all this weight on my knees and my back. That has been the hardest, not being as active because my foot swells under the weight and stress. Hope we can follow each others progress and be each others motivation! -
Need encouragement that not everybody gains back all of the weight they lost after gastric sleeve

I am a sleeve to bypass revision. When I had my sleeve I went from 260 to 220 within 6 months. I followed my program to a tee. I then developed extreme GERD. At the time I had no idea what it was but it got so bad that about 2 years ago I was burping up water and feeling gross from the GERD that I asked for a revision and got it almost a month ago. i honestly don’t know why I regained my weight back. No one knew I had the sleeve but everyone knew me as the dieter: I did Atkins, Keto, you name it. You would think I would feel full from Keto cause of the heavy protein, but nope. I felt no restriction. But I felt myself slowly losing the restriction as time went on. I had my sleeve for 7 years before revising. I have also seen where some sleevers develop GERD and the only food that seems to settle is the bad food. That didn’t happen for me but I have read it on some forums. -
Do you have to do low carb/Keto?

There is no reason to do any of those fad diets that you hear about online (Atkins/keto, etc.) and generally long term most do better without getting involved in those. Your sleeve works just fine on most any diet that you may choose, but the important part is what will you stick with long term that will allow you to eat sensibly and control your weight. I second the motion to discuss this with a dietician, particularly if you are new to vegan/vegetarian as there are nutritional issues that one needs to address - plant vs. animal proteins, iron, etc. It is difficult to do early on owing to the low protein density of plant proteins, but after a few months is quite workable. A couple of specific references to look for is Dr. Alvarez on Youtube and other social media, who is a Mexico based sleeve surgeon and is also vegan and helps many of his patients take that approach if they are so inclined, and Dr. Matthew Weiner, also on Youtube, who, while not stictly vegetarian, does advocate a veg first approach to his WLS and weight control patients. Both are useful in helping take a non-traditional WLS approach -
I’m not doing a full keto diet, but do have a focus on almost exclusively lean protein, which has worked really well. Adding in carbs now that I’m 6 months out and that has almost doubled my caloric intake (you can eat a lot more carbs).

I don't do well on a low carb/keto diet. I am willing to change what I eat but would like to go healthy vegan. Is that possible after the sleeve? Any vegans here? I don't mind supplementing with a plant-based protein for life if that is a question you all have!

The problem with keto as it was explained to my by my physician was the fact mpact on the liver can be bad and cause damage. So while you can adopt much of the keto principals going full keto was a going to be a problem if it was done for an extended period of time. It also worked for me in years pre op period but so did the diet that my dietitian had me on at the end.

i do low carb 50g or less daily but i don't do keto because the protein level is to restrictive for my taste. if you can meet your marco's and get your micro nutrients from those levels i don't see a problem. surgeons don't like anything that is not down the middle good for all cause they don't have time for one on one care so all the programs are designed to fit the avg. and cause the least problems. track carefully and get regular blood panels and be prepared to stop if any issues come up in your blood work. good luck and enjoy

Does anyone do keto? I find that it really works for me, but the dietition said they do not recommend keto. I'd like to give it a try after I go through all of the post-op phases of the diet. Any experiences will be greatly appreciated.

There are a few bariatric keto plans out there..... What I've been told is for keto do High protein 60 -80g or whatever your doc recommends. Moderate healthy fats and low carb and lower calories. Let us know how it goes. You could also have a keto rash from lack of carbs I had that and it was pretty annoying. My tongue was white too, which is another keto thing. It goes away as you start having some carbs -

Yes, you are correct in that most of us do the protein shake for breakfast. I am 6 months out at this point. I mix mine with coffee in the a.m. I feel like my stomach is smallest in the morning so actual foods is a turn off because I get stuffed after 2 bites. Protein coffee fills me up gradually from 6 am to 7 am and lets me hit half of my protein goals for the day early on. Lunch and dinner are usually the same meal because I like to meal prep in the beginning of the week to relieve the Whats for lunch/dinner decisions. I don't mind eating the same thing all week. I've found this amazing recipe for ground turkey and green beans that I eat 2 weeks out of the month. It's spicy, filling and I haven't gotten tired of it. I also eat this shrimp and broccoli meal in rotation. Now that it is getting warmer out I will use my grill to do some chicken and veggies for the week. Search pinterest for some keto inspired meals. I don't do full keto but the recipes are great inspiration. -

typical keto is low carb and low protein with high fat and that is more outside the most common WLS diet. If you looked up a low carb diet it would match up better with most of the plans given by the different surgeons. It mostly about cutting out the sugar and refined carbs. These tend to be the trigger foods for most of us. Most obese people have damaged metabolisms and sugars and highly refined and process foods play to most damaged parts of our hormonal system. Don't fear the fat but to much of anything can work against you. always remember there are essential amino acids from protein and essential fatty acids from fat that you can not survive without But there is no essential Carb. They just make you feel like you cant live without them and that is the very sign that you need fewer of them. good luck

As Creek noted, official diets are all over the map, but most have the common element of a certain protein minimum (typically 60 or more, depending upon the patient's need - smaller women need less, taller men need more,) and minimizing sugars or otherwise simple carbohydrates. Relatively few programs promote a keto type diet - those are typically adopted by patients on their own - and there is little general need for such diets but if one is into them, it won't do much harm (at least in the short term.)

Sometimes I think when people deny themselves constantly...they set themselves up for bad choices. I think you've been making some bad choices with the refined simple carbs. But I don't think the answer is to swing the pendulum all the way in the other direction and starve and create an unsustainable diet again. Something drove you to crappy choices. You need to find out what. If it's an emotional issue...therapy is a good thing. If it's a diet that's too restrictive and unlivable...don't go back there....do something new. Maybe try a moderate diet with some healthy unrefined carbs (beans/whole grains/fruit) and see if they will help control your carb cravings so you don't go overboard in an unhealthy direction with refined carbs. If I eat a carb restricted diet...I crave sugar like a lunatic. Makes me crazy. But if I eat healthy low glycemic index carbs.....I don't miss it much. I feel much better physically and want to exercise more. Different people approach this stuff different ways. Keto is the big trend and people swear by it because its' great for weight loss. But some people feel ghastly on it and it's unsustainable for them. Lot of ways up the mountain. If one path keeps getting blocked, maybe look at some of the other routes? My appointments usually started with weigh in and BP. The appointments were quick and I saw a weight loss DR and a nutritionist, and we spoke about refining my diet pre-surgery. Mine put me on a 100 grams of Carbs or less/day eating plan. No calorie limit - think Keto. I had to show that I was able to lose. I dropped 26 lbs in the 3 mos prior to surgery. I am still seeing my nutritionist but now it is every other month. I have Aetna Ins.

I have to say that Jello-o SF chocolate pudding is my go to treat. I have also tried the Enlightened "ice cream" bars once a week (my nutritionist approved) which also has protein in it (they are low sugar not SF). I have also made homemade Keto "brownies" which are completely SF and were delicious. Texture reminded me more of a cheesecake than a brownie when refrigerated. If you'd like the recipe, I will be happy to share. -

Gosh, at 6 years out I forget I had WLS. I had put on 30 lbs by drinking Starbucks Frapps. I was so angry at my gain. I had been totally embarrassed to see my primary doctor and did not want to be weighed. I finally started eating Keto and I love it. My weigh is falling off again. My husband has diabetes and is a type 2. He eats keto WOE and his blood glucose is down, his A1C has improved from 8.7 to 6.5. He is decreasing his insulin intake also.
