Actually if you use the keto calculator, you set your Protein goal first based off your lean mass. Then you set your fat goals based on how much you want to cut. You have to always base your protein off your lean mass if you want to keep your lean mass. The majority of your calories should be fat, for the entire day but you don't just ignore protein or your protein needs for your lean mass. -
Okay, I'm confused. (Not uncommon, but I thought I had this one figured out) I had my regularly scheduled meeting with my nurse at the bariatric clinic today. One of the things she commented on is that I'm not eating enough carbs... insert confused emoji here. I'm a type 2 diabetic for five years and had gestational diabetes before then, and somewhere along the way I had been told/read/somehow absorbed the idea that I'm supposed to be eating than less than 100g of carbs (net) per day. My nurse was shocked that I was trying to keep my carbs 'that low'. She wants my carbs to be 50% of my macros. I currently am striving for less than 35%. 50%, which works out to 200g (total) carbs per day on a 1500-1600 calorie diet, just feels like way too much. Am I completely off base? Have I been misinformed all this time? I'm not looking to go keto or Atkins right now, just keeping my carbs low enough to keep my blood sugars controlled and to lose the 10% that my surgeon requires. I don't get to see my nutritionist again until May. Any insight would be appreciated.

I'm there with you (day 6); I thought it was my diabetes but then I read about what's called Keto Flu or Carb flu--it's exactly what I've been experiencing. I've felt like death warmed over and last night I thought there is no way I can keep this up. Today is a better day--not as good as normal but able to function. Good luck on your surgery! Lisa

High fat is keto. Keto is 60-75% fat, 15-30% protein and 10--15% carbs. Otherwise w/o the high fat it is just a low carb diet. You will have to figure out what works for you and especially what works long term. That's it. And that varies by so many factors - how long you have been obese, how resistant your body is, how much sustainable exercise you do, your age, time, and on and on. Staying on super strict plans may work for some people, or strict with occasional treats, or some with mainly protein, all vegetables, and one complex carb a day. This is the journey - figuring that out, so don't get bogged down by trying to decide all that ahead of time. Follow your surgeon's/NUT's plan and take it one step at a time.

Actually Keto is high fat, low to moderate protein. Standard ketogenic diet (SKD): This is a very low-carb, moderate-protein and high-fat diet. It typically contains 75% fat, 20% protein and only 5% carbs (1). Cyclical ketogenic diet (CKD): This diet involves periods of higher-carb refeeds, such as 5 ketogenic days followed by 2 high-carb days. Targeted ketogenic diet (TKD): This diet allows you to add carbs around workouts. High-protein ketogenic diet: This is similar to a standard ketogenic diet, but includes more protein. The ratio is often 60% fat, 35% protein and 5% carbs. https://www.healthline.com/nutrition/ketogenic-diet-101#what-it-is If you don't have room for anything after you've eaten your protein, then that sounds like a pretty optimal post WLS diet. I guess I'm not understanding what you are asking?

If you're truly doing keto, then that isn't high protein, that is high fat/ If you are doing low carb, then that fits with most program's plans of protein first, then non starchy veggies, carbs last.

Anyone have chest pain after drinking?

This, and how. Exactly with the little burps and hitting the sternum. I'm 3 months post-op. I just accidentally had to buy a propel water on a trip and found the bottles it comes in to make things much easier for me, maybe the nozzle slowing it down while I drink. I do get in my 64 oz but I used to drink 96 oz easily daily and it was no problem. Now, however, I really have to stop myself from drinking too fast. With me, times are different too. I don't know if it's because I move around more and exercise first but morning is the worst for me until mid to late afternoon. Then it seems easier to just keep drinking in small amounts. It can take me hours to drink my 16 oz and major chest pain, no matter how slow I go, until past 3 or so, then I can take the same in about 45 minutes. Cold, or ice cold, seems to do better for me with drinking but keto ice cream or ice cream gives me amazing pain for about a half hour after even if I melt it down. If I freeze my water, it goes down easier but this really varies per person. Don't stress yourself too much with this-things do even out more as you go. -
I have relost a 40 lb regain since November using Keto. it was fairly easy but I'm still working toward goal, and actually facing a possible revision to RNY because of persistent GERD. I'm 4 years out VSG and would do it again in a second.

This YouTube by Dr. Jason Fung is one of the most fascinating and interesting theories on obesity that I've come across to date. I don't post this in order to persuade people to go "low carb" or "keto" or to being IF (intermittent fasting). I'm posting it because it explains pretty logically how we've developed the disease of morbid obesity and why insulin resistance and diabetes are a pandemic. He also gives a couple of hints about how to reverse it (in practice), but he said the longer someone has been obese, the harder it is to lose the weight and keep it off. So, uh, maybe we are victims of hormonal shananigans? What if the food industry, WHO, et al, in partnership with our hormones combined with our gut-brain axis HAVE conspired to get us and keep us fat? Will becoming aware of our conditions and the "cure" cause us to maybe take a second look at how we view food and "treating ourselves" with small portions of "everything in moderation" when it's the exact same food that led us to becoming obese morbidly obese the first time around? Is the answer in changing our relationship with food--awareness? What do y'all think? -
Hi Sleevers- We see many posts regarding weight loss stalls here. I am curious to know if most of you who have experienced or are experiencing a stall, believe that this is just a response of your body or do you believe it relates to changes in the food you are eating. Having been on the Atkins diet for many occasions in my life, and having loved it, the best part of the diet was to use Keto-sticks as a measurement device each day to know if your body is in a state of ketosis, which means you should be losing weight. Since the primary diet for weight loss surgery patients is essentially the same Atkins diet (high-protein, low-carb) our experience should be very similar to being on the core Atkins diet. My question to those of you who have problems with weight loss stalls, is if you are using the Keto-sticks to prove that your body is still in a state of ketosis. Alternatively, do the Keto-sticks show that your body is not in a state of ketosis? My interest is to find out if those of you experiencing weight-loss stalls, is your body still in ketosis, and the weight loss has paused? Or, is the situation that your carb count has risen, and your body is not in ketosis, and that perhaps is the reason for the stall? If you have experienced a weight-loss stall, and have been using the Keto-sticks, please respond here and let us know the situation. Is it your body, or is it what you're eating? -RC-

My husband has diabetes and is a type 2. He eats keto WOE and his blood glucose is down, his A1C has improved from 8.7 to 6.5. He is decreasing his insulin intake also. As a Type I diabetic who is on an insulin pump, I can attest to the fact that insulin makes you hungry. The more carbs an insulin-dependent diabetic eats, the more insulin we need to inject. I am leery of the Keto diet. Carbohydrates are not eaten, so the body doesn't need to produce as much insulin, but doesn't skipping breakfast keep the metabolism in sleep mode? (burning less fat) Anohter issue I have, the amount of saturated fat in this diet is obscene. Definitely not lean or heart healthy. All that being said, it often causes people to lose weight, the problem arises when carbs are reintroduced and saturated fats remain in the diet. Sorry to sound so critical. I just know a lot of people on this diet, and I sometimes worry that some of the science is junk science. -
Perhaps look into the keto /low carb /sugar free substitutes so you can still have a little flexibility and feel no guilt when you incorporate the items. Not sure if it applies but diet fatigue and burnout is real and sometimes you have to fight the battle. Good Luck 💚

We both had the experal too ! Wrong spelling I’m sure 🤷♀️. Those first few days on the low calories you do feel so drained and you’re hormones are nuts and you’re going to feel better I promise but I know it’s hard to believe right now. I have a friend that calls that feeling the Keto flu lol. But I can’t remember exactly what day post op it was but FINALLY my body adjusted to the low calories and it feels normal now. Granted you get tired faster during activities than you would on higher intake but your day to day activities feel a lot more normal. I feel ya on the poop probs mine are far apart too. I take stool softeners to keep it from getting constipated type pain but still lots of days in between. we took vitamins from day 1 except for calcium we start that Tuesday at 1 mo PO. Dont apologize for venting this **** is HARD and a huge adjustment and sometimes we all just need a moment to whine! The metal cravings were BRUTAL for me at first. Downright depressing and seeing other people eat made me want to cry. That part gets better too for me it was around 14 days post op. Not saying I don’t have cravings anymore but not like before. Now it’s more like hey I’m down 37 lbs, I’m in ketosis, I’m killing this, is this thing I’m craving truly worth going allll the way backwards? I’ll try to talk myself out of it and have something else. Craving pizza? For lunch I might do laughing cow with a couple pieces of turkey pepperoni. Stuff like that. Or sometimes I just need to whine to a good friend and get it out! We’re all in this together ! -

Perhaps try structured eating first (set meal times) then find a diet style you like (wholefoods, vegan, Mediterranean, keto, etc) then one by one substitute out low quality foods for higher ones (milk chocolate for dark, reg. icecream for halotop, rice for riced cauliflower, sugar for a zero cal sub, etc) until you can fully convert back to where the practice wants you to be. Well, we went on that last nature walk and it was NOT supposed to rain but in the middle of it, we got hit with a deluge of rain and wind (rain coming in sideways). Made it back to car after what felt like forever, completely soaked. We managed to walk 1.7 miles that day and it may have been more if it hadn't been for the rain but that's for another time. It's been 38 days since surgery now and I've lost 19.8 lbs. I'm made peace, at least for now, with the rate I'm losing as I was a very slow 'loser' prior to surgery (did keto for 15 months) so I have to remember without this surgery that I definitely would NOT have lost 19.8 lb in 38 days. Yes, in my first month or two of my 15 month keto journey I did lose a LOT...close to this same rate, but that was probably at least 10 lbs of water weight and that pace was not sustainable. Current weight loss rate is roughly .52 lbs a day which is actually pretty great in my mind. I do feel STRONGLY that I have to keep up the physical activity multiple times a wk. And I know there are more stalls in my future & even gains as I continue to walk with my daughter and am working on a plan to join a friend at a local gym to walk an air conditioned track with her and also to have access to other machines/weights/etc. For now I just wanted to give an update. The 'vent' that I left here the other day helped me to get things out so I could start breaking past the negativity that was going on in my mind and thinking. Thanks for the support. -

Haven't been doing well the past 5 weeks..no weight loss. Letting calories sneak in during the day. COVID is driving me nuts especially with a spouse home 24/7 eating carbs and many trigger foods that they used to not eat in front of me. Getting back to business as I still have so much to lose. Down 175 lbs from highest weight ever but lost 60 lbs of that in 3 years doing keto off and on before surgery. My 1 year 'anniversary' was yesterday (7/25) so from the date of surgery I lost 115 lbs. If I had stayed on track, that # could easily have been 10 lbs more but that is not something I can stay hung up on. -
I am 11 months post op gastric sleeve. My highest weight ever was reached in June of 2019 and I was horrified to weigh in at 502 lbs! I didn't believe it and requested the doctor to re-weigh me. That was my first wake-up call. I was suffering from very high blood pressure, debilitating back pain/spasms, knee pain and damage, sciatica, and sleep apnea. I started doing keto and got down to 415lbs. During COVID quarantine, I gained back up to about 459lbs. In July of 2020, I finally decided to take back my life and financed a vertical sleeve gastrectomy surgery (out of pocket, since insurance would not cover it). As of yesterday, I'm down to 275 lbs. I have gone from a 6xl shirt to a 2xl. From size 62 pants, to a size 44. I still struggle to see myself as anything but fat from time to time, but it's the Non-Scale Victories (NSVs) that keep me going! My only regret is not doing this sooner. Sometimes I wish I could go back and re-do certain life experiences without the extreme morbid obesity. Here's my running list: - I’m off of all medications (including 2 BP meds). - No more seat belt extension on airplanes - Seats with arm rests are no problem. - Restaurant booths are now no problem - I can fit in an MRI machine (if needed). - I can now walk ANYWHERE without debilitating back pain. - The car steering wheel no longer rubs my stomach. - I can shop in most “regular” stores for clothes. - I no longer worry about a seat being able to hold me (those plastic white ones at weddings are even ok now). - I can use my own step ladder to do things around the house like change a light bulb. - I can sit with my legs crossed. - I can now get down on the floor. - My “love life” has drastically changed/Improved. - too many to list! Notice there is a lot of use of the word “CAN” here? All in less than a year. The little black battery operated box on the floor cannot control me any longer! Yea, it’s still important. But weight is no longer the defining characteristic of my life. Sleeved 7/23/20. HW 502, SW 459, CW 275, GW healthy

Banded 10 Years Ago..... considering revision surgery

Hey guys I really need some help from "real people with real experiences", my weight is increasing despite having a lap band in and I need to do something to stop it in its tracks. I have been dieting forever but this year I have done about 7 different diets, from Keto, SW, Calorie Counting etc and I am now heavier than I was on January 1st and what worries me is what my weight will rise up to if I don't do something. My history: - Starting Weight before Lap band: approx. 17 stone (size 20) - Lowest Weight after Lap band: approx. 10 stone (not for very long, comfortable weight was about 11 stone) (size 10/12) - Current Weight: 13 stone (steadily increasing weekly) (size 14) - I had a gastric band in April 2010, everything was fine overall, but the last few years haven't been so good, I developed a pouch in oesophagus so I only have 6.25ml in my band, this isn't really providing me with much restriction at all, I do have a little reflux however. I have also suffered with constipation for about 20 years! As I said above, I really need some help from "real people with real experiences", I am considering what my options are, revision surgery from the band to: Sleeve, Bypass, Mini Bypass or anything else? I have spoken to a few surgeons this week and they have all given different advice, so it is even more confusing. My questions are: - Has anyone had a similar experiences and what did you decide to do and was it the right decision? - It worries me if I have the sleeve or a bypass, will I be in the same situation in 10 years time? - Would anyone recommend a particular company or surgeon? - Has anyone had the band deflated and maybe had the balloon? I don't know if this is even possible? I thought this would mean surgery wouldn't be needed? Any advice would be welcomed, thank you all for your time 🙂 -

May be it’s the type of carbs? From purée stage I was allowed carbs: rolled oats & vegetables (not potatoes), then fruit later on. So instead of saying ditch the carbs I should have said reduce the carbs & look for more nutritious complex carbs like in multi & whole grains (low processed), seeds & vegetables. If you’re able to eat bread & pasta on your plan look for the low carb, high protein & multi grain versions. And of course leave them to last when eating: protein first then vegetables & then carbs if you can. But maybe it’s a case of discovering what works for you and it may be reduced carbs or only complex high fibre carbs. Ketosis didn’t last for my whole weight loss phase, just in the beginning. (Thank goodness - those bad body odour, bad breath, keto flu, etc. side effects are awful.) You don’t have to be in ketosis to lose weight. That’s just one way. Continuing to be calorie deficient will enable you to lose weight. I haven’t eaten pasta, rice or bread since before my surgery three years ago. I go out often including to Italian & Japanese restaurants & haven't felt like I’m missing out yet by not eating rice or pasta. Plenty of other options on the menu. As for bread - I have 7 specialty bakeries & patisseries within a 2 km strolling radius of my home. Don’t feel I’m missing out with them either. It’s just how you look at it. But that’s me. You have to discover what works for you. -

Thank you for the support! I appreciate the ideas and support. I think what I'm really whining about is that have to get rid of carbs, when I'm doing everything else right! I do walk as much as my body allows, I have physical issues which keep me from doing 3 miles a day like I'd like to. Still I'm eating very little compared to the average person. I walk everyday, plus weights, yoga, etc. I eat a small amount of carbs and throws me into non keto and I stall. I guess my body doesn't care about all this, it is what it is! So I've gotten back into just healthy eating, lots of protein! -
Weight Gain 1 Year After Sleeve depressed not enough weight loss

I’m feeling a little depressed of lately and have been eating my emotions away.. I’m 1 year post gastric sleeve i only lost a total of 70lbs i was hoping to loose at least a 100 or more based on most people i know lost 100 or more. I only lost 70 but i know why because I’ve been eating off way off... In addition i gained like 6-7lbs i was 268 now I’m 274.i started at 243 I have to get back in track i got really excited that i can eat and been snacking on a lot of nuts plus i wanna work out more haven’t done it consistently since surgery.. i also wanna do the keto diet have anyone tried keto or intermittent fasting after gastric sleeve... I know i ‘m my own downfall when it comes to my diet!!!! I’ve been thru so much with surgery and complications i dont wanna gain it back i need some motivation and some inspiration... Feeling so defeated -
It's hard to know what's working when you keep changing several of the variables (food, exercise and meal timing). One week is not a longtime and depending on how far out you are, we lose much slower now. To shed water weight, I go ketogenic (less than 20 g of carbs a day). I can drop 5 pounds of water weight that way. Long term though I struggle to stay Keto, I feel better at around 30-40 g of carbs. Slower loss but more doable. I have to remind myself that there was a time when weighing 150 lbs (or anything under 200 lbs) seemed like an unattainable fantasy. Being a few pounds above goal? I have had worse problems and I was ecstatic to be at this weight during my first year post op. Enjoy your cruise.

Yeah getting that water is hard, kind of like a full time job! But I will help! The reason I asked about the all protein thing. Was because people that eat a very high protein diet with very little carbs will fall into ketosis. Atkins like diets can cause "keto-breath," its caused when excess ketones in the bloodstream. one chemical component of ketones, the odor is accompanied by a foul taste in the mouth. When the body is in ketosis, urine may also take on a peculiar smell. The body's capacity to store glucose is limited, so when its reserve has been exhausted by fasting, skipping meals or strenuous exercise, it switches into ketosis mode even when you aren't following a ketogenic diet. The presence of ketones in sweat may give it an acrid, ammonia-like odor. Upping your carbs a little bit might help.
