No judgment, Glad you are asking for help Gains can happen to any of us. (Iβve had one) Weight loss mode and maintaining is work. None of this is easy, right? Reality is its not going to be easy. But you can do some things to turn this around. Therapy and counseling may help you address your relationship with food, why your discipline is lacking and getting back on track. Your surgeon can talk to you about all your options. Doing this on your own or with help is a mental battle. Except that you will feel crappy and have cravings while you detox off the extra calorieβs carbs and sugar. Many go back to bariatric basics, keto paleo vegan. Log your food is the only way you will know if you are eating your weight loss calories and macros. Joining a weight loss challenge can help you stay on track. Keep us updated. Your battle with this is going to help others dealing with the same situation. Many come here looking for solutions and then disappear from the site. I hope you stay around and give people insight from your experience. -

Wow really interesting! Very much so because they site that butyrate (the ketone) produced in the gut in response to fiber consumption is the same ketone produced by the body in response to a well formulated ketogenic diet. Only in a keto diet, we may actually produced MORE butyrate in and for the gut! -
You've done great so far. Weight gain is a sign that we have fallen off track and leaning back on food for comfort. My highest weight was 278 but my surgery wait was 237. I've only lost 50 lbs with my RNY tool. I messed around and gained 10 lbs back after a year and a half but I'm still determined to lose these last 35 - 40 lbs. Right now I'm working towards intermittent fasting and working to implement Keto. It's probably easier for me than others because I've developed so many food allergies throughout the years that I can't eat a lot of foods anyway. I continue to go to Weight Watcher meetings because I still need the support of people who still struggling with their weight loss journey and I need the accountability. I've lost 5 lbs so far. I've tried going to a bariatric group held by my clinic but people were afraid to be honest in front of the nutritionist and counselor. A few weeks ago I showed some before picks I took to someone and they asked my what made me take pictures facing forward and sideways. My response was "I always knew that I would lose this weight by any means necessary". Don't let others defeat your goals because they matter to you. Be well.

No, but reading your link led me down a rabbit hole of discovery. That is, that being keto seems to confer the same gut protective attributes as eating a high fiber (insoluble fiber) diet does. And to get my 1lb of veggies in from Dr. Weiner's experiment yesterday, I had to get 50g of carbs in (whole/non-net) which means, I wasn't even blowing a 0.15 on the ketone meter this morning. So clearly that's too many carbs for me for now. And I know that it's more important for me to be in a state of ketosis in order to keep my insulin and blood sugar levels low--from a cancer recurrence perspective. Sooooo. I'm gonna go back to their recs of having no more than 30g of whole carbs per day and see how I do on the keto meter. -

@Orchids&Dragons So for the WFKD (Well Formulated Ketogenic Diet) the reqs are pretty narrow. They confirm the limit of 30g of WHOLE (non-netted) carbs per day which is very limiting--in order to become keto adapted and stay in NK (nutritional ketosis). I know I'm not staying above my 0.5mm/dl of ketones on my meter. Some days I'm there, but others I'm more like .15-.25 and I'd like to be over 1 or 1.5 ish. So far fasting isn't getting me there by the end of the fast. So that means I'm def still insulin resistant and it's time I address getting into and staying in NK. So sigh...sorry Dr. Weiner--I'm gonna have to put that on hold for now... But here was yesterday's DD (down day of 500cals +/-100cals). It was pretty easy--a CRAPton of volume--too much really. Today is an UD (Up Day -- which I'm gonna use Dr. Phinney's formulation to get to about 1100cals for the day. I'll post that tomorrow as close as I can stick to it. It only has a measly little 6oz of veg for the day. Still haven't made it to the store, so I'm using things from the freezer and the few veg left in the fridge. LOL. Ugh! I am not gonna put my psyllium on the days any more. I MUST have it to poop. I just do. But I refuse to use 50 cals up of my alotment or to use up 12g of my carbs out of the 30g. If that keeps me from getting to ketosis, well that's just the way it's gonna be. But I'm not including it in my daily totals--magical thinking I know. Ha! -

Oh wow! Great article and website. I didn't realize the Dr. Phinney site existed! And mind blown about the explanation about BHOB and SCFA in the colon doing what fiber does for fermentation in the colon (gut biome health)! So our keto needs are different--I don't need to pound down the veg to get the fiber or nutrients. I have been doing this for almost 19 years, and yet, my mind still gets blown daily with info I've missed or maybe glossed over previously! Wow. I went down the Virta rabbit hole after that article to read more about protein requirements. This is a great chart. They use "Reference Weight" or "Ideal Weight" to calculate protein requirements cuz it's based on lean body mass rather than weight that includes fat. So they base this on each ounce of protein has on average 7g of protein/oz of weight. They figure their recommendations based on 1.2-2g of protein/kg of reference weight. https://blog.virtahealth.com/how-much-protein-on-keto/ Here's an example of what that would look like for someone who requires 14oz of protein per day for their ideal weight. (BTW, they differ on their protein reqs from Dr. Fung. It really depends on the program how restrictive your pre-surgery diet is. In my program, the surgeon explained that it's primary purpose was to shrink the liver to make access to the surgery site easier. They have to lift the liver to work underneath it and a fatty liver is more fragile and susceptible to damage during the lifting. You are helping to ensure the least possible risk of complications during surgery by following your pre-surgery diet to the letter. The pre-surgery diet is essentially a keto diet made simple through the use of very low carb shakes (in my case, Optifast 900). As your body goes into ketosis, it burns fat stores for energy. Physiologically, you body burns fat from the liver first as it's easiest to access. Ketosis is an on or off thing, and when switched off, takes a day or two to get started again. If you provide your body with any other carb source (simple or complex), ketosis gets switched off. The salad wouldn't make any difference, unless there was cheese or fruit or dressing on it. The chilli however, while usually a great healthy choice, contains lots of complex carbs from the beans and would have stopped ketosis. On a keto diet, you can not have any legumes for this reason. Not to be a stick in the mud, but every single instruction you will receive on how or what to eat has a reason behind it that will contribute to the best possible outcomes from your procedure. It sounds harsh, but each time you cheat, your outcomes get a teeny bit worse. Good luck with your procedure - you're going to do great!

Looking into keto, but can I just say thank you deeply to keto dieters and their Atkins forebears for the seas of protein rich products I now get to choose from at the store other than just shakes, powders and meat. When I think what it must have been like in the olden days with no choice and no useful macro nutrition food labelling Thank you for your sacrifices :-) On a more serious note will be watching this space as always hover at percent Fats/Protein/Carbs roughly 20/35/45 with 750-850 kcal per day and losing c.1kg p.w., but it's VERY early doors and may need to change things up HW: 263 SW: 255 (21 Jan) CW: 229 Sent from my ONEPLUS A3003 using Tapatalk

I hear you on this! Like you, I've told very few people about the surgery. I have had a couple of social events that couldn't be missed - a retirement lunch for a work colleague and a funeral for a friend's dad. Both during the pre-op liquid diet. At the lunch, I found wonton soup on the menu and ordered that without wontons. Other than a weird look from the waitress, that one flew. Funerals around here are big food events though and friends make trays of sandwiches and cookies and everything under the sun. It was hard to make excuses for that, but I just said I was on a crazy shake diet and couldn't eat anything. I'm finding there's enough people on the keto kick in my social circles that there's almost always someone on a restriction of some sort. Bless your neighbour (and anyone else who loves to entertain) let me know how you make out with that one! -

One thing Iβve noticed is that food goes down easier as the day goes on. In the morning I need to drink at least 12 ounces water or else breakfast just wonβt go down. Also, I canβt eat nearly as much as I can for lunch or dinner. Iβm to the point now where I can barely squeeze in 1/4 cup food for breakfast and about 1/2 cup lunch and dinner. PROGRESS. yay I also make sure to drink 1 to 2 glasses fairlife milk daily to help keep up with protein. still havenβt even tried to eat anything other than protein. Last night my husband and I made a hamburger casserole from a recipe from keto daily. Some of their recipes can tend to be fatty, but we used 93/7 ground beef and half the recommended cheese and it turned out pretty good. -

The orthostatic hypotension is not unusual with rapid weight loss, and is one of several ramifications of your body still trying to function at your old weight when there is substantially less of you at hand - we often feel cold for the same reason, that the body is still trying to cool what is no longer there. These things do improve over time, though it may be a year or two. In the meantime, as the docs and others suggest, more salt, lots of fluids (non-alcoholic), and also exercise is also a common prescription to "keep things pumping" On the exercise front - this should improve things, but let your trainer know that you have this problem so they can watch for it. Also, another side effect of the rapid loss is that your heartrate will likely be lower, at rest and in activity, so if the trainer is pushing to achieve some specific heartrate to indicate some level of exertion, (often it is 80+% of a theoretical maximum rate) their measurement scale may be skewed and they may push you harder than they should. When I was 2-3 months out, I noticed that exercise (brisk walking short of a jog) that used to get my heart into that 80% zone (140-150 for me at the time) would barely get me over 100; even today, years later, my resting pulse is low, often 50 or below, and doctors/nurses unfamiliar with me will question it (yes, it's normal) though my BP is normal to a touch high. So, the exercise should be OK, but let them know and don't let them push you too hard - your cardiac system may have excess capacity, but you may be evertaxed elsewhere. Your ketone levels have little to do with whether or not you are losing weight. They are in indication that you are burning fat. but that can be ingested fat as well as stored fat. If you want to burn your stored fat, you need a good caloric deficit, and it matters little what style of diet is used to accomplish that - low fat, low carb, keto, paleo, Atkins, whole 30 or whatever it may be. I used a relatively high carbohydrate diet (by Atkins/keto standards - 100ish g/day) and lost quite rapidly - because of the fairly high caloric deficit; ketones were there in the blood tests because I was burning my fat stores, but those readings weren't a goal. The high fat/low carb type diets a la keto and paleo are popular these days, but the high calorie levels that often accompany them can sometimes make weightloss difficult. Clinically, high fat/low carbohydrate diets are often prescribed to avoid or minimize weight loss after a non-WLS gastrectomy, so if you are trying to use such a diet for weight loss, you have to keep a close eye on calorie levels. Being four months out, you should still be fairly restricted in you eating volume so that you should still be losing at a reasonable pace, but if you are ingesting too many calories by "eating around" your wLS - drinking calories or very high calorie foods - then you can see low or no loss at this point. -

I feel like Iβve just been told off! Being smart before you let someone cut out your vital organs is a bit of an obvious point, but granted, you donβt know me, I COULD be an idiot ,so thanks? However, I assure you, iβm not. In your defence,i donβt think I was very clear, the 2-3 weeks timeline is AFTER youβve provided all your Medical history and had evaluations. It not just cut and cheque and a cut a stomach. They have told me the barrage of tests heβd need to do first, including blood test, group blood test and rh factor, biochemistry analysis, (ALAT,ASAT, bilirubin, cholesterol,HDLP,LDLP, triglycerides, INR, APTL, prothrombin, urea, creatinine, (Na+,K+,CL-, HCO3, Ca2+), TTH, glucose, total serum protein, chest x Ray, fibrogastroduodenosvopy with anaesthetic, heliocobacter pylori test, ultrasound, ECG and Hep C . A lot of which would be performed at the clinic, if youβre not suitable for surgery when you get there, they return your funds minus a deposit to cover the travel/accommodation of getting you there. I think your own experience has made perhaps made you a tad prejudice to other processes. Irresponsible and reckless are strong words. I understand the process is different in the US but when youβre paying private for anything, things move quicker, there are fewer stages and hoops to jump because youβre not costing anyone else anything. If someone want to claim a major surgery on their insurance, obviously the insurance company isnβt going to make it half as easy for you as they would a private paying client. This why I need to make sure Iβm doing as much research as possible to know Iβm making an informed decision. Including the advice they give you for preparing before a surgery. As as per the water comment, I meant with meals-I didnβt specify that, so my bad. Iβm not the biggest drinker in all honesty though, but Iβm aware of the requirements and Iβve done plenty of keto diets before that require a LOT more than the recommended 1.9litres a day that Iβve seen and managed just fine. Someone pointed out protein water as an option too, because I struggle with the texture of shakes, so thatβs an extra incentive to drink. i know Iβm out of other optoins. βOne last attempt to do it myselfβ for me, was at least 2 or 3 attempts ago. I donβt need a huge diet before surgery , just need to eat healthily for healthβs sake because my BMI is 35.8 which isnβt far from their minimum requirements as it is. Iβd say iβve made my mind up to be honest. Iβm going to follow the clinic over the next month, thereβs a few more people that Iβm speaking to that are going for surgery this week and theyβre going to be journaling their experiences over the next 2-4 weeks and if iβm Still happy with the regimen , iβm Hoping to call and book this summer -

Well I am still only making it 24 hours. GRRRRR! I want to do 36 so bad but by 24 hours I just want to "feed" and "feed" and "feed". LOL. So hmmm, there is that. I'm sticking at 144.8lbs and it's not going anywhere! Gah! LOL. It feels like my body defends each new low and I will sit within a few pounds of it for weeks, then it relents and allows a new low. It's a bit crazy making. I'm trying to do a 36 again today. Then will be gone no IF for the weekend, but keto weekend planned. How are you doing? ((hugs)) -

I posted this in wrong forum! Moving it here: Am in a tough situation, as most everyone is. While I've been morbidly obese now for over 12 yrs, I had been able to get around and was working on adopting a Keto way of life, not just as a diet as I truly feel physically better when I eat that way. Unfortunately while I've been working on losing weight, it's been much slower than expected (I've hit that female time of 'life' and everything seems to have slowed to a crawl) - I've only lost 46 lbs in the past 6 months. I ended up tearing meniscus on 1 side of my right knee while exercising. I had surgery to trim part of meniscus that was hanging so it wouldn't get caught in knee when it bends. I also have history of throwing clots but I do NOT have a clotting disorder (like Factor V, etc.) I have a permanent IVC filter installed. My cardiologist recommended amount of Lovenox for me to bridge (for 1st knee surgery) to based on my body weight. Well, they don't make lovenox in that 'dosage'. The doctors decided to 'wing it' with the normal amount of Lovenox. Needless to say, I clotted again but I feel decision was made in error (i.e. cause & effect). Since then I've torn meniscus on other side of right knee and my back has started to spasm (diagnosed with herniated disc). No big shock as I've put my body thru too much stress with weight. My issue is I REALLY wanted to avoid surgery & truly wanted Keto as my WOL. The problem is the pain my knee and back are causing is now so severe I cannot take it anymore. I thought I had a big pain tolerance but seems I cannot handle 'chronic pain' as well. With my mobility limited until summer when pool is warm enough to swim in, I'm not losing weight fast enough to get to move on with my life and have the knee replacement my ortho is suggesting but will not do at my current weight.. So, I've been on these boards off & on for a few years, mostly researching & I feel that since I'm 50, I'm running out of time, especially with amount of pain I"m in. I truly want my life back more than I want food at this point and have felt that way the whole time I've been doing Keto. At 344 (down from 390), I'm ready for the pain to stop and I've researched until I'm blue in the face. I do know that I want to start down the road of interviewing doctors, my primary dr already was supervising my keto loss, I have a psych dr but am willing to do eval with another if they won't accept my own. I've discussed possibility of surgery with current docs for 2.5 years now. I was also diagnosed with melanoma recently and after losing my best friend to brain cancer, I certainly feel a bit of concern that since we do not know how many days we will be on this earth, that surgery is definitely on the table. I looked for a Bariatric COE in DFW area and only come up with Baylor Hospital in Grapevine; however, when I look at bios & reviews of the 3 docs they have, none of them inspired me with confidence. Anyone have a dr to recommend who has a long history of performing both sleeve and bypass with great results? I had considered combining lapband with my keto to see if that would help speed things up but looks like with my BMI, that may not be an option? I also have to carefully consider surgery options due to my clotting history. Ortho docs want to do knee replacement when my BMI is lower, until then, I just put up with pain. Any input is appreciated! -

I'm looking for a doctor who has a good reputation with lap bands. I realize the sleeve is the probably the "go to" surgery, but I've been doing Keto to lose weight and have lost 46 lbs in 6 months. I realize this is nothing compared to others. I am trying at all costs to avoid major surgery although I realize surgery is involved in lap banding. I have a history of throwing clots and even though I have an IVC-filter permanently in place, I am often looking for the 'least invasive' option when it comes to medical procedures due to my clotting history. I tore my meniscus on one side of my right knee and have now torn the other side. My ortho said I may need a knee replacement but until I get my weight down, that surgery is ruled out as well. I was hoping I could tough out the pain my knee is causing me (EVERY single day) until I lost enough weight doing a keto way of life (not a diet), but the pain in my knee is increasing and my limited mobility is starting to make me desperate. Does lap banding only work for those who have a little weight to lose (as I have more than a little to lose - I'm just looking for another 'tool' in my tool box since prior years of doing Atkins and other things have made me very sensitive/resistant if that makes sense) Any input is appreciated! -
Hello my fellow sleevers, congratulations to everyone on your one year Sleeve-rsary. Iβm glad to hear all the positive results from everyone. As for me i had amazing results iβve lost a total of 130lbs and i feel like a kid all over again. Is been a learning experience but for the most part i canβt complain. Quick tip for those who are dealing with stalls and not enough loss of weight do your reseach on the KETO diet. It has done wonders for me. I went from stalling to loosing 30 pounds in the past few months. REMEMBER TO ALWAYS CONSULT WITH YOUR DOCTOR BEFORE STARTING THE DIET.

I have been keeping up with several groups on Keto diet and there is information on intermittent fasting to promote weight loss. I have tried this ( fasting 16 hours a day and consuming all meals within 8 hours) but with the limited quantity I can eat at one meal, I have found that I canβt get all the calories in an 8 hour period. I donβt get hungry (a good thing) and will eat till βfullβ but I would have to eat every hour to get 1200 calories in 8 hours. This frustrates me because I know from past experiences that I go into starvation mode when my calories drop under 1000. Has anyone else tried intermittent fasting post sleeve?

I've never been slim. I come from a line of morbidly obese women, been on diets since I was 13. I've lost and gained the same 3-6stone countless times for 15 years. Slimming world, weight watchers, cabbage soup, cider vinegar, slim fast, Cambridge, slim and save, keto, lchf, starving, fasting, bulimia, fad diet pills, -you name it, in the last 20 years, ive tried it!!! π€£ Last year I've stuck religiously to slimming world and the gym only to maintain, I'm defeated. π @Xapphirea I hate fizzy drinks and struggle to drink much throughout the day so the fact that those are a necessity is actually a bit of a relief! I'm in the UK so woukd have to pay for my surgery. I've found a clinic that really stands out to me, in Latvia. I just looked and from the moment they ok you, and see if you're suitable, they aim to get you over within 2-3 weeks! Fortunately, I still need to save a big chunk of money before i can commit to anything anyway so itd be a good few months before i could book. I feel mostly excited about the prospect of doing this. Id do it tomorrow lol but I need to sir with the idea and broach the subject with my husband. I'm going to Rome with a budget airline on March 31st, I think squeezing my backside into those plane seats will make my mind up so thay gives me a month to process. -

Well I finally got the psychology approval which took the longest to come back for some reason and found out everything was submitted to insurance on 02/20/19.. omg I canβt believe how much Iβve been stressing over this.. It seems like thoughts of this take over most of my day.. Iβm trying my best to be positive and I am so looking forward to this new journey in life. Iβve even eaten all my fairwell foods cause I know after this Iβm done! Iβm working on a mental change now as I know this is a must in order to be successful. I can even tell the difference going from Keto to crap. Iβm excited to get back on track! I wonder how long this approval will take. Iβve seen so many different ranges. Iβm sure hoping itβs quick and even more hopeful for a yes! Iβll take a slow yes over a quick no! So sorry about the GB stuff. ((hugs)) But FYI, you can be in a "keto" state without doing fat. I just eat low carb and low fat. Works for me. If I eat high fat stuff, I get so sick. Miserable!
