    Approaching my 4-year surgiversary

(continued) Weight maintenance: YES, YOU CAN GAIN WEIGHT AFTER THE VSG! BUT YOU DON'T HAVE TO! About two years out, I noticed that I could no longer truly eat anything I want at every single meal even if I did work out quite intensely. Mind you, we are talking eating out constantly, Desserts, etc. I also became more performance-oriented, so I started to think more about fueling my body for exercise. Recently, I had a severe back injury that left me unable to exercise at all except walking for 9 months. I was also having major sleep issues due to pain and work stress and was away from my husband and very socially isolated and unhappy for a variety of work-related reasons. I love to cook and was certainly amusing myself by cooking lots. Normally, my husband would inhale most of my creations, but now I found myself eating less healthy and in greater quantities. Together with hardly any activity, this led to a weight gain of about 13 lbs in 9 months. Not the end of the world, but I was certainly worried! Thankfully, I buckled up and once I was cleared to start exercising, I restarted couch-to-5K and then signed up for a 10K race. I lost the 13 lbs in about 2-3 months with only minor diet changes - basically, eating less crap and more fruits and vegetables. I still ate some sort of treat almost every day. I didn't follow any specific diet plan like low-carb or keto, although I do try to limit my carb intake that doesn't come from fruits and vegetables. I am now closely monitoring my diet as I want to get as ripped as possible since I have lipo coming up and will be away from the gym again for at least 4, potentially 6 weeks. I eat whatever I want within reason, but I count my macronutrients, so every day I eat a certain amount of Protein, carbs, and fat. I eat a fairly high-protein diet - 120-150 grams. I have had no problem losing fat on this plan while eating 2500-3000 calories a day depending on my exercise level. SO YES, YOU CAN GAIN WEIGHT - BUT YOU CAN ALSO LOSE IT, AND YOU WILL ALWAYS REMAIN IN CONTROL! DON'T LET YOURSELF GO TOO MUCH! IT'S NOT HARD TO LOSE 5-10 LBS, BUT 25+ IS A PAIN! LAST BUT NOT LEAST: PHOTOS All these photos were taken within the last week. These are my pre-op photos:

    Worried

Well after a beautiful 2 years and 7 months relationship with my band I had to get the fluid removed for medical reasons. I am being told to eat enough to prepare my body for what is to come.. I am worried about putting the weight back on. I am seriously considering โ€˜ a Keto dietโ€™. Any advise. xx
  3. so after about six to eight weeks or not working out and just kinda sitting at my "goal ish" weight - i think i've decided to lose 10 more lbs then start weight lifting pretty hard. i need to tone big time. any advice from lifters on what they are eating now? (i've been full keto for months so carbs scare me lol) how to weight train? how not to weight train? help!
  4. After 5 months of strict Keto I'm feeling good, have lost all but 7 lbs of regain and restored my overall health. However, at the appointment with my surgeon last fall, he told me if the GERD still persists after weight loss, my only option is revision to RNY. I'm feeling focused and still have another 4 months before I see him again, but I'm still taking a PPI twice a day. I've had an EGD which shows persistent reflux, however no Barrett's or significant scarring right now. I've had GERD almost my entire life due to the way I carry my weight, in fact I had an upper GI series as a baby from such severe reflux! I've been on a PPI for about 15 years and really want to get off of it related to the longterm risks with the drug class. I'm a nurse practitioner so I am very familiar with the research on the subject. I guess this all boils down to the fact that I'm probably still looking at a revision. I'll do what I have to for my total health but really hoping that I can get another 15-20 lbs off before I see him and the GERD is resolved.
  5. The amount of info is mind boggling and yes, so much of it seems to often contradict--especially between surgeons! I love this place so far. Such great people here. I'm just a newbie too and am still pre-surg. My R/D told me in the beginning and maybe for the first six months to a year, that I will need protein drinks in order to reach my daily protein goals. She explained that the protein drink supplements would not be counted as a meal--they are extra to the meal. She made the suggestion of adding Premier Protein to my morning decaff coffee, or adding Syntrax Lemon Tea Protein Powder (which tastes like the old Lipton Powdered Ice Tea from the 60s) to my regular decaff brewed tea in order to get liquids in at the same time, I'm boosting my protein intake. She said it would then allow me to eat that 2oz of protein at a meal and only eat 3 meals without snacking. But, please don't listen to me about nutrition. LOL. I know low carb and keto and know about cooking, but I'm just barely a newb and am trying to figure out the whole eating pre and post WLS! ((hugs)) and hope we can get to be friends and cheerleaders for each other!
  6. Hey everyone. I am 5 days into my preop diet, and quickly becoming sick of everything. I have been making keto recipes for dinner with 1-2 carbs per serving, One protein shake per day (19 g of carbs) and only other carbs are in small amount so cheese or dressing. I was just told no sugar, and low carb. No fat limit. Looks like from reading on here every surgeon is different. I struggle most at work. I am a nurse and tend to get more hungry at work. A couple questions..... What is everyone using for protein shakes? I have been using GNC lean protein shakes (25 g per serv) Were you given a max carb limit? Drop some recipes in a reply you may have had and are yummy:)

    My breath or weird smell?

Bad breath is one of the most under-expected concerns that frequently develops after weight loss surgery. While it is not something that impacts every weight loss patient in the same way, it is something that should be recognized following your bariatric procedure. There are several primary causes that can cause bad breath to develop following your bariatric procedure. These include: * Dehydration * The inability of food to exit the stomach pouch * Ketosis caused by rapid weight loss While bad breath may not be as severe a health issue following surgery as other potential concerns, it is something that can interfere with your quality of life and general comfort level, and therefore is worth addressing. Luckily, bad breath is usually preventable or treatable by understanding the precise cause of the issue, and taking several basic preventative steps. Source: https://www.flowersbariatriccenter.com/patient-resources/health-issues-after-weight-loss-surgery/bad-breath-after-weight-loss-surgery/ Ketosis can cause dry mouth. One of the solutions is to rinse you mouth with Biotene Mouthwash. Here is an article about the subject. https://ketoconcern.com/dry-mouth-keto-diet/

    Two month long stall

I had the same issue at one year out. For three months I stayed within about a 3 pound range, and I still had 30 - 40 lbs to go. It made no sense to me as I was averaging about 1700 - 1800 calories a day, and as a still somewhat fat 6 foot tall guy who works out 3- 4 times a week, that's not a lot. I was always hitting 80- 90 grams of protein, but truth be told I had allowed junk back into my diet, even at my relatively low calorie range. I switched up my eating habits and am now doing low carb/keto, and the weight is coming off again. As I approach goal, I'll start adding in more healthy carbs.
  9. It really depends on the program how restrictive your pre-surgery diet is. In my program, the surgeon explained that it's primary purpose was to shrink the liver to make access to the surgery site easier. They have to lift the liver to work underneath it and a fatty liver is more fragile and susceptible to damage during the lifting. You are helping to ensure the least possible risk of complications during surgery by following your pre-surgery diet to the letter. The pre-surgery diet is essentially a keto diet made simple through the use of very low carb shakes (in my case, Optifast 900). As your body goes into ketosis, it burns fat stores for energy. Physiologically, you body burns fat from the liver first as it's easiest to access. Ketosis is an on or off thing, and when switched off, takes a day or two to get started again. If you provide your body with any other carb source (simple or complex), ketosis gets switched off. The salad wouldn't make any difference, unless there was cheese or fruit or dressing on it. The chilli however, while usually a great healthy choice, contains lots of complex carbs from the beans and would have stopped ketosis. On a keto diet, you can not have any legumes for this reason. Not to be a stick in the mud, but every single instruction you will receive on how or what to eat has a reason behind it that will contribute to the best possible outcomes from your procedure. It sounds harsh, but each time you cheat, your outcomes get a teeny bit worse. Good luck with your procedure - you're going to do great!

    eating out

I went to a Mexican place a while back and got a tostada. I just ate the toppings and left the tortilla. A lot of restaurants have started paying attention to the keto community, which is great because it means they have low-carb options. Jimmy John's has lettuce-wrap subs. Five Guys will serve a bunless burger (either as a lettuce wrap or in a bowl). Chipotle has cauliflower rice. Most chains have salads. Most sit-down restaurants have meat + sides meals (chicken breast, pork chop, steak, etc.) and will be happy to substitute any high-carb sides (rice/potatoes/bread) with veggies. You should be careful about sauces and dressings, though (get them on the side if possible, and ask for light dressing on a salad), because they can be loaded with sugar and/or fat.

    Diet questions

Keto tends to be high in fat and high in calories, the exact opposite of what the doctor wants you to do. Admittedly, it's low carb, but everything else isn't good. Also, Keto tends to work best for short periods anyway. It 'shocks' the body into working a different way. But once your body gets used to it the weight doesn't fall off like it does in the beginning. Also, most Keto dieters only eat once a day. That's not good for bariatric patients! I know a few people who do Keto so I'm basing what I know about it off of them and once they go off the Keto diet... the weight comes back. With interest. It's not worth it.
  12. I am 5'2" tall and 180 lbs, looking for an advice which procedure best and safest. I spoke to my family doctor and suggested the balloon but my worry there, can I maintain my weight after taking the balloon out after 6 months or gain back? I did Keto for 4 months and lost 20 lbs but gained back when I traveled somewhere for 2 weeks as I used to do exercise and cook my keto food. Until I slowly feel that my weight gained back and can't keep my keto routine and exercise. My family doctor says Gastric Sleeve surgery is irreversible and if there is an infection and something wrong or not for me, I can't put it back. And later on, there must be a serious side effect. And now I am here, I don't know what to do. Please drop advise and opinion and encouragement here please!!!!!!!!!

  14. A typical day begins with coffee, 1 tsp of extra virgin coconut oil, 2 equal blended. I use the Premier Protein shakes because they are keto friendly. I have nuts on hand, Macadamia, Brazil and pecans as a snack food. 4 oz of hard cheese are allowed each day. I use regular butter or vegan butter to make an egg or two, you are allowed bacon but I tend to break out from that type of fatty meat. You could dice ham into your eggs and green veggies. Peppers are also allowed. I use 1 oz of cheese in my eggs if I want. For lunch I make tuna, salmon or egg salad and eat it from the dish. I also eat salads with viangrette. Iโ€™ve had Cobb salad, spinach salad and portobello salads. I add extra hard boiled egg for more protein and I enjoy the taste. Iโ€™ve eaten fish at dinner with green veggies. My favorite is Brussels sprouts and butter. You can use Quest bars or cookies as a safe snack but try to limit them. I often have a second cup of coffee as made at breakfast for the mid afternoon. So far my blood work came back immaculate. I like this plan because it helps me with my hyperinsulinema from my PCOS. I donโ€™t spend the day constantly looking for sugar. If I do fall off, I start again at the next meal. It takes me four mealtimes to get the cravings out of my system again. If you have any questions. I can get you more information.
  15. Iโ€™m having VSG surgery September 4th at Pompeii Surgical in Tijuana Mexico, I donโ€™t have a Facebook and really no interest to create one and Iโ€™m looking for a sense of community or maybe some people who are having surgery near my same date? I have a weight loss Instagram (I did keto for a year so itโ€™s mainly from that, I am not telling them about surgery until post op) . Is anybody having VSG near my date or have experience to share at Pompeii Surgical?
  16. @@RJrocks I have a few links here that discuss the various mechanisms involved: On the process and mechanics of weight regain after loss: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4371661/ On the ever advancing field of epigenetics, or how gene expressions are altered by diet, weight loss and excercise: http://www.nature.com/articles/srep14841 http://clinicalepigeneticsjournal.biomedcentral.com/articles/10.1186/s13148-015-0101-5 http://m.advances.nutrition.org/content/5/1/71.full Regarding the claim that there is a 3-4 year period before your genotype adjusts towards that of a non obese person, I learned that from a podcast with a bariatric physician who treats 15,000+ patients per year with a ketogenic diet. I will try to find which episode it was, the podcast is called Keto Talk if you're interested.
  17. Hello, I am new here. I was approved for surgery last april but let friends and family talk me out of it, a year later and I am regretting it, my doctor has referred me again and I am anxiously awaiting approval. I so hope its approved again, but while I wait I am doing as much research as I can and have decided to start the pre op diet in anticipation. Is that crazy? I've been on this journey for some time and have lost over 100 lbs doing WW, keto and paleo over the past few years but I need to lose another 70 and its bein stubborn I NEED new tools. I hope to get the gasyric sleeve. Sent from my SM-G965U using BariatricPal mobile app

  19. Excess protein does not go to sugar and to get to protein excess you would have to be consuming like 250 grams a day for months. You should choose a way of eating that keeps you the most satisfied and that you can maintain for life. I eat keto but my protein grams are still higher than my fat grams because fat is higher in calories than protein. So yeah my calories from fat are higher than my calories from protein but I still eat more protein than fat.
  20. Dietician big proponent of low carb/ high protein. Dr big proponent of low card/high fat(almost keto in nature). Im thinking excess protein goes to sugar...so im leaning more to Dr side.what say ye?

Can I still eat Keto/LCHF after RNY or sleeve?

Hey folks...Iโ€™m still in the very early stages of preparing for a possible RNY in a couple months and have tons of questions. First being...can I still keto? For the past year and half, Iโ€™ve been following a strict ketogenic/LCHF diet. No sugar, no fruit, grains, bread, very low carb. Mostly high fat dairy, fatty meats and other protein sources. Basically avoiding anything natural or unnatural that spikes blood sugar or insulin (Iโ€™m not diabetic, thatโ€™s just what keto is about) I had to train myself to get used to that and eating high fat everything, fat in my coffee, MCT oil and powder, electrolyte supplement pills, collagen peptide supplements, etc. Iโ€™m pretty sure after RNY, keto will be a thing of the past and that kinda makes me sad. It has been a great way of eating for me, Iโ€™ve lost weight (just not enough) and I feel great on it and donโ€™t want to abandon eating this way. Are there any post-op Ketonians here that can ease my mind?

Can I still eat Keto/LCHF after RNY or sleeve?

I'm post RNY. Am a long term low carber in ketosis for a long time. As @Matt Z and @CrankyMagpie shared, it is possible. I am definitely in ketosis. I am still low carb rather than calling me a keto girl. I do have to do low-er fat in order not to have bad reactions. If I eat too richly (too much fat/too much cheese) then I will get horribly nauseated and feel like I'm so ill. But I routinely eat between 40-55% fat on some days and am fine. You'd be surprised how little extra fat that represents, once you factor in the need to stay at your personal present "protein adequate level" and reduce your carbs to 20g net carbs (or total carbs) per day. I personally aim for a loose average of 30g net carbs per day mostly from leafy greens, low glycemic veggies, berries, seeds/nuts, eggs, dairy and eat very very few sugar alcohols. I do use some small amounts of liquid stevia drops.

    Keto Diet

Keto is no better or worse than any of the other fad diets out there - it's just the one that's currently "in". As with the others, maybe 5% of people on it can lose, and most importantly, keep, a substantial amount of weight off. If it floats your boat and you can keep with it as a "lifestyle" (think vegetarian or vegan) over the long term - even after your loss has stalled and stopped - go for it. But, do realize that Keto in and of itself is not a weight loss/maintenance diet - you have to learn how to do that within the restrictions of the diet, just like any other lifestyle choice. Good luck

    Help! Headache in preop diet!

Can you do the keto diet while on the liquid Pre-op Diet? I have been doing keto for 3 weeks with much success but my liquid diet is only supposed to be 800 calories per day. I was hoping to do my bulletproof hot cocoa and a couple other keto-esque things LOL. My liquid diet starts on 11/25 for 2 weeks.
