Liver shrinking diet pre -op?

If your BMI is only 36, then there are many different reasons your surgeon may not want you to do the LSD (Liver Shrinking Diet). I would listen to what your surgeon says. You'll have to not eat 24 hours before your surgery anyway, and nothing to drink after midnight before your surgery. My guess is they feel that's enough for you. I wouldn't push it, if I were you. Anytime I had to deviate from my main eating plan, I worked with my nutritionist and spoke with either my surgeon or the nurse practitioner about it and we came up with alternatives together. I'm 2 1/2 years out from my original surgery and 1 1/2 out from my revision (due to complications from the 1st surgery) and I've been extremely successful. I suggest not doing things behind your team's backs. Speak to them about your concerns and see what you can come up with together. -

Lots of great advice here but I will add my experience with having done this before and just having a revision that exercising and eating clean makes it so much easier. My mood and energy level are so much better this time and head hunger is pretty non existent so far (part of which is a greater metabolic change witb the more aggressive surgery I’m sure but when I don’t exercise as much or I eat processed junk I can feel the difference part of it may also be that I’m just too darn busy to think much about food). For me carbs crave carbs. I learned this after my sleeve but I wasn’t super careful with it. I’m not taking about the better carbs from fruits and veggies, but the processed type. Eating even a smaller portion is a slippery slope for me so I will avoid these as long as I can and if I do have them I know I will have to white knuckle it a few days through the cravings. I make all my food from scratch and freezing food has made it possible to not cook every night. I freeze small portions and I have a note in my phone if what’s on the freezer by date. When I wake up and look at my calendar as I’m drinking my proffee (chilled coffee and protein shake) I decide if I have time to cook and if not I pull a bag out of the freezer and put in the fridge. Then when I get home I put it on the counter or defrost in microwave if necessary time wise and make a veggie to throw it together. A really nutritious and yummy dinner done in 10-15 minutes and bonus, a lot less dishes. I started walking early out and added group classes that are offered at my community center and now i am up every morning walking first thing. I actually put on exercise clothes after my shower and sleep In them and first thing I’m already dressed and ready to go for my walk. ooh and the support groups that my team offers are great in addition to staying very active on here. I actually met a walking buddy there and we walk together down by the river once a week, the day that I don’t have group classes and we cheer each other in in addition to the once a month in person support group meeting. I also chose to tell more people this time. Some were against it but I educated them and now they are starting to come around about it. I think it will be easier fir me to say no thank you to food when everyone around me knows my goals. The first time I did this so many people would say to me you’re doing so good. You can have one slice of pizza or one brownie but the problem was this was my family and I was around them several days a week so just this one became just three or four. I actually told them that when I see pizza or brownies, I already want one bad enough on my own. I don’t need someone else tempting me. I think they got it. We’ll see. I guess if they do it this time. -

Interestingly, I met yet another dietitian today, and she had a completely different perspective than the first one I consulted. It left me feeling frustrated, especially since my next appointment isn’t until May. After some reflection, I decided to take matters into my own hands and stick to a plan that feels right for me. With the help of my trusted ChatGPT, we crafted a fantastic meal plan that emphasizes protein while keeping my calories between 1000-1200. I’m really excited to start this journey, especially since I’m active most days and want to maximize my results. I just wrapped up day one of the revised plan, and I’m optimistic about the changes! They’ve been suggesting I should be losing more weight this early on, so I’m determined to make this work and see the results I’m aiming for. Thanks again -
I am in MA and have Tufts. I was not doing a revision, but for me, it took about 6 months to go from first appointment (an orientation class) to surgery day. That was with some scheduling snafus thrown in along the way. I had already spent several years in the non-surgery program, doing nutrition and trying various meds. Curious where you are going for your surgery. I started at Mt Auburn hospital and ended up at Winchester hospital because the Mt Auburn program shut down last year (hence the scheduling snafus).

One week post op and feeling scared about never having favorite foods again

Well, I will tell you that after my sleeve, there was nothing that I could not tolerate. I’m pretty early out post revision so I haven’t tried many of those foods with the exception of tacos but I haven’t had any issues with anything so far. I make tacos with ground turkey meat, 2% cheese, and just eat the filling or I have it with low-carb tortillas. Surprisingly the turkey meat and the 2% cheese do not taste much different at all by the time you add the seasoning and them little low-carb tortilla aren’t bad either. Sometimes I just do it as a salad too. You may be surprised to find that you don’t crave those same foods, though. The surgery does some metabolic changes and can change the foods that you enjoy. I mean, don’t get me wrong, pizzas still sounds good to me but all of a sudden grilled fish doesn’t sound bad either so it’s a lot easier to make the healthier choice. It really depends on what your purpose is for the surgery. If you’re only goal was to get rid of your Gerd, then you may not care about getting into a super skinny size in which case eating smaller portions of the foods you enjoyed before shouldn’t be an issue at all. But if your goal is also weight loss and you want to be able to maintain a very low BMI, then you’re going to have to make some sacrifices. In which case, my suggestion would be to search the bariatric websites and even the thread on here for recipes and try some things once you get to soft foods and regular foods. You may be surprised at things that you will enjoy. I make turkey meatballs, and I have those with peppers and onions and sauce which are delicious, turkey tacos are good too, chili with lean meat is pretty reasonable macro wise, white chicken chili is a favorite, I made a Mexican skillet that wasn’t bad macro wise you may enjoy if you like Mexican, I sometimes do the zucchini noodles if I want pasta. They are pretty bland and just pick up the flavor of whatever sauce you use and of course you will want to add some protein. I also found a recipe for spring rolls which some people call summer rolls that are so yummy. They’re not the fried ones but still really good. It’s chicken, avocado and veggies with peanut sauce to dip. These are all in the weight loss phase once I get to maintenance I can add things to jazz them up a bit You can also do chicken or cauliflower crust pizza with chicken and veggies so it has more protein Or cauliflower mac & cheese. Lean beef cheeseburger without the bun, a cheeseburger salad or low carb cheeseburger wrap. There are ways to tweak your favorites. Basically anything I am craving. I just type it in Google with the word healthy in front of it and I try some recipes until I find one I like. Some of it is bland but many things are surprisingly good. One that I still want to try is zucchini lasagna. After doing the ricotta bake on purée stage and loving it, my guess it that it’s good I still make the ricotta bake I just add Turkey sausage and veggies. Ooh and quest protein chips are really good once you can do crunchy. Also, they have an “ice cream” maker that you can control the ingredients I have heard some people talking about on here But the short answer is No, unless you have a specific intolerance, allergy or sensitivity you will not have to give up your favorite foods forever. You will just have to make tradeoffs. How often you can have them will be based on what weight you are looking to maintain. If you are willing to exercise, for instance, you will be able to eat more carbs and maintain at a lower weight. -

Well I had a revision (no surgery on my stomach) so I won’t even talk about portion. Also, I’m pretty active (2-3 hours of at least walking) so take this with a grain of salt but my calories are around 900-1000, protein is 90 plus, net carbs are 60-70, sugar is usually under 20 but my max is 30, fiber is still too low but around 10-15 with the fiber powder, fat is 20-30, water is about 80, and calcium is usually around 1800 or above. I was told that calories alone don’t mean much at all and carbs and fat depend significantly on what kinds of carbs and fat. Carbs from fruit and veggies are usually fine in our portions and healthy fats like olive oil, avocado and nuts or low fat dairy are okay too but you do have to measure of course and not have them multiple times. It’s when you get into the processed stuff that you have to watch them numbers so much closer. They sneak things in. I have noticed that the low carb stuff is usually really high in fat. My goal is to keep it to Whole Foods as much as I can. So far I have had the occasional low carb wrap, Turkey jerky, yogurt drinks, and quest potato chips in terms of packaged food. Ooh and that burrito wrap I had to seriously budget for. But I mean like twice or three times a month. Your team will have the best info for what’s right for you though. So many things make these numbers different for everyone. Did you ever find an app that you can access to log your food?. It has really helped me. Usually when I sit down to have lunch, I enter my breakfast, my lunch and play around with ideas for dinner and snack and see how that works out in terms of macros. I always have fish and frozen low carb veggies that I can pull out of the freezer if I am too high for the day. I found these burritos that supposedly freeze well I wanted to make but they used a real tortilla. They do not have the big ones in the low-carb version. I looked everywhere but I needed the bigger one and order get enough protein. I wanted to make those four nights in a row but every time I entered them I was going over on something. I finally was able to have them last night but all I had was my morning proffee and five egg whites with pico de gallo for lunch. Then I added a little bit more cheese because I was 2 g low on protein. Anyways, it makes it so much easier to be able to plug these things in and you can delete and try something else if it doesn’t work or add or subtract snacks. The only thing that I do not enter, but I always allow myself to have. I kind of learned on the preop diet was the sugar-free popsicles they’re actually 15 cal I believe but they were free foods on my preop diet and I keep them that way in my mind. It keeps me thinking that I’m never going to be not allowed to have food. Mentally it somehow makes me think that I don’t have to starve. I can always have something.. I actually really enjoyed them the tropical flavor ones on my preop but I haven’t wanted sweets very often since. I have had them a couple of times when I wake up late wanting something though. -
Hey all, so I’m in the process of getting a revision I have the Lapband wanting to remove it and getting the Sleeve. I have BCBS of MA. How was the process for you guys. What where your requirements, and how long did everything take meaning the whole process and the approval time. I went 2 weeks ago to my consultation, did the nutrition class then my ecg that same day now waiting for to talk to a psych. And the place is taking long I already called bcbs it’s in my network and also got my copay these ppl are taking long I’m ready to start my new journey

I had the sleeve and was doing well until I was hit with all kinds of complications. I had the revision to bypass and couldn't be happier. It's like night and day. I think you'll do just fine. -

yea I think mine was like 1-2 oz. I didnt have my stomach operated on because it was a revision this time so I was able to do the top of the range right away which was 2 oz. I would call your team and ask to be sure but maybe start with just one tiny bite and wait and see how it goes and then SLOWLY go up. You should hear back from them before you get to 1 oumce. I think that was the lower end but you always want to hear it from your team anyways. Or check to see if it’s written in any or the papers they gave you. -

I didn’t have band either, but I had the sleeve and gained it all back. My surgeon explained that obesity is very complex and for many people the sleeve is great and it’s less risk so they go with it when they think it is appropriate but it does not offer the same metabolic changes as the bypass or the SADI (which is what I revised to) that some people need in order to be successful. I believe the Lap band offers even less metabolic changes than the sleeve, if any. Basically with these surgeries mostly everyone will lose weight, it’s the keeping it off part that’s difficult. And that’s when you need a surgery with enough of a metabolic change to help you long term. I am 2.5 months out from my revision and I was so worried about the same thing, losing it only to regain again but I can already tell that there is more or a metabolic change. I mean I am exercising!! Me….everyday!! That never would’ve happened without this surgery. Healthy foods taste better and my mood has improved. I have tons of energy and motivation. I never got that with the sleeve. The bypass has been around for a very long time for good reason. For me the sleeve and SADI difference has been night and day. I think you will find a similar experience with your Bypass. -
Help me decide if I should revise from band to sleeve

Hi everyone! I have not been on these forums for sooooo long- and it was the band forums...anyways. I had my band placed in January 2012. All was well, I lost some weight- then I didn't. Had a hernia repair and band refill, but have been so off track for so long. I definitely learned to eat around my band. And now I have a constant lump in my throat that my doc things is ghost reflux- I don't feel any burning like typical heartburn, but have this lump in my throat- especially at night when I'm trying to sleep. And I wonder if it's from the band. I still have restriction, I still can get stuck but I eat around the band, etc. I have been doing better and have lost 25 lbs and I know that no WLS is magic. I just wonder if revising from a band to sleeve would get rid of the reflux, and I also worry about the foreign band in my body...idk, what do y'all think? I would have to be self pay this time, as I'm 99.999% sure my insurance won't cover it. But it would be worth it, and maybe jumpstart my health journey again....... -
Ask him about the statistics of long-term success with the sleeve. The sleeve is great for many people don’t get me wrong, but I believe it was 26% of sleeves that need to be revised. If I’m not mistaken, this included because of Gerd as well, but also inadequate weight loss to begin with or weight regain, which was more common with sleeve than with the other surgeries. When I talk to my surgeon about revision, I wanted assurances that I wasn’t just wasting my time going to lose all of my weight. Only to turn around and gain it back and he said the sleeve does not have the same metabolic changes that the SADI or Bypass offer. While the. Sleeve is enough of a change for many people it’s just not enough for some. In my case when I lost my weight, I was basically on a diet with the sleeve which was a little bit easier because I wasn’t hungry, but once the hunger came back, so did the weight for me so this may be different for you because you did keep it off for a while, but I would ask what the statistics are in terms of people who gain it back down the road with sleeve is that more common than with the other surgeries? I guess one kind of pro for doing the sleeve would be that if you did regain the option for a revision would probably be a lot easier. Not trying to say that you want to think of that before you even start but it is a possibility for all of us I guess. It’s a lot of questions and a lot of information when you start weighing more than two options especially but just be thorough and ask all your questions you will know which option is best for you once you feel educated about them all. I had sleeve revision to SADI (just the bypass portion) and I am really happy with my decision. My surgery was August 7th so I am about 2.5 months out and including my preop loss I have lost 55 pounds. I am over halfway to my goal. I have been eating 100% on plan though and exercising a pretty great amount (2.5-3 hours a day). The revision surgery gave me the metabolic Hca Hes tk actually do Al of that though. I never exercised or prepared fresh homemade food with my sleeve. Didn’t have the motivation. It made a pretty huge difference for me. Did your Dr do a endoscopy, barium swallow and gastric emptying study? Mine did those tree before deciding I was a candidate for SADI or Bypass. If the sleeve didn’t work for you long term then I would probably lean away from a ReSleeve. Just thinking why repeat history. Try something different but that’s just me. The research does say that the loss is more durable witb the SADI revision than the Bypass. Not certain about the ReSleeve but my guess is that too since the Virgin Sleeve doesn’t have the best statistics for durability. Also, resleeve is kinda risky. My Dr said he wouldn’t touch the sleeve with the SADI not because of my BMI but just because he doesn’t think it gives that much more benefit to justify the risks so that’s something to look into as well. Ooh and if you do have coverage for the GLP-1 and you can tolerate them, they do work. They don’t have to be long term for everyone either. If you take the time on then ti make real lifestyle changes you may be able to go off of them and maintain. Ibvuously If you just eat a few French fries for your meal you will lose, but will obviously eat more when you don’t have the meds but if you genuinely change your habits then people do maintain the loss. I have a friend that is almost to goal and she has been asking me for advice of what foods to choose to be sure to get her protein in. Kinda odd because she is smaller than me but I have heard a couple success stories with them. My friend has to force herself to eat. It’s a chore. If you do go that route, talk to your doctor about staying on the lowest dose that works for you. That’s the thing that many bariatric doctors usually recommend and it makes sense to try to avoid the side effects. . And some people go onto a maintenance dose after they reach their goal if they need it. Also, see about a three month supply when you do get to the dose that works because for my friend it’s cheaper that way. I strongly considered them but I didn’t have adequate coverage. I even considered postponing to see if things would change but my family Dr advised against waiting since these things can take a long time. There are pros and cons of each option but do your research. This is a good start but make anothet appointment with you surgeon if you have more questions. I did several before I decided. You probably only have this one more shot at this. Not a lot of surgeons will do a second revision ooh and that is one thing to consider if you do go with SADI, it’s newer so ask your surgeon if you had a complication god forbid what you would do. Mine actually brought it up that he would send me to another hospital to someone with more experience with the surgery. I mean obviously he would stabilize me but if I needed a revision because of malnourishment or something he would send me elsewhere. I kinda respected the fact that he would admit what his limits were and appreciated it but maybe you feel differently about that. Or maybe your surgeon is fine with revising if necessary but I would for sure ask.

What about trying a GLP-1 versus surgery? Check out Dr Matthew Weiner’s Pound of Cure website, podcasts & you tube channel. He’s been speaking a lot about recommending sleevers with regain to use GLP-1 meds instead of a revision surgery. You may find it interesting & something to discuss with your surgeon as another option. PS. Out of curiosity what was your original starting point weight, your maintenance weight, and current weight?

Well as you all know I had my procedure on sept 6 and now here it is Oct 20 and I haven’t lost in fact I think ive gained Ive been doing cardio and i go back to regular diet in 4 days and I am so so scared. I couldn’t even come on here and post because i got so depressed about having this revision procedure.. I have a appointment with my nutritionist on Tuesday and I’m just gonna put it out there . He felt like part of it could be constipation so I am trying ducolax and see if it’s just blockage idk

Learn2cook I am a nurse so I work a busy job as well. I have many goals such as becoming financially free, living a more sustainable life and finding a life partner at some point to name a few. As far as my health and fitness goals, I want to be strong. I want to be able to hike, and raft rivers and adventure. I dont want to be hindered by my weight anymore. I dont need to be skinny but i do what to be somewhat a normal weight. Before surgery I did try waygovy and was working out a lot so I was pretty optimized prior to surgery however i didnt lose a ton of weight with waygovy despite tracking and trying to be really mindful. My bariatric surgeon basically told me the indicator of my success is how much i can shed in the first year so i am determined. I am just hoping to get some feedback for people who have had a band before and went for revision. Thanks for your sincere reply Brittany -
I had a lap band placed in 2013 and ended up having it removed here in January. It up getting infected. Thankfully no erosion and just some scar tissue. The surgery to remove it was miserable for me tbh but at that time i started the process to revising from band to bypass. Thankfully that went through without a hitch and I had surgery on the 2nd of October. Initially with the band I lost over 100 lbs very quickly and felt so good. Went from 305 down to around 200. About a year after the band was placed I got my gallbladder out and I feel like that is when things kind of took a turn for me. I ended up having issues with the band so I just removed all the fluid after a year or so and just coasted through life with the thing. Things still getting stuck with it empty. Ended up gaining all my weight back and then some. Anyway, I just got the bypass on the 2nd and I am just overwhelmed with fear of not losing enough with the bypass as well as regain. Wondering if anyone has had similar experiences? On the pre op diet I lost 15 lbs and I have lost about 17 since surgery. I am almost 3 weeks. I read about people losing 30-40 right away. I worry im not on track even though I feel as if im doing well. I dont have any pain, my energy is good, i havnt puked at all since surgery (still on full liquids) and overall things seem to be doing well. I do need to up my protein (60-70 grams a day) but am able to get my fluids in. Time will tell I guess but would love to hear about others experiences. Thanks Brittany

There was this older lady in my craft group, and I told them that I had surgery because I figured it was more people to make me feel accountable. This is a revision surgery for me and the last time I didn’t tell people like that and it didn’t work out so well so I’m trying a different approach. Anyways, she is about 80 years old and she said to me she just wanted to tell me how happy she is for me because since my surgery, I have really blossomed. She said that I i have a healthy glow about me and I’ve really come out of my shell in terms of confidence and it shows. I think that was probably the nicest compliment I’ve ever had in terms of my weight loss. I just appreciated that it wasn’t about superficial things. Maybe there’s hope for all these superficial people that when they get a little older, they will view things in terms of stuff that actually matters. -

Do you mean SADI? Or is SASI another newer surgery? I had a sleeve revision to SADI and they warned me that diarrhea would be an issue for the first three or so months. For me I haven’t had this issue. Some days my stool is loose but it’s only once a day. I have been eating extremely clean though so perhaps that’s part of it. I would keep a log of what you’re eating and when it is happening and get with your team. You may have developed an allergy to something. That’s not at all uncommon. -

I haven’t tried pasta since my revision but I could eat it done post sleeve and they didn’t operate on my stomach so my guess is I could. I am doing zoodles for now though. I also need the sauce for my turkey meatballs. I have done like 1/3 cup of brown rice to increase my carbs and that was tolorated fine. A lot of people do have issues with starchy stuff though. Mayne a bit of a blessing. The turkey meatballs with pepper and onions over zoodles is quite tasty though. My favorite healthy meal so far. -

For me I’m just starting to notice that people treat me differently. People just being nicer and going out of their way to help me in stores. Customers striking up small talk conversations with me WAY more often. It’s just weird. I have always been told that life is a fashion show growing up and I sadly finally admit that it’s true. After losing all my weight for the sleeve quickly, gaining it back and now loosing again with the revision I am seriously not imagining this. Some people are just that superficial…a lot actually. It breaks my heart for the heavier me and other obese people. -

Hey all! I had the sleeve in 2017 and never got to my goal weight and I had GERD. So fast forward to 2024 and had my sasi. All is well except now I can’t keep any food down. I’m having diarrhea several times a day for the past month. Any one else had issues like this? Oh and then I found out I have fatty liver. 😫🤷🏽♀️😵💫 scared! Smh -

Well I read that like 26% of sleeve patients end up revising whether it’s because of GERD, inadequate weight loss or regain. My surgeon said that it’s less risky and it’s a very good surgery that works fine for many people but it’s just not enough of a metabolic change for some. I honestly think he leaned towards it because my BMi was low (for this surgery at least). I was barely 35BMI but I tried telling him that was only because I was literally dieting my whole life and even then I didn’t eat that bad. I just ate a lot of food. I’m sure he hears that a lot but I think I just needed the bigger change from a more drastic surgery. From day one with my sleeve I felt like I was on a diet. I never did find healthy foods that I enjoyed, I ate chicken breast and one of the three veggies that I liked and downthe road I ate a tiny portion of something not so good for me (nothing terrible but not good either). I also never felt the spurt of energy that I felt this time so I never got into the exercise. It just wasn’t the whole lifestyle change. It was like any other diet when my hunger came back and my portions got bigger but it was healthy food at first. Then I started gaining and it just made me so depressed I was trying so hard and I was gaining already. I went to my surgeon and asked about bypass and he said he didn’t think it was a good option for me. I still don’t understand why he said that and later changed his mind. Anyways, after that I stopped following up with my team, stopped posting on here and I never went to in person groups. All of my friends and familY had already told me how big I was before thinking it was safe to say because I was thinner then so as I continued to gain I knew what they were thinking of me so my confidence just plummeted and that just snowballed into my gaining all of my weight back and then I just said forget it I guess this is just my life and I didn’t even try anymore. Finally one day my doctor said that I should really go ask him why it is that he doesn’t think it’s a good idea because they felt I was a good candidate and he said he would do it and asked me if I ever heard of the SADI. He sent me home to do research on that surgery and come back and tell him which one I wanted. I said I think the SADI and he said he needed tests to make sure I was a good candidate. He did an endoscopy, a barium swallow study and a gastric emptying test and then he said I was good and put me on the schedule. Then at the last minute they figured out I needed the nutritionist visit and the psyc eval that they didn’t think I needed so I did those in a hurry and started my preop diet. This time I told everyone about my surgery. Even though I have terrible fears that I will regain again I know that I need them cheering me on to keep me going as well. I started the In person support groups before I even had my surgery and I started posting here again as well. I have done so much more with finding healthy foods that I don’t dread and adding exercise so I can have things other than chicken breast macro wise as well. I also know this time that even if I don’t reach my goal weight or my secret goal weight that I already feel so much better and hopefully I can keep that In mind and not get caught up trying to do this just to be skinny because that isn’t the most important thing. Last time I reached the first goal I had set for myself but I never did reach the goal that I secretly wanted to get to. But I was only 18 pounds away and I let that make me feel like a failure. I would give anything to be that weight now. I just lost perspective I guess. Being healthy is really why I did it then and now and I’m already on the road to being a lot healthier so if I do lose more that’s really just icing on the cake. I just have to remember that and I think I will be okay. -

I had more random scrapes with my revision to SADJ than I did with my sleeve and it was the same surgeon so I thought that was kind of peculiar until I really thought about it. The SADI was robotic and the sleeve was not. My surgeon had many many years more experience doing it laparoscopically than he did doing it robotically so maybe even though the robot is supposedly more precise once it gets down inside of you it’s a little harder to maneuver getting the incisions done? obviously I don’t really know for sure that was just all I could come up with. The good news is I’m two months out now and all the superficial scrapes and scratches are long gone.
