I personally don't think bypass is that much more drastic than the sleeve. Removing 80% of your stomach and throwing it in the trash seems pretty drastic to me. And RNY is NOT the most drastic procedure and isn't for the most severe cases, either - DS is the most aggressive of the more common WLS surgeries and results in the highest weight loss. It's not nearly as common as VSG and RNY, but it is sometimes recommended for people who have a lot of weight to lose (like a 60+ BMI - although some people get it who are under that). There are many people on here who have had RNY at much lower starting BMIs - similar starting stats as the VSG. if you have GERD now, I would definitely be looking at bypass. Sleeve doesn't always make it worse, but for about 30% of people, it does. I had GERD prior to surgery and wasn't willing to take that risk. both are good surgeries and I've seen people on here who have been very successful with both. If you're willing to take the risk of GERD after surgery, then the VSG may be a good choice for you. As I mentioned, I had GERD before surgery and wasn't willing to take the risk. I've been very happy with my RNY. P.S. From your title, it seems like you're not clear on "RNY vs bypass" - RNY is a gastric (stomach) bypass, and we use those terms interchangeably (there is some bypass of the duodenum, but most of the bypass is of the stomach). You may be thinking of the DS where a much greater portion of the small intestine is bypassed (??). The DS has a sleeve stomach, so the stomach is not bypassed. I probably would have gone for the DS if my insurance covered it, given my starting weight, but my insurance only covered VSG and RNY. Although there was the GERD issue with me, too. Anyway, I'm happy with my choice, so it no longer matters. -

Initially I was head set on the Sleeve. When my surgeon asked if I ever have GERD and I replied “yes, once in awhile”, he drew a picture of what happens when GERD takes place with Sleeve (increased pressure in a much smaller pouch, that raises serious discomfort, and risk of potentially serious esophageal damage.) He went on to say if your tests indicate serious reflux issues I WON’T perform Sleeve on you. If they show minor issues, I’ll do Sleeve if you still choose to, but caution you it may result in a revision up the road if GERD persists. I would much rather you go through WLS surgery once and not have to do it twice. He encouraged me to leave him, start the battery of testing and do more research. When I learned that RNY is still considered the Gold Standard of WLS, with the longest history of success, risks very low, slight potential for greater weight loss due to malabsorptive feature, my mind was well on its way to RNY. Three months out and No regrets, not at all. Challenges with both, but to be expected. Many Sleevers are very happy and have done amazing, too! It’s your personal makeup that, along with recommendations from your Dr. that will guide you. Mine is only one story. Good luck! I've had the sleeve for almost two years and haven't had any heartburn or GERD. If I had had GERD, I would have had the bypass initially. I've been very happy with the sleeve. It's a pretty personal choice based on preference. -

Have been visiting my Bariatric clinic initially for undergoing the Sleeve procedure, however with my last surgeon visit, he said he recommends that I get the Bypass. With the increase of GERD after Sleeve, and the numerous revisions from Sleeve to Bypass...he says if you'd like to avoid this in the future and save a surgery then it would be better just to do Bypass. WHOAH! Bypass seems to me like the most drastic procedure for the most severe cases (IDK, I get that sensation). My question to Sleevers: If you could do it again, would you choose Bypass? Does GERD really get that bad? -
My surgery is June 15th. I am having the bypass done (Roux-en-Y).

At A Loss (Not Weight Loss)

I understand the situation very well, lifelong chronic illness and disability along with metabolic disorders and inability to exercise at all I fought for gastric bypass surgery I desperately needed the tool because my own efforts were not sustainable. To be honest I had to drastically cut carbs/sugar and calories. Prioritize protein first, find the calories that you start losing weigh at. I only started to increase my calories once I hit goal. Make sure to get minimum of 0.36 grams protein per 1lb body weight. And honestly exercise isn’t really for weight loss, building muscle is. The more muscle you have the more calories you burn. If you can do some resistance exercise some strength training maybe through physical therapy. That might help. For me my medical condition, exertion causes muscle weakness and damage and for me to feel ill. I can understand not being able to rely on exercise. But if you are able to do limited exercise look to building muscle. And that’s also why you need to prioritize protein over calories. Getting maximum protein for minimum calories. Also be mindful of sticking to three meals and how much snacking and start tracking your daily calories and protein along with tracking foods you eat each day to see what helps or hurts. just one example: I like mixing yogurt like Fage plain, TwoGood, or oikos triple zero with 1/2 scoop of PEScience protein powder. recipe example: oikos triple zero peanut butter banana, with 1/2 scoop PEScience peanut butter cookie protein powder = 160 calories, 27 grams protein. I mix it I a bowl with a bit of pink salt and stick it under the freezer 30 minutes and mix it again. A little low sugar jam and it tastes like pb&j polaner sugar free with fiber I think ... these days I add a little low carb granola for crunch. (Sola granola 15 gram portion is 70 calories and 4 grams protein 2 net carbs) And some sugar free land o lakes whipped cream. Overall very low carbs. PEScience http://rwrd.io/c6uus2e -

This happened to my mom. She was basically starving to death. Her sleeve was too small. Her surgeon had to go in several times to put a balloon in her sleeve and blow it up a bit to stretch it. It was would for a few weeks and then the same problems would arise. So she went to a different surgeon and he had to give her a bypass. She had now gained back and maintained a healthy weight.

I had the sleeve so that's probably the first difference. Another difference is that people with "lower" BMI's compared to those with higher BMI's on surgery day lose their weight slower. I have also heard from research that the effectiveness of the surgery is related to genetics. Edit: I now wish I had the bypass -

Hello! I am new here and I just had my gastric bypass on the 3rd. I'm a 34 year old married mom I am still having some pain and a little nausea. I am supposed to do full liquids for 7 days and blended for 3 to 4 weeks. Making sure to sip, sip, sip is no joke. One of the biggest things I am most surprised about is the noises that come from my stomach area. Lots of gurgling. My hardest obstacle is my son who is 3 wanting to crawl all over me! It’s hard large and painful. Feels like a baseball underneath. All the others are greenish as it’s been over a week. I called the emergency service for my surgeon and his associate called me back. Just said if no fever it’ll be okay. ER if I want but with COVID unsure I wanna go but I had surgery and everything was impeccably clean. Maybe CITYMD. maybe it needs to be drained? 👩🏼⚕️

I started this Journey back in Jan 2016. It took me years to get my Gastric Bypass. I had went from 407 pre surgery, 399 on surgery day, and I had went down to 278. My therapist put me on hormones and I gained over 40lbs, so I quit taking them and I dropped 2 pounds and now I cant get back under 300. I'm disabled so I cant do normal exercises. I'm really not sure what my surgeon did differently with my bypass but the weight has come off pretty quick and easy but I did restrict my calories for months was very strict on myself... -

Sweetheart only thing I can tell you is remember everything that you learned for those first 6 months prior to surgery I'm not sure where you live but if you're a Costco member they have a lot of options for choosing the things that were supposed to allowed to eat but they're good like no GMOs baked whole wheat crisps with protein in every single chip and I found a big bag of chickpeas for snacking salted awesome protein and I tell you something I have had the most horrific experience with my bariatric surgery I can't specify which one because I had to have two different ones and not at my fault at the hands of the first surgeon that messed me up so bad the story is unbelievable I look horrible the first 6 months I dropped all my weight from 233 lbs down to 168 way too fast everything that I ever had as muscle boobs at all lost I don't have the hanging sagging skin but I'm skeletal I had nice boobs I know I heard stories about people who finished the bigness of their breasts but not like this mine is like just skin and I have always had nice breasts never had to wear a bra and now because those are gone that's what makes me look so skeletal my bones are sticking out for my collarbone you can see my top rib cage Alex sucked up like I'm some kind of drug addict or something I am very very sad I even have age that's all it did for me is aged me I look as old as my mother within I'd say it's been almost 2 years January it will be 2 years and I'm still having trouble finding an attorney to take the case I've had to take it and drop it the first dropped it because he did not know enough about bariatrics II dropped because all he was concerned about is how much money has law firm would lose if he didn't win and he just lost a case with bariatrics 120000 a cost at this for me said but he want to previous to that that's here nor there I'm sorry going off track here but you need to remember having a little bit of weight on you 173 I would love to be back that way that's a healthy weight unless you don't carry it very well and it's too much fatty fat instead of muscle protein protein protein do you work out cram the protein after if you are able you can buy a kayak it helps build your arms standing in front of a wall and put your palms up on it and that back couple steps and just do push-ups on the wall start there those are reps but it's also building the further back the heart of the pushing and then maybe you can start getting lower and lower until you're on a park bench and you're doing them sit on a picnic table put your feet hooked up under the seat on the other side and do sit ups if you have the energy to do that you can do those pretty much at any park I know it seems like a lot I have a damn exercise bike at right out there and I know I need to be building my muscles I literally don't have any the only reason I have a little bit back in my legs because I had to go to Florida and help my son move up here and he had mega steps up and down up and down with all the stuff but anyway I don't know how much you weigh before hand but it is not worth stressing your pouch I've had the sleeve done and who knew people have different shaped stomach sometimes there's actually several different ones I pretty much flipped out when I googled it but he and all of the bariatric surgeons are mandated and required to check the shape of everybody stomach prior to surgery and he did not do that with who knows probably none of his patients the surgeon who fixed me said I don't know why he didn't but we do end they keep track of you for the rest of your life but that first surgeon he did the cut for my sleeve and my stomach dropped and twisted almost shut and he would not listen when I kept trying to tell them there was something wrong I was regurgitating most of everything that I took in and if I was not the kind of person that if I couldn't find out by the doctors and they had no clue that I'm going to start researching what's going on with me so I was lucky enough to find out about the importance of thiamine which is vitamin B1 ... Because without that for specific amount of time and days there were two people same exact surgery that did not do the research and kept going back to the same idiot surgeon unfortunately in the hospice aim hospital and they both now cannot talk or walk and their in nursing homes ..I found an awesome surgeon that I knew exactly what was wrong and how to fix me but the only way to fix me is do a type of bypass but a shorter one okay but when he got into the side of me to do the surgery he said he could not believe how bad my stomach actually was twisted it was almost completely shut so he had to actually remove my stomach the rest of the stomach that was left from The cutting of the sleeve he said I should have never ever been offered sleeve because with the type of stomach I had there would have been no way to do it and I wasn't heavy enough at 2:33 really with my BMI to have a bypass and I wouldn't have had that anyway I wouldn't shows that I researched all my options prior to doing it begin with and that was not an option for me but in order for him to fix me that is the only thing that he was able to do and I had no choice so he made a itty bitty pause the size of an egg with the little part of some of the remaining sleeve before he had to remove it and I don't know how he did it but don't gain the weight this covered thing my personal opinion and I'm really sorry on some people don't agree I really don't share this much because it's not worth going into all stuff people it's kind of like talking politics LOL that's something you just don't do ... But I believe it's been blown out of proportion a little bit more then it needs to be maybe I'm wrong I do live in a small town in the country so it's not quite as bad as it is and I'm sure in certain States all that staying home and not working would have been necessary and doing it but not worldwide it just seems political it's definitely political... My point being don't allow the damn government to cause you to screw up everything you work so hard the gain or should I say lose... You've come too far before it gets too much more out of control if you don't want to search going up it goes fast and when you try to get it to go down it doesn't move so definitely protein vegetables fruit protein vegetables fruit avoid anything that has more than 9 grams of sugar in it and try to stay away from processed food just think healthy don't leave things out on your counter don't buy anything that you're not supposed to have at the store if you have the family that's going to be difficult but just try to remember using the small saucer for your portions anyway good luck with everything I've gone around John's barn on this one but sometimes we do that when we're talk texting lol stay safe much love good luck on your new life journey -
There is: go back to basics, counting calories along with a balanced food intake. Use a calorie counting app if that helps, set your profile to lose 1lb or 2lb per week...it will set calories based on your current weight, height, gender, incidental actiivities...give it a go for at least three to six months. If you are overeating due to emotional eating, look for activity based things to do that can replace the emotional eating. Angry go for a brisk walk or boxing class, stressed a soak in a relaxing bath, lonely or bored visit a friend or family member or even give volunteering a go. Maybe consider counselling if you aren't able to rein in the emotional eating. If it portion creep and your portion sizes have increased a lot, go back to using smaller plates and bowls so as to encourage smaller portions. You could also consider visiting a dietician for some guidance and/or consider a revision weight loss surgery.

I’m also having my bypass on the 9th! Found out on May 28th that insurance had approved my surgery and my surgeons office had already scheduled it! So I got one last meal and started my pre-op diet. Had bloodwork and the covid test today. My hospital bag is packed, I’m taking two hospital gowns with me, slippers, biotene spray, listerine strips, colgate wisps, crystal light, chapstick, a couple protein shakes , baby wipes, and facial wipes. So I feel really prepared and ready. So glad to see others having this procedure around the same time! -

Hi Bypassers! Wanna role call so we can keep track? I'm MaybeMeow. 222 pounds and 5'5". 47 years old. First goal is 165. Hoping eventually for 150. Bypass is 4 days away on June 9th. Chose it with my surgeon instead of sleeve due to lifelong reflux. I'm very nervous. Pre-op diet has my stomach going crazy as I don't usually do artificial sweeteners and I think that's what causing the runs. And I'm struggling with the low carb headaches. But I'm staying postiive!! Distracting myself with daily prep. Today's prep was making Protein Jello Shots and Protein Pudding shots. Sugar Free Strawberry Jello with GenPro powder mixed in. I have 25 one-ounce "shots" that have 2 grams of protein a piece. Saving them for post op. The pudding is Fage fat free Greek blended with sugar free vanilla pudding. Each oz has 3 grams protein. Will use them starting post op day 3 to help me get thru clear and full liquids. I'll attach a pic. Took about 40 minutes to prep. I'm not much of a cook or meal planner but this was easy. How is everyone else prepping and feeling? -

Hi everyone, I'm not usually very good at linking in on forums and other online communities, but I've logged in today for the first time since my roux en y bypass 3 years ago to see if there's anyone that has or is experiencing the same as me. For the first time, I have put on weight and I am so disappointed in myself for letting myself go. I had phenomenal success following my op - I lost around half my body weight: Reducing from 23st 5lbs (327lbs) to 11st 5lbs (159lbs) over 18 months and actually had to eat quite a lot, albeit little and often, in order to keep my weight stable. Since the coronavirus lockdown however I have been non-stop eating and as a result have gained a stone (14lbs). I'm 12at 5lbs (173lbs) now and my clothes are too tight. I can eat normal sized meals and constantly snack. Basically, once whatever I've eaten has digested I'm eating something else. I'm really worried I've stretched my pouch. Can going on a diet cause it to shrink back at all, even just a little? I'm considering a keto diet because I do like high protein and high fat (I'm crazy for butter!). All responses are welcome. It is well documented that Roux-en-Y gastric bypass (RYGB) surgery drastically alters the fecal microbiome, but data are sparse on temporal and spatial microbiome and metabolome changes, especially in human populations. We characterized the structure and function (through metabolites) of the microbial communities in the gut lumen and structure of microbial communities on mucosal surfaces in nine morbidly obese individuals before, 6 months, and 12 months after RYGB surgery. Moreover, using a comprehensive multi-omic approach, we compared this longitudinal cohort to a previously studied cross-sectional cohort (n = 24). In addition to the expected weight reduction and improvement in obesity-related comorbidities after RYGB surgery, we observed that the impact of surgery was much greater on fecal communities in comparison to mucosal ones. The changes in the fecal microbiome were linked to increased concentrations of branched-chain fatty acids and an overall decrease in secondary bile acid concentrations. The microbiome and metabolome data sets for this longitudinal cohort strengthen our understanding of the persistent impact of RYGB on the gut microbiome and its metabolism. Our findings highlight the importance of changes in mucosal and fecal microbiomes after RYGB surgery. The spatial modifications in the microbiome after RYGB surgery corresponded to persistent changes in fecal fermentation and bile acid metabolism, both of which are associated with improved metabolic outcomes. -

My cholesterol was oddly high at last check..... Doc said it was genetic/hereditary most likely and suggested whole 30. Ugh. My intake amount for solid foods has always felt abnormally small and I've tried, to no avail, to 'stretch' myself. Talk about pain! Maybe time for another swallow study. 1st go round was solely focused on the horrendous reflux issues..... Scared to death of another surgery at this point but I'm opening up to a bypass for quality of life reasons. Pot helps most everything

I'm from San Diego and live in Houston now. Dr. Ricardo Bonnor first did my VSG in 2013 then my 2020 DS revision. I have great insurance so they covered all my needs. The most I pay out of pocket for all medical bills is $5k each year. -
