    NHS Tier 4 Pre-Op Question

    Did you have any psychological input during your Tier 3? (I don’t know if you had to go through Tier 3?) I had about 4 sessions with an NHS bariatric psychologist, but I believe I’ll still be having a Tier 4 psychological consultation to sign me off 🤞 for the MDT. Also, two weeks post-approval for the LRD! That is AWESOME! I think my trust tends to do the two-week milk diet. Even though I currently eat fairly low calorie keto, I think the milk diet would help lose a bit of pre-surgery weight at least. 💗

You have been tracking your macros and measuring activity far longer than me but from my little bit of experience and what the nurse practitioner told me I would basically just cut out your extra carbs while you are being less active to maintain and then decrease your calories a bit to lose but I wouldn’t cut too too much. Your body is healing so just don’t be in too much of a hurry and be at a significant calorie deficit. We had no choice with the bariatric surgery because we had to protect our healing, intestines and stomachs with the tiny portions during recovery but it is not ideal to recover from a surgery at a such a calorie deficit. Our bodies actually need extra calories and protein for wound healing so you may actually find that your body is burning more calories than you would be if you were just being a couch potato for no reason. Just something to consider or do a little research on. The reason I say just decrease the extra carbs is because before my activity, I was eating a certain way, and then the nurse practitioner learned how active I was being from my activity logs and she told me I just needed to increase my carbs to sustain that level of activity and decrease it if I slowed down and it has worked like a charm. So basically just reversing that is what I would do if I were in your situation. You could also reach out to your dietitian or nutritionist who gave you your original macro guidelines if that is the plan that has been working for you and ask him or her what they would change given your much lower activity for the time being.

    Not to dis on your friend, but that's just not how this works. The ski slope chart itself may be about what your weight loss looks like for many people, but I promise you, you are losing fat the entire time. You just can't overcome basic physics. Think about it. If you eat less calories than you burn, your body has to make up that difference from somewhere. Even the leanest people on the planet have something like 40,000 plus calories of fat stores. This is what your body uses to stay alive when intake is less than output. Do the math: Your Total Daily Energy Expenditure (TDEE), is made up of your Resting Metabolic Rate (RMR), your Exercise Activity Thermogenesis (EAT), and your Non-Exercise Activity Thermogenesis (NEAT). The average normal sized person has a resting metabolic rate somewhere between 1200 and 1800 calories. It's even higher for overweight and obese people. This is just an estimate, but yours is probably somewhere around 1900-2000 based on your height and current weight. EAT obviously can vary quite a lot from basically nothing if you don't workout, to well over a thousand calories per day if you exercise a lot. NEAT is important as well. The basic idea is that very few of us are completely sedentary. We are moving around cooking, cleaning, doing chores, etc. This is what we sometimes call "Activities of Daily Living". This tends to add another 15% to 20% to your overall calories for most people. The point of me adding all that is because very few people really understand how many calories their body needs per day just to stay alive. If your body needs 3000 calories per day and you are only consuming 800, then the rest of your energy needs for the day have to come from your tissues. If we do things right, the vast majority of that will come from fat, though no matter how hard we try, some of it will come from muscle tissue. Regardless, (and here's the point of all this), there is never a period of time when eating a very low calorie diet like we do after bariatric surgery, when you just lose water weight, or stop losing fat. Early on, you will lose a lot of water weight, but what you are losing is not JUST water. Later, you may experience stalls, but that's not a period of time when you stop losing fat. Instead, you may be putting more water weight back on (this IS part of the healing process), but you're still losing fat underneath that water gain. Note that there will be other times when your weight plateaus or even goes up some. This is 100% normal and not a cause for concern. If you are following the diet plan your surgical team sent you, you WILL lose fat. It's impossible not to. This is why I said it's just not healthy to weigh yourself daily. Scales LIE. They don't measure our amount of fat loss, they just measure weight, and weight is made up of so much more than just fat.
  4. I agree with @SpartanMaker eating more might sound crazy but it works! Resistance training has completely transformed my body. I started incorporating it around 4–5 months post-op, and the weight started dropping like crazy. My body looked significantly better compared to when I was just doing cardio. Around 6–7 months post-op, I added 30 minutes of running at least 5 days a week, and at that time, I was eating between 2,100–2,300 calories daily. Even with that intake, I was losing weight and had actually dropped below my goal weight. I had to take a break due to surgery, and during that time my weight fluctuated upward—which was really frustrating. Even though I cut my calories and kept my macros on point, the scale wasn’t moving the way I wanted it to. But now that I’m back to resistance training (I’m not cleared to run yet), my weight is slowly trending back down again. I say try it! If you would like some guidance I have a few circuits that I do and has worked really well! I am happy to share ❤️

    How to intake more protein and cals

    may not be a popular answer, but if you are looking for a high protein, high calorie, low density food, chicharron (aka pork rinds or cracklins) was my go to back in the day when i had very low stomach capacity and was trying to stop weight loss. worked like a charm. mind you they are also very high in fat (and depending which brand or how you make them, are also high in soduim) so keep that in mind.

    It is always a challenge to know how many calories are really in restaurant food, but generally I think it's a fair bit more than the at home equivalent! 😆 There is an app now that guesstimates based on a picture or camera of the food, but I haven't tried it, no idea how accurate it would be, but may save you some time? :dunno: Considering your current underweight status, maybe focus on just trying to up the cals without worrying too much about tracking? Or does your Dietician/med team/ own Enquiring mind need to know the data?
  7. Honestly the variation in post op dietary advice never ceases to amaze me. I had very few rules - 2 weeks liquid, 2 weeks puree were I think the only guidance I was given. I got most of my information here! I am an outlier in that I adored the puree phase. My first proper food, not yucky protein shakes, for about 2 months. My pre-op 800 calorie a day diet had to be extended when my surgery date was cancelled and I had to be in a state of preparation for a cancellation (that took weeks!) I made family dinner and then just pureed that in a blender with extra gravy. No fibrous veg but I had everything else. I hope you can make it through.

    6 months post op 4 months of stall

    Thank you so much! I actually use Parrot Pal app to track my calories and it gives me an average each week of my calories and protein! That’s the number it generates for me from all my tracking, which I do pretty religiously. I’ll try adding a little more cals it’s just hard because I can’t fit in a lot each day. Thank you again.

    You will always be fabulous, slim and trim. A few kilos up or down will not change that! With that being said, I completely understand your fear and I'm sorry you're going through this. While I wouldn't recommend cutting your calories any further, have you tried dedicated walks? (5-6k steps) as they really help with weight management. From experience, people who don't enjoy conventional exercise can just hit 8-10k steps a day to maintain or even lose weight.

    Weight loss stalled

    I am 51 year old, 5 foot 6 inches and weigh 296. I started at 340. My calorie intake is 800-1000 calories a day set by my surgeon, 30 carbs a day and 64 oz of fluid with 60-80 grams of protein each day.

  12. @SpartanMaker I am not wanting to lose weight really, I am at maintenance however since I got PS surgery I am on the higher end of my maintenance and I would really like to get down to 160-165 as opposed to 165-170. I know not much of a difference especially with my height and build but in my head it makes me feel better. LOL When I was working out (which I will get back into hopefully soon) I was eating anywhere from 2200-2400 calories and staying in that 160-165 number, now I am eating anywhere from 1700-2000 and I am on the 165-170 end. I have to assume that my lack of working out is keeping me where I am at. Which I do want to recomp, I want to get a 360 body lift but want to see how lean I could get before making that decision. I may just want to save the money and pain if I look decent enough LOL For reference yesterday my day was 1961 calories, 185g carbs, 175g protein, 72g fat, 34g fiber. If I was working out and burning 700-800 calories like I was then this is great I would stay at the lower end of my maintenance. But what I am gathering I should be focusing more on the carbs, protein and fat as opposed to calories?

    @AmberFL Thanks for the additional detail! I agree with @ShoppGirl that localized swelling and postoperative edema may be causing a lot of your gain. I do also totally understand the concerns, though! I recently had a procedure done on my proximal hamstring to treat a partial tear. While not nearly on the same scale as your plastic surgery, I wasn't able to workout at all for a full month and even now almost 2 months later, I'm still having to ramp back up really slowly. For example, I'm still not allowed to do any lower body strength training and my running mileage is only at about 50% of where it was before the procedure. Like you, I also saw my weight go up during this time. Even though I knew edema was the likely initial cause, it still messes with your head. This was especially true for me because my workouts are frankly a form of therapy for me. It felt like a double blow. It really sounds like you've got the diet mostly dialed in for now. I suspect one of 2 things will happen as you heal. The most likely eventuality is that your swelling and edema goes down and you find you're happy with where you're at weight wise. If so, be sure to up your calories as your workouts resume. If you find that you are a little heavier than your ideal, then no worries. Once you are working out again, the easiest solution would to only eat back about 50%-75% of your workout calories. This will be slower, but is super sustainable, compared to a drastic cut in calories that would get you leaner quickly, but is also a lot more likely to cause some rebound weight gain. I also noticed something you said in an earlier post and I wanted to commend you for it. You mentioned something about maintaining around 1700-2000 calories. This is something a lot of people don't understand, but you can successfully maintain across a range of calories. Many people think they've got to be EXACTLY at a certain number to maintain. That's just not true. The reason is that most people will subconsciously be a little more active when the calories are higher and a little less when they are lower. Please check in every few weeks and let us know how it's going? I'm sure you'll do great, be we are always here to help if needed.

    Slowing Down 😶‍🌫️

    You can keep losing weight any time as long as you're in a calorie deficit. It's easiest to lose weight early on because (1) your restriction is strongest, so eating at a deficit is practically automatic, and (2) you have the most excess weight, and the more excess weight you have, the easier it is to lose. As you lose weight and become smaller, the calories you burn just by existing (breathing, circulating blood, digesting food, moving around) get lower. If you weigh 200 pounds and eat 1200 calories per day, you'll lose weight a lot faster than if you weigh 150 pounds and eat 1200 calories per day. You can still lose weight, but you may have to be a lot more conscientious about your food intake instead of just relying on your surgery to prevent you from overeating.

    800 calories

    I forgot to post. But according to my dietician I am over eating, they say this is the only way I am gaining weight and that it has to be a surplus of calories. I only eat once a day because of this and I now eat lean meat only. So if this does not work than I quit.

    Pre surgery liquid diet

    I had my surgery a year ago now and I didn’t follow the pre op as stringent as they said just stopped drinking alcohol l and cut down on calories still eat meals but healthy 5 days before I had my last alcoholic drink 4 days before and the night before I had chicken breast and veggies with hummus it’s will be your last meal for a very long time enjoy it but don’t over eat I ate that meal as 5pm and my op was at 12:00 next day hope this helps your going on a journey nothing is going to prepare you for this but it’s well worth it
  18. I had the Orbera 365 inserted 2 weeks ago. In the first week I lost 3 pounds and this week I haven’t lost anything at all. Is this normal? I am eating around 800 calories and walking 30 mins a day. Has anyone else had the same experience? Thanks
  19. im in the midst of a full home renovation and the only thing we have at our disposal at the moment is an upstairs bathroom sink and a microwave. so thus is our dinner options for the night: - microwave KD - canned tuna and crackers - strawberry rhubarb pie - (dark) chocolate covered pistachios - instant ramen - jalapeño corn bread - ...and a screwdriver i made cuz i couldn't decide what to eat omg ....ended up drinking the cocktail (natch) with ½ serving if pie, one forkful of cornbread, and 1/4 a tin of tuna. yay me. ha. estimated 446 calories for what i ate. hopefully i can start eating a little better when my kitchen is fully operational again!

    @ShoppGirl I hear you. Yes I saw the articles from HSS and a few others that quoted that amount, but I've found that sometimes things like this often become an echochamber, even among doctors and other healthcare professionals that should know better. All it takes for this to become the "standard", is for a 50 year old medical textbook or two to print this (based on old data or beliefs), and an entire generation of doctors come to accept it. Look no further than BMI as an example of a well meaning, but ultimately wrong calculation becoming the standard by which we judge obesity and you can see how this sort of thing may not be based on science, but on "generally accepted knowledge". Back to the subject at hand. We do know that RMR can and does increase during recovery, but the amount is not always the same and is highly dependent on both the injury/illness, as well as the individual themselves. As a general rule of thumb when talking about athletes like @AmberFL, the number of calories needed during recovery is typically a little higher than maintenance, but quite a bit lower than what they might have needed when working out. That said, we also need to recognize that Non-Exercise Energy Thermogenesis (NEAT) can vary by many hundreds of calories between individuals. This is important, because even though RMR may increase, it's logical to assume people will have a concomitant reduction in NEAT when ill or injured. The net effect of that may mean the calorie balance may not really change that much if at all. Here's an example: Let's use a 3 factor calorie model, meaning RMR, NEAT & Exercise. (I'm going to ignore TEF for now since it won't make a lot of difference here). Let's assume an RMR is 1500, since this is around the average for all humans. Let's also assume a NEAT of 600 for a total calorie burn before exercise of ~2100. I've selected this since it also is fairly close to the median. Obviously these are just averages and every individual will be different. At an RMR of 1500, if we use as an example a 25% increase due to illness/injury, that would mean the new RMR would be as much as 375 calories more. We would expect NEAT to drop in most people when ill or injured because they tend to sit/lie down a lot more, walk less, etc. Especially in people that have a high NEAT amount to begin with, a drop of 375 calories per day or more is easily within normal ranges when they don't feel good. The net result in our "average" person here is that while their RMR went up, their NEAT went down by the same amount, thus completely negating any need for additional calories. At the end of the day, like you pointed out, we all have to decide for ourselves what we believe and what we think is right. I'm certainly not opposed to anyone consulting with their doctor regarding nutritional needs, as long as you recognise that most doctors get, at best, a single course in nutrition in medical school. Many get even less. I'd tend to suggest a Registered Dietitian instead, especially when dealing with chronic or acute injury or illness. Full disclosure, my background in this area is in nutritional needs for athletes, not people that are unwell. The flip side of this is that I find there are very few RDs that have the requisite knowledge to properly coach athletes, since so much of their training goes into how to help with weight loss, or assist with the nutritional needs of diabetics, cancer patients and the like. We also know that there are some really lousy RDs out there as well, so caveat emptor. (Otherwise, why would we see some of them trying to get very active bariatric patients to stay on an 800kcal/d diet.). Any RD that does that is, i'm sorry to say, an idiot. I'll close by saying that I don't doubt you in the least that you needed to up your calories by a significant amount due to cancer and chemo. I'm sure you recognise that's really in a different league to what most people are going through when recovering from an injury/illness or even surgical procedure. In your case, you have both the fact that cancer cells are effectively using calories that should go toward healthy tissue, as well as the fact that chemo is incredibly destructive to the rest of your body.   21. Hello! I had the sleeve gastrectomy 9.5 months ago. I've been working on exercising and eating right, but I've been in a bit of a stall these past 5 weeks. I went down from 290 to 170 pounds, but I've been stuck at 170 for 5 weeks. I've been eating 1200-1500 calories a day and still not losing weight. I've done some research, and apparently, your body adjusts to low calories after a while of eating at that rate. At the beginning of my post-op phase, there were some complications on my end. I didn't eat ANYTHING for two months straight, not even protein shakes, and I was only hitting about 20oz of water daily (which landed me in the ER, but I'm fine now lol). I've looked online, and it says for my height, age, and weight, a good maintenance level would be 2100 calories. I'm eating well under that in a deficit and heavy weight lifting, so I don't know why my body won't drop anything. I'm worried that my body adjusted to the 0-calorie few months I had, then the 500-1000 calories three months after that. I've only started hitting my 1200-1500 calories in March when I joined the gym. I know the stall is not due to "muscle gain" because I'm not eating in a surplus, and I'm only eating 65-80 grams of protein in hopes of simply maintaining while I drop fat. I also read online that apparently people who go through rapid weight loss have even lower calorie maintenance than the average person, and that makes sense, but surely it cannot be under 1200, right? The majority of bodies need 1500 to operate. I'm so confused! When I ask my surgeon about calories, he says not to worry about them and eat healthy, which I'm doing. It's just frustrating because I want to work on building muscle, but I want to lose some more fat before that. I guess my question is, does anyone know anything about calories after surgery, and/or how many calories are you eating after surgery to help lose weight?

    @Mspretty86 oh that is great! My food noise came back around a month ago, basically when I halted the exercise and got my P.S. I felt really outta control, but now Im back at it, I always meal prepped but my snacking was outta control so now I do 3 whole food clean meals. 1 snack and dessert like protein ice cream. Making sure I stay in my calorie deficit but eating enough to get them gains in the gym lol
  23. I was sleeved in 2016. Lost weight, but no where near goal. Gained it all back. I just had a revision to a bypass this week (Jan 13). Things are going fine, almost too fine. Minimal discomfort, I have no problem keeping fluids or protein down. I am eating about 600 calories per day with 80 grams of protein. I’m not hungry. (I am on a 3 week liquid diet post op at the moment.) I also haven’t lost any weight. Does this sound normal?

    @swimbikerun yeah I eat protein bars but shakes are difficult for me bc I can't drink a big amount of liquid. I know, I know I'm difficult 😂 I really like the protein shots, but bariatricpal has been out of stock of them for quite awhile now unfortunately! But other than those things, I just didn't know what types of foods or meals there are that are rich on protein and/or calories but aren't a huge amount of food! Thank you for replying tho I appreciate it 😊
  25. Hey lovely people, There is already tonnes of info out there on maintenance, but I just wanted to see if anyone had any tips? My two sisters previously have had a bypass, and they've both dropped into the very underweight category, and this is something I'd like to avoid if at all possible (I know my body will decide where it wants to be, ultimately!) I am trying to ignore that little voice in my head that is making me feel a bit scared about eating more, and just plough on. I don't track calories at the moment either! Any tips are greatly appreciated ❤️
