Will the DS fix my reflux and weight gain following the vsg?

I've posted my story several times in other threads which you can find by clicking my name. To make the long story short(er), I had my sleeve 8/22/2013 & then on 12/29/2015 a revision of the (dilated) sleeve & the single-anastomosis duodenal switch. I also had my hiatal hernia repaired at the second surgery. A hiatal hernia can certainly be a cause of reflux symptoms. Personally, I would not have a gastric bypass. I don't like the artificial opening from the stomach pouch to the intestine. The duodenal switch (classic or SA-DS) has the sleeve gastrectomy that leaves you with a fully-functional stomach, just much smaller. I chose not to have a DS at the time of my original sleeve because the amount of malabsorption scared me. But 2 years later I was still at a BMI of just over 35, on 4 insulin injections a day, Metformin, atorvastatin (Lipitor) & 3 blood pressure meds so I decided something more had to be done. I am now 2½ months post-op & am off all insulin & the atorvastatin. One blood pressure med is discontinued & the other (a combo) is half what it was & I will probably be off of it by the end of the month. We did try decreasing the Metformin, but that didn't work - yet. And - drum roll - as of today, my BMI is below 30! I am no longer obese. -

As others have all said if you have reflux DS will not cure it. Your stomach stays the same the only thing that changes is the re-routing of your intestines. Personally I went from VSG to GB in September 2015 and haven't looked back. I had also considered DS but am so glad I revised to the GB. I have had no issues with dumping at all. I beleive I needed the smaller stomach created with the GB and the malabsorptive part in the bypass. As said definitely ask your surgeon what he suggests but if you have reflux only answer is GB. -

a TON of people have had lapbands revised to sleeve or bypass for the reasons you mentioned. It's either that or risk being miserable the rest of your life. I'd go with a revision if it were me. Either of those two surgeries would be a good option, although if you're suffering from GERD, you may want to consider bypass. It often improves GERD, if not outright cures it. -

@soccermonster31 - Yeah, growing up in the 80's/90's there was a LOT of misinformation about weight loss and management. Eggs were terrible for you, white carbs were optimal for fat burning (lol), coffee was the devil, and margarine was soooo much healthier than butter! I'm not angry with my mom. I know she did the best she thought she could and probably thought she was saving me from obesity. I'd love to be support buddies! When do you plan on revising? It'll be a while for me - we're going to move in the next few months and I want to be settled in our new house before undergoing a surgery. For now I'm gathering information and planning to do a consult with a bariatric center in Indianapolis. Fun story: I got my lap band in December 2015 and the surgeon, without warning, shut down his practice in January 2016. I had to go to a bariatric center 50 miles away for my fills and counseling when I'd anticipated he'd be right there down the street whenever I needed something. My lap band did not have a great start, and I wasn't super impressed with the bariatric center I was referred to. Luckily I have some choices! -

Kim, Answer to your first ? A Port Revision is when your port becomes disconnected. I had to have my port removed and a new one put in a different site and reconnected.This does not happen very much anymore because the company that makes the band has fixed the problem. Now your second ? .80cc's is less that 2cc's. Hence the higher the # the tighter the restriction. To me it seems that you would have to get more cc's to get better restriction. But since I had my revision being at .80cc is just like being at 2cc's for me. That is what is great about the band. It can be ajusted to fit your needs. XOXO~Teresa -
I had 4 with my band surgery, and they used all of those plus one other one. I had sutures with band surgery over the port incision which my revision surgeon used the same incision to remove the excised portion of the stomach. With my revision incision, all they used was a type of skin glue, no sutures, no staples, and it took almost 3 weeks for the skin glue to finally flake off in the shower. It wasn't the blue/dermabond type of skin glue. It was pretty cool because I had zero sutures or staples. Because of my complications, I did end up with more incisions. I have a total of 10 or 11 scars on my abdomen, but all of them are shrinking and fading pretty good. I did buy some Mederma, but I'm not good applying it on a regular basis. I caught it on sale for $20 from skinwest.com I heard that prefer-on stuff works to help fade scars just as good as the Mederma.

I am 2 days before surgery but I have mixed emotions. I am nervous because I worry about complications because I am a VSG to RNY revision. But I am excited because I am one step closer to starting this new journey

Any Happy Bandsters Out There With 5 or More Years Of Banding?

I credit my band with helping me lose, but I did the bulk of the work to lose. I will not discredit by band for my gain, coz it doesn't choose what I put into my mouth. You think it's sad that people take responsibility for not following the band rules and gaining? Who else is responsible if not them? I'm more saddened that anyone thinks that nothing that happens to them with regards to their weight is their responsibility. Maybe that's the difference here, and I'm glad that I don't think that way. Anyway, back to the topic, I'm 5 years banded now, did spring a leak about 18 monthsw ago, had 2 port replacements and 1 band replacement to finally resolve it. I gained 35lbs in 10 months when the band wasn't working and I lost about 40% of it when I realised how much I'd strayed from my initial goals. I then got rebanded. The reasons for chosing a band over sleeve are in this post: http://www.lapbandtalk.com/topic/165592-full-band-replacement-experiences-sought/ I won't say that initially it was an easy decision to make, which way to go in terms of revision. But when I talked it out with my physican and my family, it then became a no brainer. The band gave me a great, easy 5 years and if I get another easy 5 years out of it, I'd be grateful. If it lasts longer, than yay for me! It worked the first time and is working just as well this time for me. So I am absolutely sure I made the right decision. -
I can only speak from my personal experience and I had the RNY procedure, not a revision. I also have a high tolerance for pain mostly because I have had 5 spinal surgeries and they are pure hell. Honestly I found that first the first couple of days pain meds helped with initial surgical pain but honestly the real pain mostly came from the gas they pump into your belly for the procedure. Pain meds don't do anything for that at all. They simply don't touch it. the only thing that worked for me was walking walking walking, sipping Water and a heating pad. The other pain I had was when my bowels were starting up again, that can be very uncomfortable but I honestly found that chewing gas X tablets really helped that situation. I took liquid Vicodin the 2 days I was in the hospital and the first day at home. After that I moved to liquid Tylenol and honestly the alcohol in that simply did not feel very good so I quit taking pain killers all together on day 2 home from hospital. By the liquid Vicodin was horrible feeling to my tummy too. It honestly made me not want to take it. It really wasn't that bad but I do know that others really struggled with the pain.

Ok then, well I'm glad to know you found out about your erosion! Those are so dangerous and so good of Dr. A to stop the process and let you heal first. I don't know of anyone who had erosion and revised to sleeve or I would lead you there - sorry. Good luck in your research! I hope some will reply. -

Posted this in the pre op forum but wanted to post it here also. I am scheduled to go from a lap band to a gastric sleeve and I am so nervous. I have made the mistake of reading EVERYTHING online about the gastric sleeve and it has frightened me to the point of backing out. I am on the verge of calling my doctor and telling him I've changed my mind. Is anyone in the same boat as me? Those of you who have had the surgery, were you satisfied? Is it worth it? Will the weight loss be better than the lap band (I lost only about 20lbs TOTAL)? I appreciate any comments and advice/suggestions. I posted this in the pre op forum but wanted to post it here as well. -

I can't thank you all enough for your responses!! @asherje...I've had my band for 3 years and I can't wait to get it out. Great to know you are happy with the revision! Just curious; is nobody suggesting the mini gastric bypass (or other primarily malabsorptive procedures like BPD-DS, SADI-S etc.) for revisions? Results in scientific studies are very promising specifically on revisions. -
I also have Qualcare from the hospital I work at in NJ. They covered everything! I was sleeved on 7/17 with no problems. I don't know anything about revision, maybe just some extra paperwork? Good luck!

I am a 5 day a week at the gym girl and this was a major question I had for my doc when I made the decision to have band removed and revised to sleeve. He wants me walking from day 1 and then aerobics (cardio classes, spinning) at 3 weeks and than I can resume strength training, abs included at 4-5 weeks post-op depending on how I feel. I became an exercise fanatic during this past year with my band and I swear, that is the only reason I've lost any weight! Good luck.

Had my last dietary appointment this past Monday. Now they are reviewing to make sure they have everything and then will submit to insurance for pre authorization. I have zero patience when it comes to things like this! The office told me can take 3-4 weeks to hear back. Oye! What has everyone's experience been with this? My surgeon is also booked 2 months out, so looking at a September or so surgery date. A little background on me... I had the lap and placed November 2007. Lost 80 pounds but have struggled the entire time with finding a good fill amount. I am so tired of throwing up and basically cheating with food because the greasy/condiment filled food goes down better. The last 7 years have been filled with frustration (because I want to be healthier/eat healthier) and my band just seems to work against that. Against me. Ultimately after one pregnancy and all this time I have regained most of my weight. Has both knees operated on in February due to the pain. My body just doesn't handle all this weight well. Plus I've been overweight since childhood. So that doesn't help. I am hoping to have the band removed and a bypass revision completed in the same surgery. Anyways. Sorry I babbled. I am having my band to VSG surgery tomorrow morning and I just want it to be 24 hours from now and it will be over. I know this is normal but man, I was not this anxious and scared with the band surgery. I think a lot of it was kind of like having your first baby, you are scared because you don't know what to expect and with the second your scared because you know what to expect. Except with this, I have the added worries that I have scar tissue from the band, major organ getting removed, etc. So its not really the same thing, right? The following is my post on my blog today. I thought I would share it over here also. BCBS CA here as well. What were the insurance requirements? I found out I'm covered (I am the employer and I guess got lucky that I picked the right plan that covers VST!). I am a lapband revision, but I'm concerned that I would qualify for a new procedure (33 BMI, etc.).

Hey everyone, Just wondering if anyone has had BCBS of New England and gotten approved for the lapband revision to gastric sleeve. I've had my band for 6 months now I have lost no weight i've had fill after fill I feel terrible I'm getting acid reflux at night and I'm getting things stuck left and right. I go see a new surgeon on the 28th and I'm hoping he can help me out my old surgeon only did the band and he made me feel bad about myseflt the last time I saw him because I gained 5 lbs. I don't like the way the band makes me feel and I can feel the port all the time it really sucks. I wish I had know about the sleeve before the band because I would have gotten that instead. Keeping my fingers crossed that this new surgeon will be able to help me out thanks

Hello all! I am new to this community! I had my first appointment with my surgeon today, and I love him, he's really great. We talked a lot about different surgeries but I'm still really undecided and I'm finding myself obsessing about it even though I have several months to decide. So if anyone wants to weigh in (haha, no pun untended) please do. I'm in my early 30's, female, BMI of 44. SW is 255, GW is 125-135. I'm 5'4. I think that's all the applicable information. Anyways, I have a good friend who has similar stats but is slightly older then me. She had the sleeve and stalled out at 6 months after only a 30lb loss and started regaining. She ended up getting the Duodenal switch which has since worked fabulously for her. She has met her goal and is maintaining. My surgeon has never performed a DS but is not against it. I do not mind being a pioneer patient and I definitely trust his experience. He is not really swaying me in either direction because he wants me to make my own decision. He says the majority of his patients get the sleeve and tend to do really well with it, and the weight loss center has a really nice aftercare program. It is pretty important to me to hit my goal. I have another friend who counted calories to lose about 100lbs and has now regained it all plus some. Over being skinny, I want to be healthy, but I also want to feel comfortable in my own body, have the self-confidence to be seen in public again, etc. I'm sure a lot of you know where I'm coming from here. I also really don't want to have to come back and pay for a revision. I have kids and I have a lot of things I could be spending those thousands on instead of a second surgery. So I'm sitting here bouncing back and forth between not losing all the weight I need to with the sleeve and being worried about the harsh side effects of the switch as well as having concern for the severe malabsorption as I age into my twilight years. But then again I won't live to see old age if I don't get a minimum of 100lbs off. Help. Please. And yes I Know the surgery is a tool and should work if I use it well, and I plan to, but..I don't know. Just help please. -

Can you go to another doctor without endangering your health? He should be willing to talk to you about what might have caused it (they don't always know) and discussed options with you, like a "revision" surgery to the sleeve, for example. -

After two years of having issues with my band, I went to see a consultant. I really, really wanted a bypass as I had failed miserably with the band!! Straight out he said he would do the sleeve! The bypass was not even an option he mentioned. I was absolutely distraught and cried and cried. However, today I'm the happiest I've been for a very long time. I'm almost 7 months post op and have lost 104lbs. My start weight was 397lbs. My sleeve has been a Godsend and I'm so happy he never entertained the thought of a bypass. Band to sleeve revision peeps are having great successes -

Went to see DrProvost today (I have second with Dr Cribbins Wed)... I told dr provost I wanted to revise band to sleeve. He said he will do band to gastric bypass. He said the sleeve is not good for banders. Had I never had a band he would do a sleeve but because I do have a band he recommends the bypass. Which I am scared to death of. He said people with bands that have sleeves I told him I didn't like the thought of bypass because of re routing everything and dumping syndrome. He said dumping happen when you eat sugar or high fat foods. If you eat that you will feel very sick, and faint like you want to pass out. All the things I worried about. He said well I should not be eating that anyway so I should do bypass. I fully understand that I must change my eating habits and that sugar and high fats are off limits... However the thought of NEVER having sweets really bothers me. I think if every ONCE IN A WHILE I want to share a piece of birthday cake with my son I would like to be able to have that option once I react my goal weight. Hopefully I'm not wrong feeling that way. My question is has anyone else been advised to so gastric bypass vs sleeve and what did you decided an how did it turn out? Also, has anyone had leaks since sleeve revision. Advice needed and appreciated -

The first surgeon I went to insisted that the best thing for me was bypass even though I told him that I was absolutely not interested in that at all. He grudgingly said he would sleeve me if I insisted, but every visit, he rammed bypass down my throat. I am NOT a band revision, but he could give me no good reason why bypass was better than sleeve other than his thought that I should want to dump if I ate bad things. Uhmmm....no. I want to live a fairly normal life. I am not a huge sweets eater, but I do want to have a bite of cake or a scoop of ice cream from time to time. His nurse was a bypass recipient and she was also extremely pushy about having bypass over sleeve for the same reason. When I told her my feelings on it, she acted as if I was nuts and just like the doctor, told me that dumping was desirable. WHUCK!?!?!! I changed doctors and I am SO glad I did. SO GLAD. Edit to add: I was 4 months into a 6 month process and had to start all over. Still glad I did it.
