    Question for WLS veterans

low carb, yes - good and sustainable. keto? NO. do not start with that. unsustainable, unnecessary, and damaging in the long run. i know you are in ketosis now, largely bc of the sharp calorie decline and your body utilizing so much stored fat for energy. but seriously do not start with the eating 80% fat thing. not because fat is bad for you, it’s not, but because you should not begin ANY eating plan that you can’t sustain for the rest of your life. because guess what happens when you get off of it. yep, weight gain regain. i follow a generally low-carb (under 50g carbs per day), whole foods diet. kinda paleo, kinda Mediterranean, but always REAL FOOD. makes it all quite simple.
  2. @Mikeyy 100g a day is fine for a man. Some kidney pain can be normal in the early days of a keto diet as they are working a bit harder than usual. I start my male clients on 80-120g a day and 20g carbs. (I trained under Professor Tim Noakes and am certified to coach people in keto- fit in with my specialist interest in training as a specialist nurse in diabetes you see & certified in dietary advice! Yep having all that knowledge isn't always enough, hence me being here post-surgery!). You really do need a good amount of water to help flush stuff out as Fluffy already said. If it continues for 2 weeks or gets significantly worse or you feel unwell go and see your Gp. Healthy kidneys can handle it fine, people with a history of kidney issues have to have much less, but a week on that amount wouldn't cause an issue. However, what can cause an issue is bariatric surgery and that can cause kidney stones to suddenly appear seemingly out of the blue, hence my comment about seeing your Gp if you get severe pain.

    Annoying!

Since being back on the blood pressure meds I dropped 5 pounds....jippeee,and the bounced back one pound.I've also been having an uncomfortable pain in my kidney area and tonight when I got on the scale at 23:00,yes I know I shouldnt do that at all ever, I am up another 1,5 pounds.Did the keto stick and my goodness,I am in deeeeeep purple ketosis for the first time ever in my life. A few things might be causing this.It is now up in the 100 degrees (and over) every day and very humid.I have been struggling a bit with the water intake but not too bad.So I might be dehydrated a little. However I dont look or feel it. Then it might be that I am high on proteins.I have found these nifty little chicken breasts (frozen with a lemon and herb sauce) and that has been my main food for the past 2 days.2 of them weighs 80 grams and I have 22g of protein and I have had 6 of them yesterday and today.Breakfast, lunch and supper.Add to that about 5g protein for milk in my tea and a little in the veggies (2 tiny pieces of broccoli and half of a very small yellow squash that comes from SA and is low cal and low carb) Say it was a total of 75g for the day.The carbs was about 50 as I had a teaspoonof hummus, milk and veggies and a SF popcicle as well. If the pain persist tomorrow I will phone the BP dr. However,I have realized that I will have to start exercising pronto. As in today. To be sure that I do the right things and to not drive myself crazy all the time second guessing weather I am doing the right things,am I eating to much (I know with my head this is not possible) but my emosions tells me differently. How I hate the way the scale freaks me out.the only way I know to control this is to back to dieting the way I know well.I didnt want to obsess with food tracking, low carbing but I will have to.I also believe I should now start making hay while the sun still shines and lose as fast as I possibly can,and believe me,I have put in no effort what so ever until today. It was just lovely for it to just happen but I cannot take this out of control feeling any longer. 1.I will start by just doing walk away the pounds or some dvd here at home.I will play squash twice a week. Next week I will start going to the gym. The thing is for the first time ever I am so self concious when at the gym.I just am not motivated to expose myself to all the thin people...lol.(and we have a ladies only gym at the club as well) So,this is me for today.Crazy, obessed over weight gain that is not real and petrified because I might fail. Tomorrow will be the positive, new me....lol I have read some of my friends blogs and it is all so upbeat and positive and it seems that I am the only one with this crazy roller coaster emotions...or I am just an external prosessor that talks about it.And maybe I have to start fake it until I make it and not express myself the way I do.Maybe I will pretend that everything is just perfect and maybe it will become just that. xxo

    Do you find it hard?

I had and still have all those same fears. Its scary thinking your life will change just from the way you have to eat. But you learn so much about yourself, your body, and the changes you need to make to get the results you want. You may need to stick to a keto style diet until you hit goal, you might not. You won't know those things until your eating regular food and are able to test better how your body tolerates it. All i can say is TAKE. YOUR. TIME. With EVERYTHING. Especially introducing new food, also eating takes time. I'm a year out and it still takes me almost an hour to eat 3oz of food. But if i don't take that time and am rushing it i WILL make myself sick. Its not worth it. Weigh your food. It sounds obsessive but its the single best way to track your intake. I slacked early on but its really helpful with tracking macros. Understand that the weight comes off very differently for everyone. Everyone will have stalls, it just comes with the journey. Embrace the stall and look for other measures of loss. I know this is a lot and its barely the tip of the iceberg. We're all so different and every surgeon and nutritionist has a different plan. Follow what plan they have for you and keep a close relationship with them. I wish you well on your journey. Hope i offered a little insight. Do you find it hard?

I’m getting sleeved November 7th. I’ve been low carb/ keto for 10 weeks now. I’m lost 25 pounds. I didn’t realize how carb heavy my meals were till I started keto. I didn’t find it hard to change the foods. I’ve had a harder time giving up my pop.

I noticed this before my weight loss surgery journey when I did a Keto diet and now post bypass. I was on the verge of getting a double hip replacement due to arthritis. Now I think I’ve got some more years in my hips.I’ve even been walking and biking like crazy because now I can. 😀

    Stalls

Reposting something I posted elsewhere: First, the most common reason for any stall, including the dreaded 3-week stall is simply that you are retaining more Water. There are a few reasons this happens, but around the 3-week mark it comes down to the fact that your body is adjusting to a low carb intake after converting from glycogen to ketones for energy. More water is needed to burn glycogen, so you were retaining more water pre-surgery than you are now that you're using primarily ketones for energy. At about the 3-week mark, your body eventually recognizes that the water loss is long term, so other mechanisms have started kicking in to start holding on to more water. This water loss due to the change between glycogen and ketones is the primary reason for "keto flu", and something that increases your risk of dehydration early after surgery. The other "reasons" for plateaus really don't apply as much at the 3 week mark, but will become more important later in the process. Let's cover those too though so you're prepared: As you lose more weight, your overall metabolic rate will slow down. People sometimes think fat is not metabolically active, but that's not true. Less fat means your metabolism slows down, even if you retain the same amount of muscle. As you lose weight, it takes less effort to move, so the amount of calories you burn from activity also drops quite a bit. As you progress, you'll be able to eat more and especially if you're not tracking intake closely by weighing and measuring your food, you can easily be eating a lot more than you think. This one may be TMI for some, but you may simply be retaining more stool. This is going to be hard for some people to hear, but I can tell you one thing it's not, and that's hormones. Yes, various hormonal processes negatively impact weight loss in a myriad of ways, but they don't overcome basic physics: if you eat less than you are burning, you'll lose weight. What these hormones can do if they're out of whack, is not good things like slowing your metabolism, increasing your hunger, screwing with water weight, or even fooling you into thinking you're eating less than you are. So, with all that sciency stuff out of the way, what do we actually do if we're in a stall? Well, I think it depends on when it happens and how long it lasts: If it's early (a.k.a. the 3-week stall), just keep doing what you're doing and you should be fine. I know people don't like that advice, but as I said, it's just water, so don't worry about it. Later on, especially if the stall is lasting longer than 2-3 weeks. that's when I think it's important that you look closely at what you are actually burning, as well as really tracking what you are eating. If you don't know your BMR (Basal Metabolic Rate), you should. The closer you are to goal, the harder it is to get the balance right between intake and output to make sure you're not eating too much, so fixing this starts with knowing your BMR and accurate food logging. There's a lot of anecdotal advice thrown around about "ways to break a stall", but there seems to be little scientific evidence for any of it. Most of it certainly won't hurt to try, e.g. breaking up your routine in terms of diet or exercise, but just understand that this change may or may not have had any impact. If it makes you feel better to be proactive, go for it. Longer term, there is one piece of solid advice that's backed up by lots of research: Even if you don't really like working out, do it anyway. Those WLS patients that make a regular habit of exercising for 45 minutes to an hour most days a week are significantly more likely to reach their goal and maintain the weight loss. (One caveat here: significantly changing your exercise routine can make you retain water and possibly even add muscle, so don't freak out if you see a stall or even a gain.) I'll take that exercise advice one step further and say you really should be doing some form of strength training. This is also backed up by lots of studies, but the great thing about strength training is that it makes your burn more calories even at rest (in other words, it increases your metabolic rate). There are lots of other benefits, but the metabolic benefit is the most germaine to the stall question. Those that do strength training are less likely to stall during weight loss.

    Starting to cheat

You are soooooo close to your goal - CONGRATS- which is why weight loss has probably stalled for you, BUT I totally get it since I also am a person who needs results to spur me forward. Its the A+ on the scaled when you drop weight that makes you want to keep going because its a feedback loop so its so hard when you are on track and dont see results. That said, you are not a sinner, you are human and its about being consistent. So if yesterday was a crappy day, today - one meal at a time- try to make it a good day. And worst case scenario- and not everyone might agree with this- I try to find "emergency treats" to have on hand in the house that aren't as bad as real treats, that I can have guilt free that won't harm my diet. I love chocolate too so i got chocorite chocolates and each packet has two "peanutbuttter cups" in it. they are Keto snacks with low sugar but still fulfill my sweet tooth so if I have to have chocolate i grab one of those and i dont feel guilty because its so low cal and low sugar. That said, if you are the type to eat four of those then thats not good BUT if it just takes a little to take the edge off I think they are worth having. I have also made my own sugar free peanut butter cups (super easy to make) with Lilys chocolates and PB fit powder and i freeze them. They are super cheap to make and each one has about 25 calories and one takes the sweet tooth away. This way stay on plan 99.9 percent of the time and if you need a small treat- have it without guilt. Holidays are hard, this is hard, very very hard but you can do this!!!
  12. I'm 6 days post op. I started the full liquid diet today. Had a keto fudgsicle was okay'd by my surgeon. About 10 min after, I started to feel light headed, flushed and heart racing. Yep.. dumping syndrome hit. Guess that's out. I'm struggling not to eat reg food. I want corn chips and cheese. 35 years of over eating, and eating junk when ever I wanted.. hard to adjust too. Anyone flub up and eat solids on the full fluid post op diet? And what happened??

    Denied and Grieving

I am 50 and did keto this past year. Well, when I intensely focused on it, in about 9 months and NOT being able to exercise at all due to torn knee and it had not gotten warm enough to get in pool, I lost 45 lbs in that 9 months (although keep in mind I weigh a GREAT DEAL more than you so consider your possible loss on more of a % of ttl basis & if you can manage some things at this time of year like getting on pool, I still think you could lose a nice amt of weight...but I'm also so angry/frustrated on your behalf..but want to encourage you as well if Keto is your only option left) My primary dr. is huge fan of keto but my bariatric dr is not. I actually do not have cholesterol issues but my primary, who I have known now for over 20 yrs, has had other patients whose cholesterol, diabetes, etc. improved. However, I think 'impact' of keto on things like cholesterol can be very individualized. Also, my bariatric dr said if people didn't go to the 'extremes of keto' like eating a ton of beef each and every day, if they limited some of the things that we know often impacts cholesterol, then he'd be more a fan of the general approach. So yes, even at 50, I lost 45 on Keto and that was not working as hard as I could. 18 yrs ago, I lost 80 lbs on Atkins and kept it off for over a decade as I was finally able to be active and yes, was MUCH younger at the time, but hey, I did lose on Keto. I'm torn for you though and I apologize if you've answered this 100 times...does your insurance not have an Appeals process??
  14. All of the studies on this, many done from 2012 to as recently as feb this year, show a better preservation of REE with very low carb on calorie restriction (regardless of the calories) when compared to moderate/higher carb intake with equal calories. I'd also bet your nutritionist will spout the traditional rubbish for the conventional "SAD" guidelines (which are ludicrous)... the "permanently" lowered REE is a myth too If you're interested, do a search for REE preserved on VLCKD ... you'll get plenty of results.. read the studies, not the stupid magazine summaries that often miss the point entirely.... another interesting take-away with these studies is the lead time to adapt. The preservation of REE was best expressed with people who are already keto adapted (that is people whose bodies are already able to use ketones as a fuel). Dosnt take long. and of course, permanently elevated insulin levels with frequent meals throughout the day with carbs will always shut off fat burning... I dont understand why people dont get that. Insulin is the fat storing hormone

    Dreamfields Pasta?

I have used Dreamfields for at least 3 years now. My diabetic mother in law also uses it. She test her blood sugar after eating (as she always does) and she does NOT have a spike in blood sugar like when she eats regular Pasta. I used to test with Keto strips and would eat the Dreamfields on my low carb diet. I never went out of ketosis from the Dreamfields. So, my conclusion is it may not work for everybody but it works for my mother in law and for me. Your mileage may vary...

    Any advice...

I’ve been doing Keto for a year, I’m 18 months post sleeve. I’ve really turned off meat and chicken though. I have a piece of fish most nights. I do like bacon but it has to be really crispy for me to enjoy it. I find it very easy to eat salads, with egg or avocado and spinach leaves. And love nuts and seeds. So I make my own nut granola and if I want bread or a sweet treat make it myself Keto friendly. I tend to eat the same things every day. I like Keto because it reduces my appetite. I still have a lot of restriction though so that helps too. The sleeve has made it easier for me to stay Keto and give up bread and potatoes as they are hard for me to eat.

    Any advice...

One should always take conversations about which diet is best with a grain of salt. This choice is as personal as they come. Diet is a hot button issue and we're still learning a lot about it. In particular, we're learning about the kinds of bugs different diets grow in your GI tract and how they contribute to defense against disease and weight maintenance. I think those bugs....our gut microbiota....will hold the key to future developments in nutrition and the treatment of dozens of illnesses, including obesity. https://www.ncbi.nlm.nih.gov/pmc/articles/PMC5433529/ Did you know that you have about the same number of bacteria cells in your GI tract as human cells in your body? We're complex symbiotic organisms who count on those bugs for our health. Half man...half bacteria....here we come:) Keto works. You lose weight fast, and it helps to control your appetite. However...diets high in animal protein and fat are correlated with increased risk of cancer and increased mortality from other causes. Keto does a spectacular job of wiping out your gut microbe diversity. This isn't a good thing. Thinner people, and healthier people...are documented to have higher gut bug diversity. People who die young and have chronic illness...have low gut bacteria diversity. The people in cultures who live the longest on the planet, with the lowest rates of cancer and heart disease....eat diets high in complex carbs, dietary fiber, fruits, nuts, legumes and vegetables...and are more sparing in proteins and fats. People in these cultures also exercise. And again...they have highly diverse gut microbiota. If you want to look at it from an evolutionary perspective...what diet have millions of years of adaptations prepared us best for? Look at the natural diets of the great apes. They eat a ton of fruit, veggies, and very fiberous foliage. They eat legumes and insects (a terrific source of fiber)....and only opportunistically and sparingly... eat eggs and meat. Their fat sources and most of their protein comes from plant sources. Their gut microbiota is also very diverse. And they exercise. Interesting pattern, no? My personal feeling is that keto is an effective tool that can be used for short period of time because the benefits outweigh the risks....sort of like chemotherapy. Ketotherapy? Being morbidly obese carries risk. But for a maintenance, lifelong diet......I am convinced that a diverse gut microbiota is the key to good health, and will eat a diet that supports it. At this stage in my obesity recovery...for me...I believe that keto carries more risks than benefits. Best wishes to all...and respect meant for everyone's choices. Everyone has a right to eyeball this choice on their own and make the decision that makes the most sense to them. Food for thought: https://www.medicalnewstoday.com/articles/273533.php

    Keto

Anyone start eating keto right out of the gate? Like as soon as you were off of soft foods, etc? I'm still a few weeks away from the normal diet phase, but I'm not thrilled about the idea of living off of sugar-free/fat-free products. I'm a bit concerned that I won't be able to tolerate the fats associated with keto, so I'm curious what others' experiences have been.

    Keto

@@Laughkc11 First time I heard about the Keto diet was a year after surgery. I liked that it burns fat instead of using carbohydrates for energy. I eat avocados, real butter, and eggs. Pork sausage is also hard for me. I still eat turkey sausage.

    Cookbooks

Do any of you use any cookbooks that are Bariatric friendly, or keto friendly?
  22. My standard *lie* was Keto/ Low Carb because it was the food plan closest to what I followed immediately after surgery BUT if I had to do it over I would confidently say :
  23. Yeah..same with me on any carbs...fruit, oatmeal, etc. I had done keto (under 20 grams a day) for 15 months prior to talking to Cigna about surgery..and my dr is supportive of low carb/keto; however, he did want me to change things up as I'm a very slow loser. I figure I have to at least try to follow what his plan is...so I'll just keep following the macros he set which has me at 60 grams a day for carbs...80 grams for protein, and 27 grams for fat. It's an 800 calorie a day (or less I guess) plan with 40% coming from protein; 30% coming from carbs and 30% coming from fat. Fluffychix recognized what it was right off...I just seem to forget even more things each day.
