Type 2 db, apnea, high cholesterol - BMI 28 - is surgery an option?

I was recently diagnosed with type 2 diabetes and recently have been having sleep apnea issues. I've had above-normal cholesterol for a few years. My BMI is 28, I am 56 years old. I've tried many many times to lose weight. Are there any weight loss surgery options for me since I do not have a BMI over 30? Thanks in advance for the advice. -
that sounds so wonderful.. totally there with you with the idea of puréed meat!! i am looking forward to surgery and getting to puréed just for being to eat the plant protein Good lol refried beans, stewed/puréed lentils and split pea soup will be such a treat

Update! Some positive! I'm almost 5 days post op from VSG in Turkey. My first night was rough. I vomited foam and was nauseated. Gas pains were definitely present and my heating pad seemed to just make my nausea worse in my case. I was definitely having buyers remorse on day two...... but now my gas pains are gone and I'm walking a ton. They keep me until tomorrow morning. I was cleared for liquid diet and fluids yesterday after my 2nd leak test cleared and my drain was removed. So happy to be off my IVs and have my abdominal drain removed. I'm eating (though my hospital food is meh which makes it harder), getting my protein shakes down and getting about 34 Oz (1 liter) of water in on my second day allowed. I know i can meet my 2 liter goal very soon. I'm feeling very positive and cannot wait to get home to some well seasoned broth. -

I basically lived off beans & legumes during puree stage. The pureed meat thing just didn't work for me, so refried beans, lentil soups, etc. with added unflavored protein powder was my go-to food. -

I had been thinking the same. Mostly I'm just doing my thing, trying to figure out how to live with all the changes I'm experiencing. My challenges lately: I'm still not regular, with my guts bouncing between one extreme and another. I'd really like to find the right balance, but haven't found it quite yet. I'm struggling with water more now that I have to not drink 30 mins before and after a meal. If I take ~30 minutes to eat, than means I'm not drinking for and hour and a half per meal, or roughly 4.5 hours out of the day with 3 meals a day. Add in a snack to hit my protein goals (which I often have to do), and we're at 6 hours sans liquids. Some days I do okay, but I find once I get behind, it's almost impossible to catch back up. I'm still struggling to slow down my eating and chew better. I knew this would be a problem for me and it is. Thankfully it's not been too big an issue as my pouch seems to be surviving me not doing this quite as well as I'm supposed to. I'm trying to understand what both being hungry and being satisfied feels like with limited success. About all I can tell at this point is no pain and some pain. Some foods seem "different" than before. Because I'm never really actually hungry, I sometimes either don't eat at all, or don't eat very much, just because I don't like what I'm eating. I have the alternate issue sometimes as well. When I actually find I enjoy something, I find I can eat more than i thought and more than my team says I should be able to. (See previous bullets about trying to understand fullness). I'm not concerned, but on occasion I have found myself pushing the limits of this capability and paying the price afterward. Enough for now. What about everyone else? What's working or not for you right now? -
1-week post op; feeling all over the place

First of all, congratulations. In terms of your thread title, I would suggest that you feel like you're all over the place because you essentially are! This procedure is significant, and it results in significant changes to our bodies. I know that I felt very similar in the first weeks post-op. It is excellent that you're getting your fluids. Lots of people struggle with this at first (I did my first few days) and it sounds like you're keeping up. Likewise regarding your pain and moving around. These all sound like you're doing pretty darn good. Like you, I little-to-no taste or desire for sweet things post-op. In fact, I wanted more savory things, and they sat better. I drank a lot of broth in the first few days post-op. I finally settled on the Premier Protein Cafe Latte flavor (yes, it does have caffeine, and some providers/programs don't allow that, but it was the only one I could choke-down at first). To me, it seemed to be the least sweet of all the other ones I tasted. The program I am in has a reduced protein goal for the first month post-op, so it is a little easier to get close, but they told me to focus on the fluid first, protein second, doing the best you could. When I started taking the bariatric vitamins, my nutritionist recommended the Bariatric Fusion chewable ones. They made me sick the first day I took them. Day two of them was no better - epic nausea - so I stopped. I checked in with nutrition, and they suggested Bariatric Fusion one-a-day capsules. I started taking them at about the 3 week mark and they sat/sit fine. You may want to check with your provider to see if that is an okay choice for you. As time went on, I was able to tolerate different flavors of the Premier Protein without issue. As an example, I start the morning with a serving of the Chocolate powder mixed in 8 ounces of unsweetened almond milk. The almond milk seems to reduce the overall sweetness, at least to me. -

Glad it is going well. Doing okay. I seem to have hit a stall this week, but I also started doing workouts this week. Hoping I will bump over this hump. I have monkfruit sweetner, may have to give it a try. My biggest issue is water right now. No issues with the protein. Jennifer -

How is everyone doing? This group got really quiet and I miss new stuff to read. Do you guys go to different groups that do more talking? I just hit my 1 month yesterday and had my appointment with my dietitian and all is going really well. I was sad at 1st because I thought I wasn't hitting my protein goal every day and then he told me that the goal was lowered after surgery. So before surgery I was supposed to hit 75 to 90 but after surgery it is 60 to 80. So I have been hitting my protein every day yay! I also finally found something that helps with my constipation and that is monk fruit sweetener. My daughter got me into drinking tea in the morning instead of coffee and I added the monk fruit to make it sweeter since I wasn't using my protein shake as creamer like I was for my coffee. Apparently it acts as a laxative and now I get to have my tea and also don't have any stomach issues. Its soooo much better. All of my incisions are almost all healed. Just one is is taking its time. Pain wise I feel almost back to normal and my energy is doing great. I'm slowly increasing my exercise and I'm so proud of myself that I'm walking a mile every day. I know that's not a lot to most people but to me it was a huge 1st step. I've always hated exercise so even to be doing it every day is a good step for me but a mile was my 1st goal. Now to hit 2 miles a day Hope everyone is doing well and please come back and post so I have something to read lol! -

Mine was on the 11th too. I meet my protein goals by adding protein powder (and water) to my pre made protein shakes. I also drink premeir protein clear drinks instead of plain water. That was the only way to meet my protein goals. I tried adding protein to my soup but it made it taste so bad and have a weird unpleasant consistency. I'm glad you are doing well! Keep it going! Scared but excited. -
I found the standards I was given were quite strict and still reasonable. These are used Provence wide here and I found little deviation to different plans offered by other hospitals here in Ontario. My hospital is involved with The University of Toronto medical school as several hospitals are all involved together. A large group or team gives me confidence in their work and structure they offered me. I was also given the option to vary my plan slightly but I personally recommend being a bit more strict with yourself and allow your body to transition and heal. Time and your body mechanics will tell you when to progress. It is a long journey we are all in. I spent 30 years being fat and unhealthy and I will not loose the weight in fast succession. It will take lots of effort and time to make real permanent changes. I wish you all the best on your weight loss journey to better health. From Canada Cheers!

I just hit 50.5 pounds lost total, 36.4 since my bypass. I feel like my weight loss is slow but oh well. Nausea is finally waning and zero dumping! My taste buds have changed dramatically and I crave savory foods over sweet. My labs are great and my A1C is 5.1. It’s never been that low! Feeling pretty good!

I had mine on the 11th. Things went pretty darn well and I was sent home the next day. I think I had 3 of my pain pills (including in the hospital). I only took 1/2 pills... I do not need to plug myself up and I don't want an oxy addiction. I, too, have moments where I kind of forget that I had surgery, it's weird. I'm on full liquids right now and am able to meet my goal of 64oz a few days, but never my protein goal. I'm allowed to try some very thin puree, but haven't yet. I want to and I don't want to. Broth and water have been my friend. I made a HUGE batch of bone broth and have that at least twice a day. Sometimes I put protein in that, but my tummy can tell when I do. I put some spices in it today and then had kind of a freak out moment because - what if a spice gets stuck in a healing staple wound?! I get panicked sometimes about little things like that. I'm able to take a mouthful of water and break it down into about 4-5 swallows, which helps with my intake (sip, sip, sipping is hard). I've been moving and walking, but every day is a little different. Some days my energy levels are good, and others I'm a bit more tired. I had one poo like 3 or so days out, but it was very little, and nothing since even though I've taken stool softeners, magnesium, and magnesium powder. Today I tried some milk of magnesia in hopes that I will go tomorrow. That's my 1-week wrap up! LOL. Hope you are all doing well. And for those of you still on the docket for surgery - best wishes! -

I talked with my pharmacist and told her of my surgery and sleeping struggles. She recommended I can take melatonin . I found these that are 3mg. A recommended dose can be from 5 to 10 mg. Si day one I tried just one 3mg. I slept ok. But still struggled a bit next day I tried two 3mg. And was great. Just to see I tried three 3 mg the following day and wes lights out quickly. Awesome! So I found that for me 2 or 3 work great. I try to keep it at two but I have the option of three. So I would ask your pharmacist and see what they would recommend. I also if woke up at night have a small glass of warm milk. And nighty night. -

I'm glad I didn't get that option. I would have been scared that the pain would be unbearable. The pain hasn't really been that bad for me. I opted for no pain killers both in the hospital and at home. I hope that everything goes well for you. Positive thoughts for you! Scared but excited. -
I'm not sure if everyone does it, but they have an optional spinal injection to help with pain management post-op. It has been highly recommended and I intend to do it, but nervous about doing a shot in my spine / how much that'll hurt Unfortunately, I'm not that lucky. Mine is a nerve block shot in the spine while I'm awake. I have to be awake for it because I have to be hunched over a bit I guess while sitting up. Hence my dread lol. I'm glad to hear it helped with the pain though!

I think the hardest part about recovery for me was the first night. I had a lot of very intense gas pains in my chest and shoulder but after that night I only took pain medicine for a couple more days and then it was completely bearable. After that. I've had a C-section and that was definitely a worse and harder experience than this was. Just get your fluids in and your protein in and you'll be fine -

Bariatric surgery is a major step, so I don't blame you for being scared. I just wanted to reach out and validate your feelings. I was scared, too, which is why I chose a non-surgical option (Endoscopic Sleeve Gastroplasty). It is not yet covered by insurance so it is not as popular, but in my experience thus far and per the medical literature it is safe & effective. Maybe ESG is something to keep in mind for later, if you still need a "tool" in the form of a sleeve. But I would wait until your weight stabilizes after the meds, exercise, and new healthy eating habits are established. FWIW, I do not think your PCP is fat phobic. Obesity is a serious health risk, the older and the bigger you get. No one sees this more than a diligent PCP. Mine is 100% in favor of my new tool because she is the one who has been prescribing all my meds for hypertension and pre-diabetes, referring me to specialists, etc. I am just glad you went to see an endocrinologist and found a medical reason for your obesity. In case no one else has said it lately, great job!! Even with new meds, losing that much weight is an accomplishment. I agree with the other suggestions here re: strength training and all. post op, was there any issues with tolerating spices? not heat/capsaicin (that’s obvious to me as a no eat for a while, but not my favorite thing to eat anyway) but flavorful spices like cumin, paprika, bay leaves, dried herbs, fresh herbs, ginger, cinnamon etc etc etc that add bursts of flavor, especially savory, to things? did you have any issues with spices in particular? were you told to avoid black pepper? thank you!

I'm only 8 days post op so I haven't hit this yet, but my program says to try to keep it under 30 carbs on average. MD said he wants people to stay under 30, but also live life. You wants some cake at a bday party one day? Eat it. But the next day get back to protein/veggies. They also acknowledged that the carb count will be higher during the pureed/soft diet part because a lot that is more carb heavy. I just know carbs are going to be my weakness. I have 0 hunger since sx but I still think about carbs (warm yeast rolls?! Honey butter croissants?! Yessssss). If I'm thinking about it while not being hungry at all, then I know it will be a disaster if I start eating it.

I survived for the first 5 days post op on just water. I'm 8 days post op now. I packed water, sf jello, broth for immediately post op. I hated the jello after because of the sf taste (fine with it beforehand) and also hated the broth for no good reason. Finally had creamy soup yesterday and had gas pain/bloating. We will see how it goes. I did like the easy crunch ice (from Zaxbys/sonic). Medicine wise: I packed chloraseptic spray (throat sore post intubation for surgery) and gasX. My advice: prepare to be flexible with finding what you like and be PATIENT with yourself. I kept feeling like I should be able to drink more, get more protein in (starting day 6). Truth is that I'm no where near my goal for fluid or protein. I just can't take that much in. But I get a little more each day, and I'm happy with that for now.

We have the same surgery day. I have been stressing about fluids and protein (just started protein drinks back Sunday). I haven't come close to goal yet on either. Finally, I just decided that I was 8 days out from having like 80% of my stomach removed. It's just going take time. I have decided to just try to do a little bit more each day and see how my body feels instead of going by what's on the paper. I have gotten a little more each day, so I consider it a success. I just started back soups Sunday as well, and have only been able to take like 1-2 oz. Also got bloating and gas pain with it (may be the type of soup though). Going to try something different, but I think the amount is on par. I would use unflavored protein in the soup (even if only a little) to boost protein count. In terms of sweetness, I haven't had that issue yet, so I would have to defer to others. I am trying to do the Fairlife. I like the 26g protein better than the 42g protein, but need more protein. So I mix the 2 to get a flavor I like with more protein. I think just being flexible and listening to your body is good enough during this stage. I can't take multivitamins yet until I hit pureed/soft per my MD, so I will have to see how that goes. -
I tried some creamy blended soup, but I got bad gas pain and bloating from it. It was Panera broccoli and cheese (blended to make sure no solids and watered down a little). Only had about 2 oz. Going to try a different one and see if that helps. Sugar free Jello makes me break out. So I am surviving on protein shakes and water. Which isn't awful because I'm one of the ones that have 0 hunger post op. I feel like I could never eat again and be ok. Which is great in some ways but also means I feel full whenever I try to drink anything. I'm looking forward to pureed/soft because I am craving normalcy. Sitting at a table with the family and taking a bite of something. I am dreading the 3 week stall, but I am happy to be on the road to a more normal diet pattern.

You got this! Those first few days post op are rough emotionally and physically. Just be gentle with yourself and the process. I have to keep reminding myself that I am only 8 days post op from having like 80% of my stomach removed to keep it in perspective. Am I drinking enough fluids? No, but I'm only 8 days po and getting better each day. Am I drinking enough protein? No, but getting better each day. Just keep the perspective that each day something will be even a tiny but easier.
