Eating expectations vs reality

Hi, I had RNY on Tuesday, July 25th. My clinic advised against protein shakes afterwards but has me making my own liquid drinks with defined amounts of proteins, vegetables and added broths, etc. I’m not even able to eat half of what they told me I should try for. Is this normal? Does it just take time? -
Hunger Pang Hey guys I need so much help. From about my third or fourth day post op VSG I have had a battle everyday. #1 Before my surgery I have never truly experienced hunger pangs except for when I was younger. After I got older and even eaten less I did not deal with hunger pang. Since my surgery and I was able to eat purée, the first day I went to eat my food the pang/pain became so great my esophagus went into a spasm. I researched and asked my Dr’s office about this. They ask me if I’m drinking enough water, am I eating slow, am I drinking enough protein? To all their questions was ‘Yes’ I think so. Why am I’m dealing with hunger pain and it’s not head hunger because when I do eat my 4oz of food, it seem that I’m even more hungry. I drank more broth, but would be awaken at 3 or 4 AM starving. Once I was so wore out because of this I ate a little too much and I was in so discomfort I ended up falling asleep sitting to let the food digest. Has anyone experienced this? If so, what did you do?

Dollar Tree finds are so fun! I wish I liked salmon, mini packets would be handy for snacks! It may not be the best place for electrolyte waters though as stated above, check and see if they have sugar in them. I have some that don't but they were pretty pricey and not from the dollar store. Who knows though, you never know what brand you are going to find at Dollar Tree, sometimes you luck out! As for protein, it is the same thing, check the sugar, cheaper formulas often use sugar to sweeten and that will ruin your low carb efforts. Also check the protein source. If I recall correctly bariatric patients tend to do best on whey protein isolate or whey hydrolysate because they are gentler on the stomach and absorb better. Plus they should be lactose free, lactose intolerance is common after surgery as your stomach heals...

I feel you! Being in this situation is no fun. I don't have a surgery date yet, I didn't like the first surgeon I saw so I go see a new one on August 3rd. Hopefully one that will answer my questions and think a little outside the box. I'd talk it out with the surgeon (or his nurse/PA) a little more to understand his thought process. He sees more patients than you or I do, he should know what the risk of GERD is vs the risk of not absorbing your meds. I think I read about 30% of sleeve patients get new or worsened GERD/gastritis from the surgery?? That means there is a 70% chance you won't have that issue. For those that have it, it seems very frustrating, but I haven't seen any data on how many sleeve revisions are from GERD. I wish my meds (and autoimmune disease) were flexible enough to give me 3-6 months to figure out new dosing strategies, but they aren't. If I go into a med induced depression flare I will lose the benefit of the surgery because I won't be able to function well enough to make good food and get exercise. Not everyone has depression that severe though. I think a lot of patients have some flexibility there and can afford to play around with their meds to find a new combo. I will be fine with handling some tweaking to the dose, but I don't have the option of another med combo. Everyone is different though and it is worth talking it out with the doctor until you feel sure about your choice! -

My surgery date is August 9th, and I'm on day 3 of my pre-op diet. Less than 1000 calories, 2 protein shakes and two low carb snacks, and 1 low carb meal at supper. Days 10-13 will be liquid diet only. I've been able to lose 30 pounds before surgery with the help of injectables, increasing exercise, and modifying diet. Off of the injectables now and have had to limit exercise. I've been struggling with the feelings of hunger. -

I agree it depends on your program. I'd ask them. As far as the comment about carbs, that also depends on the program. Ours was a balanced plan so we didn't have to count or limit carbs - just protein (we were supposed to have 60+ grams of protein every day). -

Your team are your go to here because we all have very different surgeons with totally different ideas on what we should be eating at any given part of this surgery. No two plans are the same Once you are on real foods as to soft foods and your restriction kicks in, then you will have a better idea what your intake will be. Protein will fill you { BTW you are doing great with the amount of protein you can manage ] If your team has given you no guide lines then do not worry. I was told to do what I could. Protein first, Veg second and carb last if there is room. Your carb intake looks fine to me, its a tiny amount. Do not use any other persons plan to the minutest detail. They may have to restrict their carb intake but you may be free to taste them. -
Pre-Surgery Liquid Diet

The pre-op diet may have been the hardest thing during this entire process. It took me about 6 days before those hunger pains and desire to eat literally everything I saw to go away. My pre-op diet consisted of (2) protein shakes and (1) frozen meal under 300 calories and under 700mg of sodium each day. I was allowed to eat all the broccoli and cucumber I wanted though. It's tough, but stick with it. You'll need that discipline post-op. I lost 15 lbs on the pre-op diet. My RnY was on May 15th. I started losing weight on my own on Jan 18th in preparation for this and I'm proud to say I'm down 90 lbs! I started at 380 and weighed in today at 290! The difference in energy is amazing, not to mention I'm wearing clothes I haven't worn in many, many years. Even lacing up my work boots can be done sitting down. I'm sitting closer to the steering wheel in the car and stupid things like putting the seatbelt on are so much easier. It's also amazing how hard it is to meet the 60 grams of protein each day without relying on a shake. I've been released to introduce all foods. I've got what I refer to as good eating days and bad eating days. Today happens to be a bad day. Can't find any rhyme or reason to it either. But I'm celebrating many more wins than that! Stick with it - you got this! You'll be on that liquid diet for a few weeks out of the hospital too. -
I stopped at the Dollar Tree tonight after work and happened upon a couple of items I added to my surgery prep box. I found little packets of peanut butter powder, pouches of salmon, and a kitchen strainer. They also had some protein shakes and electrolyte water but I don’t know enough about either to buy any. I hadn’t really thought about that store as a resource for stuff I might need/want related to the surgery so I’ll definitely check back. And at $1.25, I’m not out much if I don’t like or need it.

GERD/Gastritis with Gastric Sleeve

That happened to me. I had the option for either surgery, so I chose the sleeve because I have MS and take several meds that can't be crushes, broken, and don't come in alternate forms. For the first 8 months, things were great. Then the GERD hit, which became gastritis and esophagitis, which was so bad it caused me to need 80mg daily of Nexium (and still had gerd break through), which caused polyps to develop throughout my stomach. 4 endoscopies later, I had no choice but to get the revision to bypass. Never been better. The PPI is down to 40mg per day and at 6 weeks post op we'll go down to 20mg daily and at 10 weeks we'll see how I do without any at all. The pain from the bypass was SO much less than the sleeve, the ability to get in my pills and fluids are SO SO much better. I wish I had just done it first. As for my MS meds, I am still on them, but since we know it's basically like taking 1/2 doses (that's the way it is for extended release, but not so much full release that requires the med to be taken more than once per day) I am having a MRI at 3 months post op (unless I have break through symptoms start before that) and if that's the case, I'll go on a different med (likely an IV infusion med that I would have to do 1x per week). There's ways around things with the bypass. Talk to your psych dr and see what can be done about your meds before completely writing off the bypass. I wish I had just skipped the sleeve, to be honest. -
Hey everyone! So I was scheduled for Gastric Bypass on 08/07/23, but after reviewing my medical history, my surgeon believes that Gastric sleeve would be a better option for me (due to Psychiatric medication I currently take and past history of addiction). I am a little worried because I heard the chances of GERD/gastritis are higher with the sleeve? I already suffer from occasional GERD and gastritis, but I agree with my doctor that the malabsorption of Gastric bypass probably isn't a good fit for me. Anyone get the sleeve and have issues with GERD/gastritis before? Did it improve or get worse? TIA 😊

Hi all. I had surgery July 13th. Some days I feel like my porch isn’t working. Day 12 post op was the first day that I was hungry but I have also been having menstrual bleeding since day 2 post op. I known I shouldn’t rely on the scale but this week I didn’t loose much of anything. I know everybody journey is different but I’ve seen where people loose 20 pounds and I’m no where near that. It’s a lil disheartening. I follow up with surgeon tomorrow. I am focusing on hitting close to my protein goal. Although getting the fluids in has been challenging. I am working at it. Any advice would be appreciated. pre surgery weight 262

Yes i am having the same issue i work overnight till 5am and then go to gym after work then sleep most of day to go back and do it again . but i know I'm not getting enough protein and definitely water in . It worries me .

I agree with @catwoman7 & @CarmenG. Every food is different so you can’t rely solely on a weight measure or only on a cup measure. When I reached maintenance, I began checking the recommended portion sizes of different foods and then from there I worked out what I could physically eat of that food. Also considered the calories too which also influenced how much I could eat. It’s not perfect but a good place to begin. I eat about a recommended portion size of most foods. For example I eat a little more of vegetables but only about a half a portion of rolled oats. As you likely know, it can differ day to day - some days I can eat my portion & then another I can’t. Have you checked your BMR? Again a good place to start to give you an idea of the calories your body needs based on age, heigh, weight, gender & activity levels. I’d also go back to tracking all your food for a while just to ensure you’re getting in the nutrients & calories you need. I don’t measure & weigh religiously anymore but do random checks to ensure I’m still on track & of course I check any new food I introduce. It can be easy to let things slide or over/under estimate. I was never given calorie goals by my surgeon or dietician only pretty general portion requirements. My surgeon told me to start with 1/4 - 1/3 cup from purée & slowly increase to about a cup by when I reached maintenance. He also drew a picture of a side plate & sectioned off two other circles: protein & vegetables (interestingly no carbs) & said that’s what a meal should look like for me in the future in maintenance. It’s funny that picture really stuck with me & I often think of it when I’m serving a meal or eating out. And using recommended portion sizes as my reference actually does mean my plate looks much like his drawing. -

This past Monday, my PA told me I should probably be consuming 1,000 calories a day. Ta loco! (He's crazy). I reminded him I'm only 35 days out of surgery. I said I was consuming about 600 cals a day now, 60g protein, and 41 carbs (because 21 of my carbs and 60 calories come from my chewable vitamins and fiber supplements). He said I should up my cals. I told him, I'd up them to 700, but that was it. I'm not saying the kid doesn't know his stuff, but this isn't my first bariatric surgery. We know our bodies better than our docs, PAs, and nutritionists. Unfortunately, we know our bodies because we learn through trial and error. That's how we figure out when too much it TOO MUCH, or what foods our particular stomachs will reject or be fine with. "Since grams can be used to measure both solid and liquid ingredients, both types of ingredients can also be measured in a cup. However, while liquid ingredients are always the same weight in grams, solid and dry ingredients are not. Remember that grams are a measurement of weight or mass." Solid food is more dense than liquid. You'll need less solid to fill you up. Liquids will go though faster and easier. Listen to your tummy. Grams only measure the weight of something, not how bulky or dense it is. Also, everyone's tolerance, hunger, and nutritional needs will vary from person to person. The medical teams can give us guidelines, but even if they've personally had bariatric surgery themselves, ever body is different. I don't think that 1 cup of food is too much at 5 years out. However, only your stomach can decide that. -

Same. I have to start drinking my water at about 7 or 8 AM, then I wait about 20 min to eat. After I eat, I usually wait 45 min to an hour before I start drinking water again. However, I'm trying to discipline myself to only eating my regular 3 meals and a snack per day. The reason for this is because I'm a teacher. I can't just be eating and drinking all day. I'm teaching/speaking/lecturing/working one-on-one with my kids for 49 min at a time from 8:15 AM to 4:00 PM. I get a lunch and a conference period somewhere in the middle of the day. My last water bottle is usually consumed after 9 PM. What I did at the beginning was set reminders on my phone for my water and my vitamins. After a while, though, it just becomes part of your regular day. Always have a water bottle in your hand. Have one where you sit to watch tv, have one on your desk, take one with you in the car (you can take sips at the red lights), carry one in your purse. I also invested in protein shots. Bariatricpal sells 3 oz shots of 25g of protein. I use them at the end of the day if I haven't consumed enough protein throughout the day. -
Not Allowed To Gain Weight

Yeah, I have to do all the standard things like 6 months of supervised weight loss visits, nutritionists, psych, etc... I just thought the weight clause was particularly cruel. I'll be taking @summerseeker advice and putting heavy things in my pockets. LOL I can't even tell what stage my cycle is in to make sure I go in with the period bloat (hysterectomy) so that's super annoying. @SleeveToBypass2023 I don't get an option about which insurance plan we have, this is through my partner's workplace. It costs too much to go with someone else. But I'm glad you didn't have to fight as hard to get your surgeries, BCBS was good when we had them! @New To This23 That sounds like a nightmare! I am so sorry you are going through that! I am worried about this scenario because my body is given to weird spurts of weight gain that have nothing to do with what I'm eating or how I'm exercising. I did 18 months with a nutritionist and lost 10 lbs, which was really due to the Ozempic I started. LOL I've never been able to lose more than 15-20 lbs, my body is just wicked stubborn. I will keep my fingers and toes crossed that you make that weigh in with room to spare!! Keep us posted! -
Not Allowed To Gain Weight

Have you tried Blue Cross Blue Shield? I didn't have to do the 6 month nutritionist visits and there was no clause about gaining weight. I saw my surgeon for the first time Feb 28th 2022 and I was approved and in the surgery room literally having my surgery on May 3rd 2022. I just had to meet their criteria which was bmi over 40, at least 1 comorbidity (I had 3), and have tried at least 2 other weight loss options. Then my blood work and ekg had to come back that I was healthy enough for the actual surgery. And that was it. -
Type 2 here… My last shot of insulin was in my pre-diet phase. My sugar dropped like a stone after. I actually ended up back in the hospital two weeks after surgery with starvation ketoacidosis. My doctor said I was following the post surgery diet to strictly(all protein) and I needed to add back in a few healthy carbs here and there to keep my insulin level steady. I think that shot you took was a mistake. Until it clears, you will need carbs to keep your sugar up. After that, monitor your levels and adjust diet as required. Of course, all of this should be checked by your doctor and his team.

Since you’ve started maybe just keep to one or two shakes a day & have a meal of portion & calorie controlled protein & vegetables/salads for the other meal/s. You’ve started to break some of your sugar & carb dependencies & it seems a waste to throw that away. A two week pre surgery diet is pretty common though some are only on a one or two diet. They can take the form of an all liquid (protein shakes) or two protein shakes & one meal of protein & vegetables or a specific diet of solid food (I was on keto). Just depends on your surgeon. Mine gives different patients different diets depending on several factors including starting weight, weight loss/gain history & medical status. All the best what ever you choose to do.

That’s always an interesting question @Nomorecookies. I always tried to eat (drink) a lot of the excess liquids first then eat the solids. Same with soup. (Actually still sort of do this but I think it is a habit now.) But I also look at it as the milk & the liquid stock in a chunky soup actually are a food too, containing protein & nutrients. I also took ages to eat cereal & soup in the first several months (may be a year 🤷🏻♀️) so it was a way of giving the liquids a little time to pass through my digestive system before starting on the solid components. But check with your team to see what they say.

I had surgery on Thursday, 7/20. I was there for pre-op at noon, went into surgery around 3pm, and was home by 9pm. I felt awful for the first four days, but things are starting to turn the corner. I’m getting down fluids and protein easier and even had butternut squash soup today. So good! I’m actually looking forward to purées.
