Can I go from a gastric bypass to a Sleeve?

"The patient is a 39 year-old woman who initially underwent laparoscopic Roux-en-Y gastric bypass in 2001. A revision was then performed in 2003 for suboptimal weight loss, where the gastric pouch was revised and a Silastic band was placed to the distal gastric pouch. She then underwent revision to a distal gastric bypass. She developed symptoms of reflux and also had weight recidivism. Endoscopy demonstrated stenosis of the distal gastric pouch at the location of the Silastic band. She was taken to the operating room for conversion sleeve gastrectomy, removal of the Silastic band, and hiatal hernia repair. " None of this applies to the OP. Again...No. To remedy severe complications experienced with other weight loss surgeries...it's a rare possible option. For simple lack of weight loss, it is accurate to say that there is no surgeon alive who would convert RNY to sleeve. So again, No. Just No. -

Hello, I guess this is my introduction to the forum, please point me in the right way if I'm in the wrong place. I'm a 46 Y/O guy, about to go in for a revision to my RNY that was done a while back (exact date when I remember...) My new date is this coming Tuesday, May 30th, 2017. My first surgery was somewhat successful, but I guess I got tired of the restrictions and requirements after a while, and I started cheating more and more in my diet. I dropped down to around 300 lbs after starting at 618, my heaviest. A few years ago I had an accident, where I was left in a lot of pain, and since I didn't have insurance I left it go untreated, and that was the beginning of my weight gain, to where I'm now, 504 lbs as of last Monday. Well, here I am, not as excited as the first time, and not scared either, more of a feeling of "whatever". Now, don't get me wrong, I will make it work, it's not as fun being fat and 46, like it was being fat and 30. Anyway, I'm an open book, ask away if you have any Q's, and I'm looking fwd to learning from ya'lls successes and mistakes, especially now that I found a forum dedicated to guy's issues, because my first time around, it was mostly ladies, with very few guys, and most of those guys were young. Pancake, thank you for the reply. How did the revision go and how are you liking your sleeve? I just got the call this past Friday that UHC approved me!! They just accepted my dr's notes from the past few years, so I didn't have to do 6 months supervision! I'm having my ultrasound and pulmonologist appointment on 6/17 and then I'll get my surgery date. So excited I can believe everything is falling into place finally!! -

I have UHC and I'm a railroad employee (BNSF) I had the lapband back in 2011 and they covered it 100% no deductable, no copay. Then in Jun 2012 insurance changed. I had a revision from band to sleeve in Jan 2014 and had to pay a deductable and copay ( it came to about $900.When I went for my lapband I just had to document my weight loss attempts over the years. UHC was very easy to work with on getting it covered. -

I will be having revision surgery in Oct. With Dr. Aceves. I wasn't working when I got the lapband three years ago, so this wasn't an issue. My co-workers know that I am having surgery, but not the details. I've been told that my surgeon will need to clear me to return to work with a letter. I don't want to reveal what type of surgery I had or explain why I had it in Mexico (I work at a US hospital!) Did any of you have this dilemma and how did you deal with it? Thanks. -

Hi all. 9 years ago I had a gastric sleeve done by Dr Ariel Ortiz in Tijuana. I have since stretched out my sleeve and am looking into a revision. Has anyone had a revision through Dr Ortiz? Was it successful? Cost? Or if you used another Dr, would you recommend them? Was it successful? Cost?

Hi Rhon, Thanks for the reply. =). Today I completed day 2 of the 5 Day Pouch Test. I will say after two days of liquids, I'm ready to chew almost anything!! The scale this AM said lost 2 lbs. Probably just water. But after today, I'm remembering how much self control I actually do still have and am hopeful. You are extremely brave to go through the revision. I'm scared to death that the new dr's will recommend that for me. Although after briefly talking to one of the best surgeons in my area, (who by the way,refused to follow any work done by my GBP surgeon), I'm now afraid that the new dr's will say I won't be eligible for any revision. I was told that the way my GBP was done makes it impossible for any revision. What the heck did that guy do to me? I guess I will find out when the hospital gets my records over to new dr's. I wish you all the best in health Rhon. Keep us posted. I love it that you're not giving up on you. Keep smiling!!! -
Lap Band 9/2007 by Dr Spivak in Houston: 20 lb weight loss immediately after band. Lost another 60 with running, boot camp, exercise and benefit of band as tool to eat less. Reached weight of 135 and felt great, but intermittent episodes of severe restriction, vomiting. Dr Spivak had moved out of the country and his website directed his patient to the physicians at TLC Surgery in Houston. Made an appointment with Dr Sherman Yu. Found out in July that my esophagus and pouch were dilated and i had reflux esophagitis (asymptomatic). All Fluid had to be removed from band and I returned to bad habits. Gained 50 lbs back and felt miserable. I was approved for band removal and revision to sleeve right away, but I chickened out and canceled my surgery in February thinking I could do it on my own. Wrong! i rescheduled and had my revision on 5/12/14 at Park Plaza Hospital. In the hospital, I received IV acetaminophen, dilaudid PCA and the On-Q painbuster ball. I walked 4 hrs after surgery and went home around noon the next day. The staff at PPH was amazing. Dr Yu gave me prescriptions for pain and nausea meds at my preop appt so it would be available when I got home from the hospital. He also is using new compounded lotions/creams for scar, nausea and pain which I am using as well. It is pod#4 today and feel great!! Of course, I have some incisional pain, but I really cannot complain at all. The last time I took pain meds was 2 days ago. I was sent home with the On-Q pain ball and removed it myself on pod#2. No nausea or vomiting, able to drink without problems. I drove myself to the movies yesterday and walked 2 miles today. I hope I'm not speaking too soon and jynxing myself, but I really don't think so. I just wanted to put a positive experience out there for those folks who have heard the horror stories. I don't know about revisions, but yes, vertical sleeve and gastric sleeve is the same. The procedure is called vertical gastric sleeve. -
Hello MsMook, welcome to VST!! I'm a revision patient as well. There's a lot to know, so it would be more helpful if you might give us a little background on your situation. Don't be shy - when it comes to the band there is bound to be someone here who has lived what you're going through, in all kinds of complications scenarios. Ask lots of questions... we are always willing to either lend an ear or help out.

Thx so much ladies. Well my story is not that far off from most. I gained most of my weight after I had my son in 2008. I am an asthmatic and have been put on steroids constantly which adds to my weight gain. I had lapband surgery done Feb 2010 and lost around 55lbs but gained most of it back. I have done tons of research on the sleeve and sleeve plication and now I am looking into having a revision.

Well today marks my week post surgery (revision surgery Nov. 14th) and I am glad to report that I am down 10lbs. I can also tell the difference between being just banded and also having plication as well. I hope to keep moving the the right direction but as of today I am very pleased. -
I just found out today that my band has slipped and I soon will have to make a decision what to do. You say you revised to bypass, did your insurance cover this? Can you provide me some details? I honestly want this thing out of me. I never expected this journey to be easy, but I am tired of vomitting daily and my teeth and body aching. My son told me today he wants it out too because he is tired of seeing sick all the time. Ugh, decisions...

It has been a while since I was posting regularly here. I had vsg in May 2013, reached my goal weight and a healthy BMI in about 16 months, and have had no trouble maintaining. I've been very happy with the sleeve overall. But I have had some trouble the whole time. Basically I dump (severe diarrhea and sometimes severe hypoglycemia too) after almost every meal. We are talking agonizing, clear-the-ladies-room, stuff. I actually experienced it right after surgery in the hospital and they said it was normal and would go away. It never has. I've worked with my surgeon, dietitians, other gastroenterologists, I've had all kinds of tests including numerous endoscopes and colonoscopies. I tried going dairy free, gluten free, low carb, low fat. I had my gall bladder removed after a severe attack and hoped it might help. I took Questran at 4x the normal dose. I tried calcium. I tried eating g things that tend to block other people up. I ate 120+ grams of low fat Protein a day. Nothing helped. Now I have another problem too. Despite being on nexium since surgery, I have severe acid reflux and bile reflux that have caused my stomach and upper bowel to develop severe ulcers. The acid reflux is a relatively common long term complication of the sleeve. The bike reflux is often genetic and doesn't cause a lot of trouble with a full size stomach apparently. My current gastroenterologist is going to revise my sleeve to a bypass, which will eliminate both types of reflux because it will be impossible for the acid and bile to reach the pouch from the blind stomach. He also thinks that the bile reflux is causing the dumping because it is impeding the break-down of food, which is passing through me only partially digested. So I will be having the revision Monday morning and I get to re-experience the joy of the 6 week post-op diet. My surgeon is confident that any weight I lose will only be temporary, since I really don't need to lose more. I may actually gain a bit because I will be absorbing more nutrients. I don't know how many sleeve to bypass revisions there are, so I wasn't sure where to post this. Hopefully it is helpful to someone.

Thanks Cheri_J for your story! I´ve only had to do one upper endoscopy but that´s plenty enough.... And no fighting the insurance either. I live in Europe so here you don´t fight the insurance companies...you fight the public surgery queues instead... I guess it all adds up in the end?! Did your surgery take much longer than normal RNYs? My surgeon has done quite many revisions and says they normally take about twice the time. And did you have any issues with the old band? My worry is that the band will not come out easily and that they will have to postpone the RNY part....would be just my luck. Glad to hear that you have had a good weight loss despite being a revission, thats another fear of mine that it will be less effective the second time around. I´ve gotten to a good start when it comes to pre-op diet and exercise. A bit tired of the monotone liquid diet but less than two weeks to go.. I´m confident that it´ll be worth it. -
GERD causing revision

I'm in the same boat, sort of. I was sleeved 2 years ago and immediately had severe GERD with the worst symptoms when my stomach was empty, so my weight loss was slow and I've regained the little I lost. I was supposed to be having revision to bypass in April, but that was postponed due to the pandemic. At this point, I'm just waiting until they can start scheduling surgeries again, hopefully by the end of June. My surgeon said my weight loss should be closer to 50-60 lbs, which I think I'd be happy with. I have regretted going for the sleeve since day one and am hoping that bypass alleviates some of GERD symptoms. -
Pouch reset diet won’t shrink your stomach. It will help you feel your restriction better. It's a kick start to get back on track with healthy habits. Yes, I’m on my soap box because this topic comes up so often. Your best restriction is in your first six months to a year. With the sleeve you feel less restriction as you progress *it’s not back to full size* this sensation seems common for sleeves but, won’t happen to everyone. I wish people knew this before choosing the sleeve. Just because we have more space does not mean you have to eat over your calories and macros and gain weight. You can eat more bulk in food and keep your calories in range. With the other foods allowed on your plan, Eat dense protein and as much veggies you want to fill the extra space. ***For anyone who feels they have stretched their surgery, get it diagnosed by a surgeon. Look into revisions. Address why you are over eating so you don’t stretch your next surgery***

I have thrown up for TWO years!!!!! After I revised to bypass, I am not throwing up anymore and there are others that had revised from band to other wls have stopped throwing up as well. It is not a mental thing, it is band induced. I couldn't eat dense Protein without it coming back up. I did not have any damage.

I had the band done 5 years ago and lost 80 lbs. In the beginning I always wanted for him to add fills and he wouldn't till I mastered it.I admire this. He takes the time to see you. I have been having problems the last year and I am getting the revision surgery to the sleeve on June 16.I found out he trained some other top band sleeve surgeons.
