  1. Hello, I'd like peoples advice on which surgery to go with. Here's. My stats. I'm 5'7, 295 lbs. My BMI is 46. I need to lose 150 lbs to be at 150. I'd like to get the sleeve but with 60-70% excess weight loss plus 10-15% regain, I'm looking at being around 220 lbs when this is all said and done. With bypass, I know several people who have lost 100% of their excess weight and had it easier keeping it off. The problem is they have bad vitamin deficiency even a decade later and can't even take some antidepressants. My question is to you is, how many of you got sleeved with a 46 BMI?     High fats

    Thanks for all your info people! Talked to my surgeon today who was one of the first who started the loop DS or SIPS and he recommended that procedure for best insulin stability long term and weight loss.. and he just uses the same procedure code as for regular DS so it's no problem.     Ok. So would you mind sharing how the food tolerance and hunger/cravings were at first and now? The thing is I don't see enough people who keep the weight off with the sleeve like they do with the bypass, and when I do see people 5+ year later even on YouTube, they all gained it back. It's like it was pointless even removing the hormones ghrelin if most people still can't control their cravings and regain the weight...both surgeries reduce the size of your stomach, but the sleeve appealed to me because they remove ghrelin with it....but its like it's only temporary. For a person like me who is constantly hungry and craving, that worries me that I'll be just as hungry again 1 year post op. And that's why I'd prob regain. Maybe I do need a surgery that'll hold me more accountable if I do cheat...I also am pre-diabetic, my blood sugar is starting to climb...tomorrow's finally my consult, and I'll see what the surgeon recommends.     I'm afraid I'll fail cus im always hungry, always eating the next meal every 3 hours and my cravings are out of control. What do you think I should do??     I'm just worried because most people I see on here who had the sleeve whicb I'd feel more safer getting, none of them were as heavy as me with as much to lose as me and thats why I want the sleeve but keep pulling back...I'd love to see a woman on here who had the sleeve with a 46 or higher BMI who now has. 20 or 25 bmi. That's all I'd like to see so that I don't go into this always paranoid it won't work enough cus im too big. That's all . My appt with my surgeons January 27th and ultimately he will leave it to me to decide but I just want real testimony from real people who were as big as me . I'm 5'7 293 lbs, and I want to be 150 lbs. If I follow the guidelines they tell you that sleeve will give you a 60-70% excess weight loss, that means I'd be 220.     First of all I just wanna say congratulations on your success! All of the points you gave on why you chose to go with the sleeve sounds a lot like me which is why I know my Dr. Is gonna push for the sleeve lol. And im less afraid to go with that too. I'm afraid of dumping syndrome, of meds and vitamins not being absorbed enough, or having my intestines rerouted. Etc. The only thing that bothers me is why the statistics say the sleevers lose 60-70% excess weight and bypass lose more. I'm asking from a completely objective standpoint - if both surgeries are just as easy to fail as they are to succeed, why do the research charts show this difference? Also, I wanted to ask, did you get to choose the size boujie you wanted for your sleeve or know what size Dr used for you?? Also, did you cravings and hunger completely return after 6 months or it does but slightly? Lastly, can you just do complete keto during the first year after surgery so you stay under 20 net carbs and in ketosis? Or our body doesn't need to be in fat burning mode because of the surgery?     Wait, you mean to tell me most people who start at 300 don't reach at BMI under 30??? 10-15% only??? Thats not what I'm seeing across these boards, both surgeries people losing most if not all their weight , including my friend who had bypass 10 years ago she was 450 lbs, still having trouble putting back on weight since hes only 70 lbs now, but on here and on YouTube I see most people reach their goal with the surgeries...to me its a matter of keeping it off , and I wanted the bypass because it's easier to lose 20-30 lbs more than ur goal weight so that when u regain a few, you stop at your goal weight. Due to the malabsoprtion qualities it's easier to lose passed your goal weight in that first year so I wanna take advantage of that first year but I can't get malabsorption due to the meds I'm not willing to stop forever Sent from my SM-N986U using BariatricPal mobile app

    So this pretty much sucks...I was informed that the gastric bypass for me is out of question since I'm on ADHD meds, specifically Vyvanse which is an extended release and there's no alternative that works as good as vyvanse so my only option now is the sleeve.     And im just saying in my experience and those I personally know, all started gaining back 2, 3 years after the surgery even when they ate healthy...like it started becoming a struggle for them to control hunger and cravings again Moreso for those who had sleeve done than those I know who had bypass, and 10 years later this girl I know who was 450 is at 80 lbs now from bypass and can't put weight back on 10 years later to her ideal weight at 120....shes had no problem keeping weight off and doesn't even try...but the sleevers I know all gained back a lot more than the expected 15% overtime Sent from my SM-N986U using BariatricPal mobile app

    Not really set on the bypass, I keep going back and forth between the 2 each day, I see all kinds of weight lost and weight regained with both but what gets to me is how it says with sleeve patients lose 60-70% and with bypass its 70-80% and I just keep looking at that 80% ...but for me most important is hunger control. I've read somewhere on here people who complained about when they had a sleeve and then got a bypass, they miss the hunger control they had with the sleeve that bypass didn't give them...but bypass gives you a smaller pouch than the sleeve so makes you full much faster....ugh decisions decisions...     Yeah I kinda initially also related tue RNY to death risks based on what I remember from the 90s...when it was still an open surgery... Does the ghrelin hormone also get reduced as with sleeve?     I am more leaning towards the bypass but I just wanted to see opposing views and see someone who had the sleeve maybe 3 or 4 years ago with my starting weight and see where they are now, I still have yet to. It's not even that the people I know didn't follow the diet, but overtime the stomach stretched and they said the appetite control is completely gone at that point, whereas with bypass I still see patients after 5 years more successful at keeping the weight off and their stomachs not as stretched as with sleeve due to smaller portions than with sleeve so that still keeps them eating fewer calories. But I'm not knocking the sleeve, I just wanted to be challenged in my perspective and the research I've done and see people who have been successful at it 5+ years cus I've seen plenty of bypass patients who have but not sleeve, if that makes sense Sent from my SM-N986U using BariatricPal mobile app

    For me to stay at my 150 loss goal I would need to lose 170-180 so when I regain I stay at 150 and I see bypass being more effective at that for my starting weight as opposed to sleeve I'd def regain more than where I need to be. Sent from my SM-N986U using BariatricPal mobile app

    You definitely will gain back 10-15% at the 18 mark, thats stated as a fact by all Dr's but from bypass it happens after 3 or 4 years and with bypass, from what I've researched, many lose much more with bypass and then when they gain back the 10-15% it's not over their goal weight, as opposed to sleeve, most lose less than 80% excess weight and then gain back 15% after 18 months and I just didn't wanna risk it. I know 3 people personally who had the sleeve, no GERD, and got resleeved or went to get a bypass all because they all gained 4o, 50 even 70 lbs back after 3, 4, 5 years and they dieted and did everything accordingly. I rather just get bypass the first time as it does have a more effective and higher rate of success. It's still the gold standard. But a part of me considered the sleeve mainly due to less complications, however I dont want the weight to creep back in so fast at 18 months that I didn't have enough time to lose all the weight.
