I have Emblem...I went for my Appointment in October 2014..completed all my requirements Cardiologist, Pulmonist, Endoscopy, Colonscopy,Nutritionist, Mental health, 4 Weigh In,letter of support from doctor...I received my approval last week it takes 4-5 days after your surgeon submits your documentation to Emblem. Also your requirements is based on your medical history.

My surgeon finally tells me at the last minute It's a good chance when I wake up from surgery he will not do the sleeve due to scar tissue from previous surgery. Are you kidding I refuse to put myself through pre-op, anthesia, more scars and NO WLS. He never mentioned Open surgery?..And of course he will still get paid from Ins. and I will be shit.out of luck.
