I know each state is different.. but I have fidelis and was approved only by my BMI I had no other co morbities . I was borderline high blood pressure but that's it..you can always call member services and ask..hope this helps

You can ask them to weigh you...I didn't own a scale back then ...lol...at my pre op app I weighed 304 and day of surgery I was 289..so I list 15 pounds on that evil diet...lol

I must say this is my year!!!! I had my life changing surgery and Saturday my boyfriend of 6 years proposed to me..So now I am engaged.. can't wait to see what the rest of the year brings..
