Hi, I could use help getting started...

Hi Everyone, I have been working on insurance requirements to get this ball rolling for about 6 months. I have a lot of it done, however I would love to see some sample physician documentation on weight loss program notes turned in to insurance company. I am 100 lbs overweight and have been overweight most of my life. I am so tired, I know you all know what I mean. Any help would be appreciated. :)

Hi--I just got banded on 6/23/08---I am about 140 lbs overweight--at time of banding---and had slightly elevated cholesteral---otherwise no comorbities. MY PCP just wrote a very short letter stating my age, height and weight. He said that I had tried a variety of diets "to no avail" (true--I have lost weight, just never kept it off). He also said that I suffer from elevated cholesteral (true), knee and back pain (not really---maybe ON OCCASION), and that I was at high risk for heart disease (true, especially bc my dad died of a heart attack at age 47.) He then just stated that he recommends me for the lap band.
