    Hi I am Karen. I am new to this sight and have been jumping hoops for about six months. I am having to do the 6 month weight loss appt. with my doctor. I would love to get an idea of what my notes should look like that my doctor sends to the insurance. Neither of us are clear on that. By the way, have you got a surgery date yet?

    Hi Everyone, I have been working on insurance requirements to get this ball rolling for about 6 months. I have a lot of it done, however I would love to see some sample physician documentation on weight loss program notes turned in to insurance company. I am 100 lbs overweight and have been overweight most of my life. I am so tired, I know you all know what I mean. Any help would be appreciated.
