    Hi Nik, I am also from the Chicago suburbs - Villa Park. This is my first post here. I am going through the pre-surgery testing and I hope to have my referral sent to my HMO of Illinois by the end of June. I am having my surgery done at Loyola University Hospital in Maywood. Where are you having yours done at?
