Meeting with PCP to start the process

Hi guys,

I called this morning and made an appointment to speak with my PCP about bariatric surgery. I've done my research, have a list of all of the things I've tried to lose weight in the past, and spoke with my Mom to discuss our family medical history. Is there anything I'm missing? What kind of questions should I ask her?

    Thank you! The appointment went very well and she agreed to write the referral to my insurance company. The insurance person in the office said we should know whether they approve it this week. ??????
