My insurance company sent me the weight loss policy. I was reading through it and it stated that I must complete 6 months of a physicians supervised diet and a 3 month pre-surgical multidisciplinary preparatory program as well?

Im confused, what is the difference between the two, they require exactly the same things except 1 is 6 months and the other is 3 months? Will I have to complete both of these programs? I included a screenshot of the policy.

The second paragraph on the first page makes it sound like you must complete one of them, not both. Give them a call to find out exactly what is required.

It states the 6 months or a 3 month pre-surgical multidisciplinary . The 3 month one seems to be the better one to do. That is what I have to do for the BCBS Fed. Do you have any co morbidities?
