Anyone else have to write a letter to Fed BCBS?

I just got a call from my surgeons office saying that I need to write a letter to Federal bcbs saying why I think I need the sleeve surgery. And what I have done over past year to lose weight... Anyone else have to do this? I am otherwise done with everything, just waiting on approval..

I did not have to write them a letter. My PCP had to send them a letter of necessity. After they received my packet I was approved in a week.

I have BCBS of tx and I had to write a letter. It was just a couple of paragraphs.
