My consultation is 6 weeks away! But the surgeons office is sending me paper work through the mail. I guess on medical history etc.

wonder why it's so far out? Do I have to complete anything during this wait for just the consultation?

If your program is like mine then all you need to do is fill out the paperwork. Mine took some time so don't put it off. Some of the answers required some phone calls and others required some thought.
