I have CIGNA and did the 4-month monitored diet. They didn't have any weight loss requirements and the rules were loosely defined.

During that time I lost 15 pounds and gained back 11. I was approved but my dr made me do a 2-week preop diet and I lost 31 pounds during that.

    Cigna didn't set specific weight loss requirements. I lost 10 lbs during the 4 month process. I got approved in less than a week.
