Have done all my pre-op requirements done and now all they are waiting on is the proof of my hypertension from my primary MD. All it is is a simple fax and thy still haven't done it and it's been a week! I called them last week and promised to send it today. Of course they didn't . It's stressing me out! I need I get my surgery in 2013 so I can use my flex spending account money or I will lose it! Grrr

keep on calling them until its done or if you can go pick it up from the office and fax it yourself. Hope everything works out

Sqeaky wheel gets the oil. Bug them. Do you know why it hasn't been faxed? For example are they waiting for the surgeon to write up something?

Or better yet, go by there.
